Clinical Significance of Serum Immunoglobulin,Light Chain Quantification and Fructosamine in The Diagnosis of Multiple Myeloma

Abstract  Objective To investigate the clinical significance of serum immunoglobulin(Ig),light chain(LC)quantification and fructosamine(FMN)in the diagnosis of multiple myeloma(MM). Methods A total of 80 MM patients were selected as the MM group.At the same time,70 healthy persons who received physical examination in our hospital during the same period were selected as the control group.2-3mL fasting venous blood was collected from the two groups,FMN was measured by nitrotetrazolium blue colorimetry,and Ig and LC were measured by immune scattering turbidimetry.The Ig,FMN and LC were compared between the two groups,and the sensitivity and specificity of IgA,FMN and κ/λ for single and combined diagnosis of IgA MM were calculated. Results The IgA of patients with IgA type was higher than that of the IgG type,LC type and control group;the IgA of patients with IgG type and LC type was lower than that of the control group;the IgG of patients with IgG type was higher than that of the IgA type,LC type and control group;the IgG of patients with IgA type and LC type was lower than that of the control group;the FMN of patients with IgA type was higher than that of the IgG type,LC type and control group,and the differences were statistically significant(P<0.05).The κ/λ of the IgAκ,IgGκ and LCκ was higher than that of control group,while the κ/λ of the IgAλ,IgGλ and LCλ was lower than that of control group,the differences were statistically significant(P<0.05)The diagnostic specificity of IgA,κ/λ and IgA+κ/λ was higher than that of FMN and IgA+FMN+κ/λ,and the differences were statistically significant(P<0.05).The diagnostic sensitivity of FMN,κ/λ,IgA+κ/λ and IgA+FMN+κ/λ was higher than IgA,but there was no statistical significance(P>0.05). Conclusion FMN has high sensitivity in the diagnosis of IgA MM.The combination of Ig and LC quantitative detection can reduce the rate of misdiagnosis and missed diagnosis and provide guidance for clinical diagnosis and treatment.
