1 The Impact of Non-invasive Ventilator Combined with Antibiotic Step-down Therapy on Respiratory Function in Emergency Severe Pneumonia Patients with Respiratory Failure

Objective To observe the clinical therapeutic value of using a ventilator combined with antibiotics to lower the ladder in the treatment of emergency severe pneumonia patients with respiratory failure. Method 80 patients diagnosed with emergency severe pneumonia complicated with respiratory failure were selected.The patients were randomLy divided into a control group and a study group,with 40 patients in each group,using a random number table method.The control group was given antibiotics to reduce the risk of respiratory failure,while the study group received non-invasive ventilators on top of the control group.The respiratory function of both groups of patients was analyzed,including dynamic lung compliance(Cdyn),work of breathing(WOB),oxygenation index(PaO2/FiO2),heart rhythm,respiratory rate,and cardiac function,including left ventricular end diastolic diameter(LVEDD),left ventricular ejection fraction(LVEF),inflammatory factors such as C-reactive protein(CRP),tumor necrosis factor(tumor necrosis factor).Necrosis factor(TNF-α),total adverse reaction rate,and overall efficacy rate. Result After treatment,Cdyn,WOB,PaO2/FiO2,and LVEF in both groups of patients were higher than before treatment(P<0.05);Heart rate,respiratory rate,LVEDD,CRP,and TNF -αwere lower than before treatment(P<0.05);The total adverse reaction rate of the research group was relatively low,and the total effective rate was relatively high(P<0.05). Conclusion The combination of non-invasive ventilator and antibiotic ladder descent can effectively improve the respiratory function of emergency severe pneumonia patients with respiratory failure,and the overall effective rate is high,which is worthy of clinical promotion.

4 The Effect of Lenvatinib Combined with Xindilimab on Tumor Marker Levels and Survival in Patients with Advanced Liver Cancer

Objective To investigate the effects of lenvatinib combined with sintilimab on tumor marker levels and survival in patients with advanced liver cancer. Method A retrospective collection of data was conducted on 96 patients with advanced liver cancer.They were divided into a control group(48 cases)and a study group(48 cases)according to different treatment plans;The control group was treated with lenvatinib,while the study group was treated with lenvatinib combined with sildenafil monoclonal antibody for at least 3 cycles;Compare and analyze the treatment efficacy,tumor marker levels,liver function indicators,quality of life,adverse reactions,and survival status between the two groups.The disease control rate of the research group(70.83%)was higher than that of the control group(41.67%)(P<0.05);The research group had lower levels of alpha fetoprotein(AFP),carcinoembryonic antigen(CEA),abnormal prothrombin(PIVKA II),carbohydrate antigen(CA199),total bilirubin(TBIL),aspartate aminotransferase(AST),and alanine aminotransferase(ALT)compared to the control group after three cycles of treatment;The study group had higher scores in all dimensions of the EORTC QLQ-C30 quality of life assessment scale for cancer patients treated for three cycles compared to the control group(P<0.05);There was no statistically significant difference in adverse reactions between the study group(14.58%)and the control group(10.42%)(P>0.05);The survival rate of the study group was higher than that of the control group,and the median survival time was longer than that of group A(P<0.05). Conclusion The combination of lenvatinib and xindilimab is effective in the treatment of advanced liver cancer patients,reducing tumor marker levels,alleviating liver function damage,improving quality of life,and increasing survival rate,with good safety.

8 Application Value Analysis of Double Cavity Bronchial Intubation Based on Visual Laryngoscope Combined with Fiberoptic Bronchoscope in Anesthesia of Thoracic Surgery

