The Value of MRI DWI Combined with PWI in the Diagnosis and Grading of Intracranial Glioma |

Abstract Objective To investigate the application value of MRI diffusion weighted imaging(DWI)combined with perfusion weighted imaging(PWI)in the diagnosis and grading of intracranial gliomas. Methods The data of 55 patients with intracranial glioma admitted to our hospital were reviewed.All patients were pathologically confirmed as intracranial glioma,including 21 cases of high-grade glioma and 34 cases of low-grade glioma.All patients received conventional MRI,DWI and PWI,and compared the detection rate of different methods and DWI combined with PWI.ADC and CBV were obtained,and the ADC and CBV were compared between the tumor parenchymal region and the corresponding region of the healthy side.The rADC value and rCBV were obtained,and the ROC curve was used to analyze the diagnostic efficacy of different grades of glioma. Results The detection rate of intracranial glioma by different methods was statistically significant(P<0.05).The detection rate of DWI combined with PWI was higher than that of conventional MRI and DWI,and the detection rate of PWI was higher than that of conventional MRI(P<0.05).ADC and CBV in the tumor parenchyma area were higher than those in the corresponding region of the healthy side,and the difference was statistically significant(P<0.05).The rADC value of patients with high-grade glioma was lower than that of patients with low-grade glioma,and the rCBV value was higher than that of patients with low-grade glioma,and the difference was statistically significant(P<0.05).According to ROC curve analysis,the AUC values of rADC,rCBV and combined judgment of glioma grading were 0.841,0.822 and 0.916,respectively. Conclusions MRI? DWI and PWI can effectively detect intracranial glioma,and the combined examination of DWI and PWI can improve the diagnostic efficacy of intracranial glioma grading.
