Abstract Objective To construct a scientific and practical home nursing model based on Family Doctor service for Elderly Type 2 diabetes patients in Community. Methods Based on literature review,investigation,semi-structured interview and expert meeting,the framework and specific content of home nursing model of elderly type 2 diabetic patients based on family doctor service were constructed. Results The expert judgment coefficient was 0.910,the degree of expert familiarity was 0.820,and the degree of expert authority was 0.865.The home nursing model contained Objective and purposes,service team,and service processes and content. Conclusion The home nursing model is logical,reliable and comprehensive,which can be used as reference for development of home care for elderly patients with type 2 diabetes in community.