Objective To explore the clinical value of using visual laryngoscope combined with fiberoptic bronchoscope(FOB)to perform dual lumen bronchial intubation(DLT)in thoracic surgery anesthesia. Methods A total of 76 patients with thoracic surgery were randomly divided into control group(38 cases)and observation group(38 cases).The control group was treated with traditional DLT,while the observation group was treated with visual laryngoscope and FOB assisted DLT.Hemodynamic indexes,intubation positioning time,glottic exposure time,one-time intubation success rate and complications such as laryngeal edema,oral injury,vocal cord injury and gum injury were compared between the two groups. Results Before intubation,the hemodynamic levels of the two groups were compared(P>0.05).Immediately after intubation and 3 minutes after intubation,the heart rate,systolic blood pressure and diastolic blood pressure of observation group were lower than those of control group(P<0.05).Compared with the control group,the intubation positioning time and glottic exposure time in the observation group were significantly shorter,the success rate of one-time intubation was higher,and the incidence of intubation complications was significantly lower(P<0.05). Conclusion In the anesthesia of thoracic surgery,DLT assisted by visual laryngoscope combined with FOB has a good effect,which can greatly improve the success rate of one-time intubation,significantly shorten the time of intubation,and reduce the incidence of complications of intubation,which can be clinically popularized.

11 Comparison of the Application of Different Induction Doses of Remimazolam Tosilate in the Induction of General Anesthesia for Patients with Acute Cholecystitis undergoing Laparoscopic Surgery

Objective To investigate the efficacy and safety of different induction doses of remimazolam tosilate for general anesthesia in patients with acute cholecystitis undergoing laparoscopic surgery. Methods A total of 93 patients scheduled for laparoscopic surgery were randomly allocated into three groups(R1,R2,and R3),each comprising 31 patients.Anesthesia was induced with intravenous remimazolam tosilate at doses of 0.20 mg/kg,0.25 mg/kg,and 0.30 mg/kg over 30 seconds,respectively,followed by sufentanil 0.4 μg/kg and rocuronium 0.6 mg/kg for endotracheal intubation.Anesthesia maintenance was achieved using a continuous infusion of remimazolam tosilate at 0.5 mg/(kg·h),remifentanil at 6-12 μg/(kg·h),and sevoflurane at 1%.The effectiveness of sedation,hemodynamic parameters,stress response indices,anesthetic drug consumption,patient recovery status,and incidence of adverse events during anesthesia induction were recorded and compared among the groups. Results The time to loss of eyelash reflex and onset of sedation were significantly shorter in group R3 compared to groups R1 and R2,and in group R2 compared to group R1(P<0.05).There were no significant differences in heart rate(HR)and mean arterial pressure(MAP)before anesthesia(T0),at tracheal intubation(T1),5 minutes post-induction(T2),and at the end of surgery(T3)across the groups(P>0.05).Serum concentrations of norepinephrine(NE)and cortisol(Cor)were lower in group R3 than in groups R1 and R2 at the end of surgery and 6 hours postoperatively(P<0.05).Additionally,the intraoperative doses of sufentanil and remifentanil were significantly lower in group R3 compared to groups R1 and R2(P<0.05).No significant differences were observed in recovery times or overall incidence of adverse reactions between the groups(P>0.05). Conclusion Both 0.25 mg/kg and 0.30 mg/kg remimazolam tosilate are safe and effective for inducing general anesthesia in patients with acute cholecystitis undergoing laparoscopic surgery.However,the 0.30 mg/kg dose offers advantages by shortening the onset time of sedation,reducing intraoperative anesthetic requirements,and mitigating the stress response,making it a preferable option.

15 Comparison of the Clinical Effects of Transurethral Prostatic Hollium Laser Excision and Transurethral Prostectomy in Patients with Benign Prostatic Hyperplasia and Its Impact on Prostate Function and Sexual Function

