Abstract:Objective To evaluate the predictive value of the systemic immune-inflammation index(SII)combined with body mass index(BMI)for early metabolic dysfunction-associated steatotic liver disease(MASLD)in adolescents. Methods A cross-sectional study was conducted using data from 1,227 adolescents in the 2017~2020 National Health and Nutrition Examination Survey(NHANES).Early MASLD was defined by a controlled attenuation parameter(CAP)248 dB/m.Multivariable logistic regression was used to identify risk factors for MASLD and construct a combined predictive model. The predictive performance was evaluated using the receiver operating characteristic(ROC)curve. Results Among the 1,227 participants, the prevalence of early MASLD was 26.8%(329/1,227).Multivariable regression analysis showed that BMI category was an independent risk factor for early MASLD,with obese adolescents having a 4.730-fold higher risk compared to those with normal weight(95% CI: 3.947-5.668,P<0.001).ROC analysis revealed that the area under the curve(AUC)for SII and BMI category alone was 0.610 and 0.819,respectively.The combined predictive model significantly improved the AUC to 0.827(95% CI: 0.800-0.855,P<0.001).Furthermore,a synergistic effect between SII and obesity was observed, with high SII status increasing the prevalence of MASLD by 14.0% in the overweight/obese group. Conclusion The combination of SII and BMI provides a robust and non-invasive tool for predicting early MASLD in adolescents.Given its cost-effectiveness and accessibility, this model can assist clinicians in identifying high-risk individuals for early intervention.
