To Investigate the Effect of Inorganic Selenium on Metabolic Bone Disease in Rats

Objective
To explore the effects of different selenium levels on metabolic bone disease in rats and observe the response after vitamin D treatment.
Methods
Rats were randomly divided into a blank control group,a non selenium supplemented group,a low selenium group,and a high selenium group.There were 30 rats in the blank control group,32 rats in the non selenium supplemented group,42 rats in the low selenium group,and 42 rats in the high selenium group.Except for the blank control group,the other three groups of rats were modeled using hydroxyethylenediphosphonic acid.After 7 days of subcutaneous injection,8 rats were randomly selected to collect samples for modeling and vitamin D testing.The successful modeling was confirmed,and then the rats were fed in groups.The low selenium group was given inorganic selenium [1.0mg/(kg·w) sodium selenite] by gavage,the high selenium group was given inorganic selenium [5.0mg/(kg·w)sodium selenite] by gavage,and the non selenium group was also given an equal amount of physiological saline by gavage.After 4 weeks of continuous collection,samples were collected to test serum bone metabolism biochemical indicators; Administer a one-time intramuscular injection of 9000 U/100g of vitamin D,continue the above dose of selenium gavage for 4 weeks,and collect samples to test serum bone metabolism biochemical indicators.
Results
When supplementing with selenium alone,there was no significant increase in serum 25 hydroxyvitamin D levels in the experimental group rats,and there was no significant difference between the groups.However,the alkaline phosphatase in the high selenium group was lower than that in the non supplemented and low selenium groups(

0.05);After selenium combined with vitamin D supplementation treatment,the serum levels of 25 hydroxyvitamin D in the experimental group rats were significantly increased compared to before,and the low selenium group was higher than the non selenium group(
P<
0.05).
Conclusion
Low selenium can promote the conversion of vitamin D to 25 hydroxyvitamin D in a rat model of metabolic bone disease.

The Effect of Xinbikeduobao on Lung Function and Inflammatory Response in Patients with Bronchiectasis

Objective
To explore the effects of Xinbikeduobao on lung function and inflammatory response in patients with bronchiectasis(BE).
Methods
80 BE patients admitted to our hospital were randomly divided into two groups using numerical expression,with 40 patients in each group.The control group received basic symptomatic supportive treatment,and based on this,the study group received additional treatment with Xinbikeduobao for 4 weeks.The efficacy,lung function,blood gas analysis indicators,inflammatory response,and quality of life were compared between the two groups.
Results

Comparative Observation of the Clinical Efficacy of the Same Scheme in the Treatment of Simple Pulmonary Tuberculosis and Pulmonary Tuberculosis with Diabetes

Objective
To compare the therapeutic effects of the same antituberculosis methods on simple pulmonary tuberculosis and pulmonary tuberculosis with diabetes,so as to provide corresponding reference for clinical treatment scheme selection.
Methods
120 patients with pulmonary tuberculosis were selected and divided into group A(75 cases) with simple pulmonary tuberculosis according to whether they have diabetes or not;45 patients in group B with pulmonary tuberculosis and diabetes.In terms of treatment method selection,both groups of patients were intervened with the 2RHZE/4HR anti tuberculosis treatment regimen,which lasted for 6 months.Compare the efficacy,sputum conversion,immune function,and adverse reaction indicators of two groups of research subjects.
Results
After 6 months of intervention,there was no significant difference in the total effective rate of treatment between the observation group and the control group (89.33% vs 91.11%,
P>
0.05).The sputum conversion rates after 1 month,3 months,and 6 months of treatment were 72.00%,85.33%,and 94.67% in the observation group,and 40.00%,48.89%,and 66.67% in the control group,respectively.The sputum conversion rates in the observation group were higher than those in the control group at different stages after treatment(

0.05).After 6 months of treatment,the peripheral blood T lymphocyte subsets CD3

in both groups of patients increased,while CD8

decreased.The changes in these indicators were more significant in the observation group compared to the control group(

Conclusion
The same antituberculosis program intervenes in pulmonary tuberculosis with simple or combined diabetes,and the treatment effect is different.In diabetes,the antituberculosis drug intervention has poor effect on sputum bacteria negative,and at the same time,the improvement of immune function is poor.Therefore,in practice,it is necessary to cooperate with certain blood glucose control when treating pulmonary tuberculosis with diabetes,and then combine the antituberculosis program intervention to achieve good treatment effect.