Objective To observe the clinical effects of holmium laser enucleation of the prostate(HoLEP)and transurethral resection of the prostate(TURP)in the treatment of benign prostatic hyperplasia(BPH)patients and their impacts on prostate function and sexual function. Methods The control group(n=34)was treated with TURP,while the observation group(n=34)was treated with HoLEP.The surgical-related indicators,prostate symptoms(IPSS,QOL),and sexual function(IIEF-5,EHGS)scores,as well as the occurrence risks of complications(secondary bleeding,urinary incontinence,urinary tract infection,and bladder irritation symptoms)were compared between the two groups. Results Compared with the control group,the observation group had better surgical indicators(t=-11.903,P=0.000;t=-11.801,P=0.000;t=-16.369,P=0.000).After treatment,the IPSS and QOL scores of all patients decreased(P<0.05),and the observation group was superior to the control group(t=-14.349,P=0.000;t=-7.969,P=0.000);the IIEF-5 and EHGS scores increased(P<0.05),and the observation group was superior to the control group(t=9.098,P=0.000;t=8.563,P=0.000).The postoperative complication rates of the two groups were 5.88% and 26.47%,respectively,and the observation group was lower(χ2=5.314,P=0.021). Conclusion HoLEP has a good clinical effect in the treatment of BPH,can effectively reduce intraoperative blood loss,shorten recovery time,improve prostate symptoms and sexual function of patients,and has high safety,which is worthy of promotion.

18 Clinical Effects of Lower Limb Muscle Chain Training and Quadriceps Femoris Training in the Rehabilitation Treatment of Knee Osteoarthritis

Objective To explore the important role of active training of lower limb muscle chain and quadriceps muscle according to the diagnosed cases of knee osteoarthritis. Methods KOA patients were selected and randomly divided into two groups,with 50 cases in each group.The control group used tkwse to train the quadriceps muscle,and the observation group used the lower limb muscle chain training program. Results After treatment,the Lysholm of the study group was higher(P<0.05),the WOMAC score was lower than that of the control group(P<0.05),and the peak moment(PT value)and total work done(TW value)of the study group were significantly better than those of the control group(P<0.05). Conclusion In the clinical treatment of KOA patients,the effect of limb muscle chain training is prominent in carrying out effective rehabilitation treatment,which can significantly enhance the function of the knee joint,accelerate the recovery process of knee flexor and extensor muscle group,and improve the performance of the knee joint.In contrast,lower limb muscle chain training can significantly improve the condition of KOA.

21 Study on the Relationship between Preoperative Immune Inflammatory Index and Postoperative Recurrence of Adenomyosis

Objective To explore the relationship between preoperative immune inflammatory index(SII)and postoperative recurrence of high intensity focused ultrasound(HIFU)in patients with adenomyosis. Methods 120 patients with adenomyosis who underwent physical examination were randomLy selected and divided into relapse group(21 cases)and non-relapse group(99 cases)according to the recurrence situation.Kaplan-Meier method was used to draw the recurrence curve.The clinical data of recurrent group and non-recurrent group within 1 year after operation were compared.The risk factors of recurrence within one year after operation were analyzed by binary logistic regression.Pearson and Kendall analyzed the relationship between preoperative SII and independent influencing factors of recurrence within one year after operation. Results All the 120 patients were followed up,and 21 cases recurred within one year after operation,with a recurrence rate of 17.50%.The comparison of demographic data showed that there were significant differences in age,uterine volume,uterine cavity operation number,lesion type,ablation rate and preoperative SII between recurrent group and non-recurrent group(P<0.05).Multivariate logistic regression analysis showed that age≥35,preoperative uterine volume≥200,preoperative focus type,high preoperative SII and high preoperative uterine cavity operation were independent risk factors for recurrence within one year after operation(OR=27.424,11.976,39.553,1.110,11.357,P<0.05).Pearson and Kendall analysis showed that preoperative SII was positively correlated with age,preoperative uterine volume and ablation rate(P<0.05).However,there was no correlation with preoperative lesion type and preoperative uterine cavity operation(P>0.05). Conclusion Multivariate logistic regression analysis shows that age ≥35 years old,preoperative uterine volume≥200 cm,preoperative focus type,high preoperative SII and high preoperative uterine cavity operation number are independent risk factors affecting postoperative recurrence,while high ablation rate is a protective factor,and preoperative SII is positively correlated with age,preoperative uterine volume and ablation rate,so preoperative SII can be used as one of the important indexes to predict postoperative recurrence of adenomyosis after HIFU.

