Abstract:Objective To investigate the long-term efficacy of adjuvant chemotherapy after surgery in patients with stage IB-IIA cervical cancer and potential risk factors for recurrence, and to analyze the prognostic factors. Methods A retrospective analysis was conducted on the data of 162 patients with stage IB-IIA cervical cancer and potential risk factors for recurrence admitted to our hospital from January 2019 to January 2022.All patients underwent radical hysterectomy.Patients were divided into a chemotherapy group(n=89) and a control group(n=73) based on whether they received adjuvant chemotherapy after surgery. The chemotherapy group received adjuvant chemotherapy with paclitaxel combined with cisplatin, while the control group received only routine follow-up.A 3-year follow-up was conducted, and the progression-free survival(PFS) rate and overall survival(OS) rate of the two groups were recorded in detail.Logistic regression analysis was used to explore the relevant factors affecting the long-term prognosis of the patients. Results After 3 years of follow-up,the PFS rate in the chemotherapy group was 84.27%(75/89),and the OS rate was 87.60%(78/89),while the PFS rate in the control group was 63.01%(46/73),and the OS rate was 75.34%(55/73).The 3-year PFS and OS rates in the chemotherapy group were significantly higher than those in the control group(P<0.05).Poorly differentiated tumors, lymph node metastasis, vascular invasion, and tumor diameter≥4cm were risk factors affecting the 3-year PFS and OS rates(P<0.05),while adjuvant chemotherapy after surgery was a protective factor. Conclusion For patients with stage IB-IIA cervical cancer and recurrence risk factors, adjuvant chemotherapy after surgery can significantly improve their long-term PFS and OS rates and improve prognosis.Furthermore, poor tumor differentiation, lymph node metastasis, vascular invasion, and tumor diameter affect the long-term prognosis of patients.