Comparative Study of Single-port Thoracoscopic Subsegmental Versus Segmental Lung Resection for the Treatment of Pulmonary Nodules in the Elderly

Objective
To Comparative analysis of the clinical efficacy of single port thoracoscopic lung segmentectomy and segmentectomy in the treatment of elderly pulmonary nodules.
Methods
72 patients with pulmonary nodules were randomly divided into two groups,with 36 patients in each group.The control group underwent single port thoracoscopic segmentectomy,while the observation group underwent single port thoracoscopic segmentectomy.Compare two groups of perioperative indicators[number of resected segments,bleeding volume,drainage volume,length of hospital stay,visual analog scale (VAS)],lung function,stress indicators[cortisol (Cor),malondialdehyde (Mda),reactive oxygen species(Ros)]levels,complications,and FACT-L scores for lung cancer patients.
Results
Compared with the control group,the observation group had fewer sub segments,less bleeding and drainage,shorter hospital stay,and lower VAS scores(

0.05); After surgery,compared with the control group,the observation group had higher FVC,MVV,FEV

,and FACT-L scores,and lower Cor,Mda,and Ros scores(

Conclusion
Single port thoracoscopic lung segmentectomy can optimize perioperative conditions,reduce postoperative pain,have minimal impact on lung function,alleviate stress reactions,and improve quality of life in elderly patients with pulmonary nodules.

The Effect of Enteral Feeding with Blind Insertion of Nasoenteric Tube on the Nutritional Status and Prognosis of Patients with Stroke Combined with Dysphagia

Objective
To study the effect of blind insertion nasoenteric tube enteral feeding(EN) on the nutritional status and prognosis of patients with stroke combined with dysphagia(DAS).
Methods
78 cases of DAS patients in our hospital were selected as the subjects of this experiment, and all the included subjects were divided into the nasoenteric tube group(n=39) and the nasogastric tube group(n=39) according to the method of randomized numerical table.The nasogastric tube group used nasogastric tube EN,and the nasoenteric tube group used blind insertion nasoenteric tube EN.Nutritional status[albumin(Alb),hemoglobin(Hb),transferrin(TF),prealbumin(PA)],prognosis[Fugl-Meyer Motor Function Assessment Scale(FMA) scores,Barthel Index(BI) scores],and the rate of complication were compared between the two groups before and after treatment.
Results
After treatment,Alb,Hb,TF,and PA levels were higher in the nasoenteric tube group compared with the nasogastric tube group(
P
<0.05);FMA score and BI score were higher in the nasoenteric tube group compared with the nasogastric tube group after treatment(
P<
0.05); and complications were lower in 1 case(2.56%) in the nasoenteric tube group compared with 8 cases(20.51%) in the nasogastric tube group(

0.05).
Conclusion
Blind nasoenteric tube EN used in DAS patients can achieve good therapeutic effects,which is conducive to improving the nutritional index of patients,restoring their ability to exercise and activities of daily living,improving the prognosis,and the risk of complications is low.

The Effect of Stepped Decompression Treatment on the State of Consciousness and Serum miR-34a and miR-16 in Brain Injured Patients with Ventricular Intracranial Pressure Monitoring

Objective
To explore the influence of stepped decompression under ventricular intracranial pressure monitoring on status of consciousness and serum miR-34a and miR-16 in brain injury patients.
Methods
The 120 patients with craniocerebral trauma were divided into the observation group(n=63) and the control group(n=57).The control group underwent traditional decompression,and the observation group received ventricular intracranial pressure monitoring on the basis of the control group.The two groups were compared in terms of clinical efficacy,intracranial pressure level,neurological function and occurrence of complications.
Results
Compared with control group,the clinical efficacy in observation group was better(
P<
0.05).At 1 d,3 d and 1 w after surgery,both groups showed a stepwise decrease in intracranial pressure,and the observation group had lower intracranial pressure compared to control group(
P<
0.05).The score of Coma Recovery Scale-Revised(CRS-R) after treatment was increased in both groups,and the CRS-R scale score was higher in observation group(
P<
0.05).The levels of miR-34a and miR-16 in both groups were decreased after treatment,and the levels in observation group were lower(

0.05).The incidence rates of intracranial infection,delayed hematoma,gastrointestinal bleeding,and renal insufficiency were similar between the two groups(

0.05),but compared with control group,the incidence rate of acute encephalocele was lower in observation group(
P<
0.05).
Conclusion
The ventricular intracranial pressure monitoring combined with stepped decompression can effectively treat patients with craniocerebral injury,improve their nerve function,and reduce the risk of intraoperative acute encephalocele.