26 Efficacy of Silver Ion Gel Combined with Hyperbaric Oxygen Therapy in Patients with Diabetic Foot Ulcers

Objective To investigate the efficacy of silver ion gel combined with hyperbaric oxygen therapy in patients with diabetic foot ulcers. Methods A total of 48 patients who received hyperbaric oxygen therapy were included as the control group,while another 47 patients treated with silver ion gel in addition to the same treatment served as the observation group.The control group(n=48)underwent conventional treatment and local hyperbaric oxygen therapy,whereas the observation group(n=47)received additional silver ion gel application.The efficacy,lower limb microcirculation and hemodynamic parameters,ulcer size,granulation tissue condition,transcutaneous oxygen pressure(TcPO2),skin temperature,ankle-brachial index,serum markers,and adverse reactions were compared between the two groups. Results The overall efficacy rate was significantly higher in the observation group(91.49%)than in the control group(75.00%,P<0.05).Post-treatment,the observation group showed significantly better results in TcPO2(59.41±3.18),dorsal foot TcPO2(48.46±3.46),dorsal foot artery flow velocity(18.65±5.10 cm/s),and dorsal foot artery diameter(0.18±0.05 cm,all P<0.05).Ulcer dimensions(length 2.62±0.36 cm,width 1.85±0.33 cm)and proportion of fresh granulation tissue(87.23%)were also superior in the observation group(all P<0.05).Both groups experienced significant improvements in skin oxygen pressure,skin temperature,and ankle-brachial index,with more pronounced effects observed in the observation group(P<0.05).Additionally,post-treatment levels of interleukin-6(IL-6)(8.79±1.35 ng/L),neutrophils(7.92±1.33109/L),and high-sensitivity C-reactive protein(Hs-CRP)(1.72±0.36 mg/L)were significantly lower in the observation group(P<0.05).There was no significant difference in adverse reactions between the two groups(P>0.05). Conclusion The combination of silver ion gel and hyperbaric oxygen therapy is highly effective for diabetic foot ulcers,improving lower limb microcirculation and hemodynamics,promoting ulcer healing,reducing inflammatory responses,and presenting minimal adverse effects.It is worthy of wider clinical application.

30 Effects of Acid Ice Stimulation Therapy Combined with Neuromuscular Electrical Stimulation on Rehabilitation and Nutritional Status of Patients with Swallowing Dysfunction in Parkinson's Disease

Objective To investigate the effect of acid ice stimulation combined with neuromuscular electrical stimulation in patients with swallowing dysfunction in Parkinson's disease. Methods 78 patients with swallowing dysfunction of Parkinson's disease were divided into control group(39 cases,receiving routine swallowing rehabilitation training)and observation group(39 cases,receiving acid ice stimulation therapy combined with neuromuscular electrical stimulation)by random number table method.Both groups were continuously observed for 60 days.The recovery of swallowing function,rehabilitation,nutritional status and complications of the two groups were evaluated and compared. Results The scores of normal eating,drinking test,clinical examination and total score in the standard swallowing function scale(SSA)after intervention in the observation group were lower than those in the control group(P<0.05).The total effective rate and nutritional indexes of the observation group were higher than those of the control group(P<0.05).The complications of aspiration and aspiration pneumonia in the observation group were less than those in the control group(P<0.05). Conclusion The combination of acid and ice stimulation therapy with neuromuscular electrical stimulation can promote the recovery of swallowing function,further improve the rehabilitation effect,improve the nutritional status of patients,and reduce the incidence of complications.

