腹腔镜卵巢囊肿剥除术对育龄期卵巢囊肿患者卵巢储备功能、生育能力及并发症的影响

Abstract Objective To investigate the effect of laparoscopic ovarian cyst removal(LOCR)on ovarian reserve function,fertility and complications in childbearing age ovarian cysts(OC). Methods 112 OC patients of childbearing age diagnosed were selected.Using simple random allocation,the patients were divided into groups A and B,with 56 patients in each group.Patients in group A were treated with LOCR and group B with conventional laparotomy.Observe the general situation of patients during the operation(operation time,intraoperative bleeding,intestinal exhaust time,etc.),after different time pain,compare the two groups of ovarian reserve function [serum follicle-stimulating hormone(FSH),estradiol(E2),luteinizing hormone(LH)],quality of sexual life and postoperative fever and wound bleeding complications. Results Group A took time(61.48±4.99)min,intraoperative bleeding(77.18±6.97)mL,first discharge time(12.55±2.14)h and postoperative time to ambulation(3.94±1.37)d.Compared with Group B,each group was significantly decreased(P<0.05).At 6h,12h,24h and 72h after surgery,the postoperative pain level in patients in Group A was significantly lower than that in patients in Group B(P<0.05).E2,LH and FSH decreased in both months compared with preoperative,and the change in group A compared with group B(P<0.05).After 3 months postoperative follow-up,29(51.79%)were pregnant in group A;28(50.00%),with no significant difference between the two groups(P>0.05).Group A(5.36%)had a lower complication rate than Group B(19.64%)(P<0.05). Conclusion LOCR has less damage,shorter recovery period,lower complication rate,less damage to ovarian reserve function,and less impact on patient fertility.
