Clinical Effect of Emergency Laparoscopic Cholecystectomy Combined with Common Bile Duct Exploration and Lithotomy T-tube Drainage in the Treatment of Acute Calculous Cholangitis

Abstract：Objective To investigate the clinical effect of emergency laparoscopic cholecystectomy combined with common bile duct exploration and T-tube drainage in the treatment of acute calculous cholangitis. Methods 48 patients with acute calculous cholangitis were selected as the study objects,and were randomly divided into control group and observation group according to random number table method,with 24 cases in each group.The control group was treated with common bile duct exploration and lithotomy and traditional open surgery,while the observation group was treated with laparoscopic cholecystectomy combined with common bile duct exploration and lithotomy.The operation status and recovery status(intraoperative blood loss,operation time,hospital stay,hospital cost,abdominal drainage time,intestinal function recovery time),incidence of complications(incision infection,postoperative bleeding,stone residue,bile leakage),serum inflammatory factors(TNF-α,IL-6),renin-angiotensin system(RAS)activity index levels(A ngⅠ and AngⅡ). Results There was no significant difference in operation time between the two groups(P>0.05).The amount of blood loss,hospitalization cost,hospitalization time,abdominal drainage time and intestinal function recovery time in observation group were significantly lower than those in control group(P<0.05).The complication rate of observation group was significantly lower than that of control group(P<0.05).After treatment,the levels of TNF-α and IL-6 in two groups were higher than before treatment,but the levels of TNF-α and IL-6 in observation group were lower than those in control group(P<0.05).After treatment,AngⅠ and AngⅡ levels in two groups were higher than before treatment,but AngⅠ and AngⅡ levels in observation group were lower than control group(P<0.05). Conclusion For patients with acute calculous cholangitis,emergency laparoscopic cholecystectomy combined with common bile duct exploration and lithotomy T-tube drainage can effectively improve surgical indicators and promote physical recovery of patients,which is worthy of clinical reference.
