Effects of Transumbilical Single-port Laparoscopic Surgery on Microinflammatory Indexes and Stress Response in Children with Appendicitis after Operation

Abstract:Objective To explore the effects of transumbilical single-port laparoscopic surgery on microinflammatory indexes and stress response in children with appendicitis after surgery.Methods A total of 80 children with appendicitis were selected and divided into two groups with 40 cases in each group according to random number table method.The control group was treated with conventional three-hole laparoscopic appendectomy(LA),and the observation group was treated with transumbilical single-hole LA.Surgery-related indicators,microinflammatory indicators [C-reactive protein(CRP),interleukin-6(IL-6),tumor necrosis factor-alpha(TNF-alpha)],and stress responses [norepinephrine(NE),cortisol(Cor)] were compared between the two groups.Results There was no significant difference in operation time between the two groups(P>0.05).The blood loss in the observation group was less and the anal exhaust time was shorter than that in the control group(P<0.05).1 day after surgery,CRP,TNF-alpha and IL-6 in both groups were reduced compared with those before surgery,and the observation group was lower than the control group(P<0.05).One day after surgery,Cor and NE were higher in both groups,but lower in observation group than in control group(P<0.05).Conclusion Compared with conventional three-hole LA,transumbilical single-hole LA can reduce the amount of blood loss in children with appendicitis,shorten the time of anal exhaust,and reduce the microinflammatory reaction and stress response.
