Correlation between SFRP4 Levels in Peripheral Blood and Insulin Resistance and Obesity in Patients with Newly Diagnosed Type 2 Diabetes |

Abstract Objective To investigate the correlation between the expression level of secreted crimp-associated protein 4(SFRP4)in peripheral blood and insulin resistance(IR)and obesity in newly diagnosed type 2 diabetes mellitus(T2DM)patients. Methods Research methods: Fifty newly diagnosed T2DM patients were selected.According to body mass index(BMI), they were divided into non-obese subgroup(n=25, BMI<25kg/m2)and obese subgroup(n=25, BMI≥25kg/m2).At the same time, 40 healthy volunteers who underwent physical examination in the hospital during the same period were selected as the control group, including normal weight group(n=20, BMI<25kg/m2)and obese group(n=20,BMI≥25kg/m2).The concentration of SFRP4 in each group was determined by enzyme-linked immunosorbent assay(ELISA).Fasting blood glucose(FPG), glycated hemoglobin(HbA1c), triglyceride(TG), total cholesterol(TC), high density lipoprotein cholesterol(HDL-C), low density lipoprotein cholesterol(LDL-C), fasting insulin(Fins)and other metabolic indexes were detected.BMI, waist-hip ratio(WHR), insulin resistance index(HOMA-IR)and islet β cell function index(HOMA-β)were calculated to investigate the correlation between SFRP4 and IR and obesity degree. Results The plasma concentration of SFRP4 in T2DM was increased.SFRP4 was positively correlated with weight、BMI、FPG、HbA1c、TG、HDL-C、HOMA-IR(P<0.05),and negatively correlated with HOMA-β(P<0.HbA1c、05).Multiple stepwise regression analysis showed that HbA1c and BMI were independently associated with SFRP4,HbA1c and BMI were independently associated with PGRN. Conclusions It was found that the concentration of secreted curl associated protein 4(SFRP4)was significantly increased in newly diagnosed type 2 diabetes mellitus(T2DM)patients.Correlation analysis showed that SFRP4 was significantly positively correlated with body mass index(BMI), fasting blood glucose(FPG), glycosylated hemoglobin(HbA1c), triglyceride(TG), high-density lipoprotein cholesterol(HDL-C)and insulin resistance index(HOMA-IR)(P<0.05).There was a negative correlation with islet β cell function index(HOMA-β)(P<0.05).Further multivariate regression analysis confirmed that HbA1c and BMI were independent predictors of SFRP4 concentration.The study concluded that the abnormal increase of SFRP4 in plasma of T2DM patients was particularly prominent in the obese subgroup.In addition, the change of SFRP4 level is closely related to T2DM, insulin resistance and obesity status.This finding suggests that early intervention of SFRP4 expression levels may open up new research directions for the prevention and treatment of T2DM.
