Clinical Characteristics,Drug Resistance of Isolated Strains and Serotype Analysis of Invasive Pneumococcal Disease in Some Children in Zhengzhou Areas |

Abstract Objective To investigate the clinical characteristics,drug resistance and serotypes of invasive pneumococcal disease(IPD)in children in our hospital.Methods The clinical data of 340 children diagnosed with IPD were collected,and their clinical characteristics,drug resistance of isolated strains and serotype distributionwere analyzed.Results Among the 340 children patients,there were 172 males(50.59%)and 168 females(49.41%),with a male-to-female ratio of 1.02:1 and with age from 5 months old to 14 years old.There were 151 cases(44.41%)<2 years old,135 cases(39.71%)of 2~5 years old and 54 cases(15.88%)of 5~14 years old.The high incidence season of IPD was in winter and spring,222 cases(65.29%)in total.Sepsis-induced pneumonia was the most common type of infection[189 cases(55.59%)],followed by 73 cases(21.47%)of non-infected bloodstream infections and 55 cases(16.18%)of meningitis.44 cases(12.94%)of children patients were with underlying diseases and 158 cases(46.47%)were with risk factors for drug resistance.The SP strains isolated from the children patients had a resistance rate of 0 to Vancomycin and Levofloxacin,and the resistance rates of 23.03% and 10.09% to Cefotaxime and Amoxicillin respectively,and had the resistance rates of up to 90.94%,90.04% and 88.41% to Erythromycin,Clindamycin,and Tetracycline.SP common serotypes were 19F(25.00%),19A(17.35%),1(15.29%),6B(13.82%),and 23F(10.59%).There were no statistically significant differences in the coverage rates of PCV7,PCV11 and PCV13 among children from 5 months to 14 years old(P=0.246,0.221,0.606),and there were no statistically significant differences in the coverage rates of the three vaccines(χ2=0.262,P>0.05).Conclusion IPD is prevalent in children with age<2 years old.Sepsis-induced pneumonia is the most common type of SP infections.IPD is more likely to occur in children with underlyingdiseases and a certain risk of drug resistance.The drug resistance rate of SP strains is high in Erythromycin,Clindamycin,and Tetracycline.PCV13 vaccine can cover more IPD serotypes.
