Abstract:Objective To evaluate the clinical efficacy of the basal anchoring with cavity compression closure technique in preventing postoperative dead space formation after resection of subcutaneous benign cystic-solid masses. Methods This study was a retrospective case series.A total of 42 consecutive patients with clinically and ultrasonographically diagnosed subcutaneous benign cystic-solid masses who met the inclusion criteria were treated with this technique between June 2024 and August 2025(all cases were postoperatively confirmed as benign). After complete mass excision and intradermal tension-reducing suture,a “U”-shaped non-absorbable anchoring suture was placed deep in the subcutaneous layer.A molded saline-moistened gauze bolster was placed over the incision, and the suture was tightened to generate vertical, uniform pressure, which was maintained for 2~3 days. The primary outcome measure was the success rate of dead space closure. Results Postoperative dead space closure was achieved in all 42 patients, with a success rate of 100%.No patient developed persistent seroma. The overall postoperative complication rate was 4.8%(2/42).Compared with conventional methods reported in the literature, this technique achieved superior outcomes in terms of seroma and overall complication rates. At the time of suture removal, the mean Vancouver Scar Scale(VSS)score was 2.8±1.5,and the mean Patient Scar Assessment Scale(PSAS)score was 18.5±7.2,indicating favorable early healing outcomes. Conclusion The basal anchoring with cavity compression closure technique is feasible, effectively ensures definitive dead space closure, and achieves reliable primary healing.Various strategies for seroma prevention described in the literature have inherent limitations, whereas this technique eliminates dead space at the source through active mechanical compression, demonstrating clear clinical advantages.Preliminary application suggests that it is both safe and effective.