Effect of Miniplate Internal Fixation Therapy on Hand Function Foot Function and Pain Level in Patients with Hand and Foot Fractures

Objective
To investigate the effect of microplate internal fixation therapy,and to analyze the effect of this therapy on joint function and pain degree.
Methods
The patients selected 71 patients with hand and foot fractures for retrospective study,which was divided into the reference group(35 patients treated with Kirschner integration) and the study group(36 patients treated with microplate internal fixation),compared the Broberg and Morrey hand function score,Maryland foot function score and pain degree before and after treatment,and observed the incidence of postoperative complications in the two groups.
Results
After surgery,the hand function and foot function scores were higher than the reference group(

0.05); after surgery,the pain score was lower than the reference group(
P<
0.05);the postoperative complication rate in the study group was lower than the reference group(
P<
0.05).
Conclusion
Micro plate fixation can significantly reduce the pain of hand and foot fractures and reduce the hand function and postoperative complications.

The Effect of Rocuronium Bromide and Phenylsulfonyl cis Atracurium Anesthesia on Anesthesia Efficacy and Complications in Patients undergoing Cholecystectomy

Objective
To compare the application effects of different muscle relaxants in rapid induction tracheal intubation general anesthesia for cholecystectomy.
Methods
A total of 54 patients were included in this clinical trial.All patients underwent laparoscopic cholecystectomy(LC) and received general anesthesia with combined muscle relaxants and induction tracheal intubation during the operation.The patients were randomLy divided into two groups,each containing 27 cases,and were respectively designated as the control group and the observation group.The control group was given rocuronium bromide muscle relaxant anesthesia,and the observation group was given cis-atracurium benzenesulfonate muscle relaxant anesthesia.The anesthesia effect and blood flow of the two groups were compared.Dynamic changes[heart rate(HR),mean arterial pressure(MAP)],stress indicators(blood sugar,plasma total cortisol,histamine levels),and complications of anesthesia,etc.
Results
In terms of the average onset time,maintenance time and recovery time of muscle relaxation,the control group was shorter than the observation group(
P<
0.05).In terms of hemodynamics,there was no significant fluctuation in HR and MAP levels in the observation group after drug administration compared with before(
P>
0.05),while in the control group,MAP level decreased and HR level increased after drug administration compared with before,and there was a significant difference between the two groups(
P<
0.05).There was no significant fluctuation in blood glucose,total plasma cortisol and histamine levels in the observation group after drug administration compared with before drug administration in the same group(
P>
0.05),while in the control group,the above indicators fluctuated significantly compared with before drug administration in the same group,and there was a significant difference between the two groups(

0.05).The incidence of anesthesia complications was higher in the control group than in the observation group(

0.05).
Conclusion
In the anesthesia regimen for patients undergoing cholecystectomy,rocuronium has a faster onset,but the efficacy of cisatracurium besilate lasts longer,but Cisatracurium besylate has a longer duration of drug efficacy,provides more stable hemodynamic conditions for patients,and offers advantages such as fewer complications and mild stress responses.

Study on the Therapeutic Effect of Percutaneous Nephrolithotomy(PCNL) without Placement of Nephrostomy Tube for Upper Urinary Tract Stones with Renal Effusion

Objective
To investigate the effect of not placing nephrostomy tube in patients with upper urinary tract stones and renal effusion treated by percutaneous nephrolithotomy.
Methods
274 patients who underwent percutaneous nephrolithotomy were retrospectively included in the study.The patients were divided into group A(with nephrostomy tube,n=113) and group B(without nephrostomy tube,n=161) according to the treatment modality.The postoperative recovery,complication rate,inflammatory factor level,stone clearance rate and renal function indexes of the two groups were compared and analyzed.
Results
This study showed that the postoperative recovery of Group B was better than that of Group A,and the difference was statistically significant(
P<
0.05); the complication rate of Group B was not significantly different from that of Group A,and the difference was not statistically significant(
P>
0.05);comparing the levels of inflammatory factor in the two groups at different postoperative time points,the level of inflammatory factor in Groups B and A were both elevated postoperatively,and the indexes of Groups B and A did not differ significantly,and there was no statistically significant difference between the groups(

0.05); the level of stone clearance and renal function indicators of Group B were compared.Group B had no significant difference in stone removal rate compared with Group A,and there was no statistically significant difference(

Conclusion
In patients with percutaneous nephrolithotomy,no nephrostomy tube is placed,which has the advantages of fast postoperative recovery,low pain,short hospitalization time and reduced treatment costs,and better meets the therapeutic needs of patients.

