Abstract:Objective To investigate the association between the triglyceride-glucose index and sarcopenia in patients with chronic heart failure, and to evaluate its screening value for sarcopenia risk. Methods A total of 150 patients with chronic heart failure who were treated at our hospital between December 2021 and December 2025 were enrolled.Based on diagnostic criteria for sarcopenia, they were divided into a sarcopenia group(38 cases)and a non-sarcopenia group(112 cases).Baseline data, laboratory parameters, and echocardiographic measurements were collected, and the TyG index was calculated.Multivariate logistic regression was used to identify independent risk factors for sarcopenia,Pearson correlation analysis was employed to assess the relationship between the TyG index and other indicators, and the trends in sarcopenia detection rates were analyzed by stratifying patients into four quartiles based on the TyG index. Results In the sarcopenia group, fasting blood glucose, triglycerides, and the TyG index were all higher than in the non-sarcopenia group(P<0.05).Multivariate logistic regression analysis revealed that the TyG index(OR=4.159,95% CI:3.636~4.682,P=0.003),NYHA functional class(OR=3.323,95% CI:3.067~3.579,P=0.025),left ventricular end-diastolic diameter(OR=3.387,95% CI:3.151~3.623,P=0.014)were all independent risk factors for sarcopenia.Pearson correlation analysis showed that the TyG index was positively correlated with left atrial diameter, left ventricular end-diastolic diameter, interventricular septal thickness, and left ventricular end-systolic diameter(r=0.159~0.231,P<0.05),and negatively correlated with left ventricular ejection fraction(r=-0.187,P=0.022).After stratification by TyG index quartiles, the prevalence of sarcopenia gradually increased from 20.00% in the Q1 group to 51.35% in the Q4 group(χ2=85.486,P<0.001). Conclusion The TyG index is an independent risk factor for sarcopenia in patients with chronic heart failure. It is significantly associated with cardiac chamber enlargement and impaired cardiac function, and the risk of sarcopenia increases progressively with rising TyG index values.Given its simplicity and low cost, the TyG index may serve as a potential biomarker for screening the risk of sarcopenia in patients with chronic heart failure.