33 The Effect and Safety of Low-dose Sodium Valproate on Cognitive Function in Patients with Bipolar Disorder

Objective To investigate the effect and safety of low-dose valproate on cognitive function in patients with bipolar disorder(BPD). Methods A retrospective study was conducted on 115 patients with BPD.Groups A,B,and C were set up according to different treatment doses of sodium valproate,with 40,40,and 35 patients in each group.Group A received 10mg/kg valproate,Group B 15mg/kg valproate and Group C 20mg/kg valproate.Comparing three groups of general data and clinical efficacy,test the plasma concentration related indicators(mean plasma concentration,steady state plasma concentration trough time),and evaluate the three groups of young manic rating scale(YMRS),the Montreal cognitive assessment scale(MoCA)score and liver function indicators[alanine aminotransferase(ALT),glutamase aminotransferase(AST),total bilirubin(TBIL)]level,and finally compare the adverse reactions. Results The total effective rates of the three groups were comparable(P>0.05);The average blood drug concentrations of the three groups were comparable(P>0.05);Compared with group A,groups B and C had shorter time to reach the steady-state blood drug concentration trough(P<0.05);Multiple linear regression was used to correct the baseline data of three groups.Before treatment and 2 months after treatment,the MoCA and YMRS scores of the three groups were comparable(P>0.05);Two months after treatment,YMRS scores decreased and MoCA scores increased in all three groups compared to before treatment(P<0.05);Before treatment,the liver function levels of the three groups were comparable(P>0.05);Two months after treatment,the liver function levels of all three groups increased compared to before treatment,and group A had a lower level(P<0.05);Compared with groups B and C(17.50%,31.43%),group A had a lower overall incidence of adverse reactions at 7.50%(P<0.05). Conclusion 10mg/kg,15mg/kg,20mg/kg and 20 mg/kg of BPD can improve manic and depressive symptoms and cognitive function.However,10mg/kg of valproate had the least effect on liver function and had high safety.

37 Study on the Correlation between Platelet Antibodies Cross Matching and Ineffectiveness of Myeloma Infusion

Objective To explore the correlation between platelet antibodies,cross matching results,and ineffective platelet transfusion in patients with multiple myeloma. Method A retrospective analysis was conducted on 124 patients with multiple myeloma.The patients were divided into an effective group and an ineffective group based on whether platelet transfusion was effective.Among them,there were 100 cases in the effective group and 24 cases in the ineffective group.Compare the clinical data,platelet antibodies,and cross matching results of two groups of patients,and analyze the correlation between platelet antibodies,cross matching results,and ineffective platelet transfusion in patients with multiple myeloma,as well as the influencing factors of ineffective platelet transfusion. Result Factor analysis showed that splenomegaly(β=3.524,...),OR=3.933,95%Cl=7.047-13.397)、 Heat generation(β=1.630,OR=5.102,95%Cl=1.389-18.744)、 Platelet antibody positivity(β=2.114,OR=8.280,95% Cl=2.056-23.340)and cross matching positivity(β=1.548,OR=4.701,95% Cl=1.104-20.016)are both influencing factors for ineffective platelet transfusion in patients. Conclusion Positive platelet antibodies,cross matching,fever,and splenomegaly are independent influencing factors for ineffective platelet transfusion in patients with multiple myeloma.Clinical attention should be paid to patients with multiple myeloma who are accompanied by these influencing factors.

40 Effects of Celecoxib Preadministration Plus Lumbar Plexus Block Combined with General Anesthesia and Lumbar Plexus Block on Postoperative Pain,Inflammatory Factors and Joint Function Recovery in Patients with Hip Replacement

Objective To investigate the effects of pre-administration of celecoxib combined with lumbar plexus block and general anesthesia on postoperative pain,inflammatory factors,and joint function recovery in patients undergoing hip replacement surgery. Methods A total of 86 patients admitted to the hospital for hip replacement surgery were randomly divided into an observation group(n=43)and a control group(n=43).Both groups received lumbar plexus block combined with general anesthesia,followed by patient-controlled intravenous analgesia(PCIA)postoperatively.Additionally,the observation group took oral celecoxib 8 hours before anesthesia induction.Pain levels at rest and during activity at different time points postoperatively,as well as serum levels of inflammatory and pain factors,were compared.The time to first straight leg raise,hip flexion to 90°,and the maximum flexion and abduction range of the affected hip joint postoperatively were recorded.Postoperative recovery and perioperative adverse events were also documented. Results The Visual Analog Scale(VAS)scores at rest and during activity at 6,12,24,and 48 hours postoperatively were significantly lower in the observation group than in the control group(P<0.05).Serum levels of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),prostaglandin E2(PGE2),and β-endorphin(β-EP)at 24 and 48 hours postoperatively were significantly lower in the observation group than in the control group(P<0.05).The time to first straight leg raise and hip flexion to 90° was significantly shorter in the observation group,and the maximum flexion and abduction range of the affected hip joint at 24 and 48 hours postoperatively were significantly greater than those in the control group(P<0.05).The recovery time,extubation time,and time to ambulate were significantly shorter in the observation group,and the number of PCIA compressions within 48 hours postoperatively was significantly lower than that in the control group(P<0.05).The incidence of postoperative delirium and the total incidence of perioperative adverse events were significantly lower in the observation group than in the control group(P<0.05). Conclusion The combination of celecoxib and lumbar plexus block effectively enhances postoperative analgesia,reduces levels of inflammatory and pain factors,promotes hip joint function recovery,and decreases the incidence of postoperative delirium in patients undergoing hip replacement surgery,warranting its promotion.

