视神经脊髓炎患者内分泌代谢紊乱与病情的相关性

Abstract Objective To observe the endocrine-related indicators and endocrine disorders in patients with optic neuromyelitis in different conditions,and to analyze the correlation between endocrine and metabolic disorders and optic neuromyelitis patients. Methods The clinical data of 40 patients with acute treatment of optic neuromyelitis were retrospectively analyzed and included them in the observation group.The clinical data of 40 patients with remission of optic neuromyelitis diagnosed and treated in our hospital during the same period were collected.They were included in the control group.The levels of total thyroxine（TT4）and adrenocorticotropic hormone（ACTH）in the two groups were compared.The effects of TT4 and ACTH levels on the condition of neuromyelitis patients were analyzed and explored the predictive value of TT4 and ACTH levels in the patients with neuromyelitis. Results The level of TT4 in the observation group was lower than that in the control group,and the level of ACTH was higher than that in the control group.The difference was statistically significant（P<0.05）.Logistic regression analysis showed that low expression of endocrine hormone TT4 and overexpression of ACTH were the influencing factors of patients’ condition with optic neuromyelitis（OR>1,P<0.05）;by drawing ROC curve found that the single detection of endocrine hormones TT4 and ACTH levels for the prediction of optic neuromyelitis disease AUC were 0.774 and 0.860,respectively,all have certain predictive value. Conclusion Most patients with optic neuromyelitis are associated with endocrine and metabolic disorders,which may be related to the aggravation of the disease during treatment.It is recommended that the level of endocrine indicators of patients with optic neuromyelitis can be monitored during the treatment to guide the prediction of the risk of aggravation of the patient and make active interventions.