Objective To understand the comorbidity pattern of chronic diseases in a certain area of Sichuan,and to explore the influencing factors of chronic comorbidities in middle-aged and elderly people,so as to provide a reference for the prevention and control of comorbidities. Methods In 2023,a multi-stage stratified random sampling method was used to recruit 4521 middle-aged and elderly residents in the area for a household survey on social determinants of health for chronic diseases.The number of chronic diseases in different gender,age,marital status,educational level,occupation,drinking habits,and types of medical insurance was compared.The comorbidity pattern of chronic diseases in the area was analyzed by potential class analysis,and the influencing factors of chronic disease comorbidity were analyzed by multiple linear regression. Results A total of 4521 middle-aged and elderly people were included,of which 886 were chronic disease comorbidity patients,with a comorbidity rate of 19.60%(886/4521).Among the non-chronic disease patients,3635 were non-chronic disease patients,Three category model was selected as the optimal model after considering the potential class analysis comprehensively.The results of multivariate linear regression analysis showed that middle-aged and elderly individuals who drink alcohol,have lost a spouse,and have urban employee medical insurance were positively associated with the co-morbidity of chronic diseases(P<0.05),while those aged 45-59 and in employment were negatively associated with the co-morbidity of chronic diseases(P<0.05). Conclusion The chronic disease co-morbidity pattern in the study area is mainly composed of three types of "multiple system disease type""relatively healthy type",and "cardiovascular disease type".In daily health management,attention should be paid to middle-aged and elderly individuals who drink alcohol,have lost a spouse,and have urban employee medical insurance,and targeted health management should be carried out.Reduce the incidence of chronic comorbidities.

The research progress of integrated Chinese and Western medicine in the treatment of viral pneumonia has revealed its multiple mechanisms in improving patients' immune function,regulating inflammatory response and alleviating symptoms.By integrating the holistic concept of traditional Chinese medicine with the modern treatment methods of Western medicine,this combination model not only improves the treatment effect,but also effectively reduces side effects and promotes the rapid recovery of patients.The significance of this study is that it provides a new way of thinking for the treatment of viral pneumonia,emphasizes the importance of interdisciplinary collaboration,provides a more comprehensive and personalized treatment plan for clinical practice,and promotes the application and development of integrated Chinese and Western medicine in the treatment of respiratory diseases.This article discusses the clinical characteristics of viral pneumonia,the basic principles of integrated treatment of traditional Chinese and Western medicine,the mechanism of Western medicine treatment,the mechanism of traditional Chinese and Western medicine treatment,and the synergistic effect of integrated Chinese and Western medicine,in order to guide the development of related work.

This article reviews the epidemiological characteristics,toxin classification and mechanism,clinical manifestations and diagnosis,diagnosis and treatment progress and treatment plans,prevention and control strategies and public education,research hotspots and future directions of common mushroom poisoning.Mushroom poisoning has become a global public health concern,especially in regions rich in mushroom resources such as Yunnan and Hunan in China,where poisoning incidents are particularly prominent.This article aims to provide references for the diagnosis,treatment and prevention of mushroom poisoning,and to effectively reduce the incidence and mortality of mushroom poisoning through measures such as strengthening epidemiological monitoring,improving public identification ability,improving the rescue system,and conducting in-depth scientific research.