Effect of Single-bundle ACL Reconstruction with Residual Preservation on Knee Joint Mobility and Stability after Surgery

Objective
To analyze the effect of single-bundle anterior cruciate ligament(ACL) reconstruction with residual preservation on knee joint mobility and stability after surgery.
Methods
70 patients with ACL injury in our hospital were selected and were divided into preserved and unpreserved groups according to the treatment method,with 35 cases each.The unpreserved group was treated with single-bundle reconstruction of ACL with unpreserved stump,and the preserved group was treated with single-bundle reconstruction of ACL with preserved stump.Lysholm knee function,knee mobility,Tegner score,axial shift test,proprioception[passive angle regeneration test(JPS),passive activity perception threshold(TTDPM)],osteopontin(OPN),matrix metalloproteinase-1(MMP-1),and bone marrow protein(BMP)were compared between the two groups.1(MMP-1),and bone-γ-carboxyglutamic acid-containing protei,BGP)levels.
Results
At 6 months postoperatively,there was no significant difference between the 2 groups in the positive comparison of knee mobility and knee axial shift test(
P>
0.05),and the Lysholm knee function and Tegner score were higher in the preserved group than in the un-preserved group(
P<
0.05);at 6 months postoperatively,the JPS and TTDPM were lower in the preserved group than in the un-preserved group(
P<
0.05);and at 6 months postoperatively,the OPN,MMP-1,and BGP were lower in the preserved group than in the unpreserved group(
P<
0.05).
Conclusion
ACL injury patients were treated with single-bundle reconstruction of ACL with preserved stump,which could promote the recovery of knee function,improve proprioception,and reduce the levels of OPN,MMP-1 and BGP.

Application Effect of APC Assisted EMR in Large Gastrointestinal Polyps and Its Influence on Safety Analysis

Objective
To explore the effect of argon ion coagulation(APC) assisted endoscopic mucosal resection(EMR) in the treatment of large gastrointestinal polyps and analyze its impact on safety.
Methods
82 patients with large gastrointestinal polyps in our hospital were selected and divided into two groups(n=41) by random number method.The control group was treated with endoscopic mucosal resection,and the study group was treated with argon ion coagulation.The perioperative indexes,blood lipid levels,therapeutic effects and complications of the two groups were evaluated.
Results
Operation,hospital stay,intraoperative blood loss,and postoperative 1d and 3dVAS scores in the study group were lower than those in the control group(
P<
0.05).There was no statistical difference in the levels of TC,TG,TNF-α,LDL-C and HDL-C between the two groups before surgery(
P>
0.05).Two months after surgery,the levels of TC,TG,TNF-α and LDL-C decreased,while the levels of HDL-C increased,and the rates of TC,TG,TNF-α,LDL-C and complications in the study group were lower than those in the control group.The HDL-C level and total effective rate were higher than those of control group(

0.05).
Conclusion
The application of APC assisted EMR in large gastrointestinal polyps has a good effect,which can accelerate the postoperative recovery of patients,reduce the occurrence of complications,and improve the blood lipid level of patients.

Effectiveness of Hysteroscopic Orthopedics Combined with Maflon in Patients with Abnormal Uterine Bleeding Due to PCSD

Objective
To analyze the efficacy of hysteroscopic incision diverticuloplasty combined with oral contraceptive pills in abnormal uterine bleeding due to previous cesarean scar defect(PCSD).
Methods
80 patients with abnormal uterine bleeding due to PCSD were enrolled and were divided into the reference group and the observation group by complete randomization,40 patients in both groups,the reference group was given hysteroscopic incision diverticuloplasty,and the observation group was given oral contraceptive pill mafenacoum on top of the reference group,and the efficacy of the treatment was compared between the two groups after three months of treatment.The two groups were compared in terms of efficacy,clinical indicators,i.e.changes in the duration of vaginal bleeding,menstrual period and menstrual flow,and the incidence of adverse reactions.
Results
After the treatment,the effective rate of the observation group was 95%(38/40),which was higher than that of the reference group(80%)(32/40)(
P<
0.05); the duration of vaginal bleeding in the observation group was significantly shorter(5.19±1.08)d than that of the reference group(7.62±1.54)d,and the number of menstrual periods in the observation group was significantly shorter(5.84±1.35)d than that of the reference group(10.34±2.29)d,and the number of menstrual periods in the observation group was significantly shorter than that of the reference group(10.34±2.29)d,and the number of menstruation days in the observation group was significantly shorter than that of the reference group(10.34±2.29)d,the amount of menstrual flow in the observation group(46.83±8.52)mL was significantly shorter than that in the reference group(77.58±12.36)mL(

0.05),the hospitalization time in the observation group(5.63±1.78)d was significantly shorter than that in the reference group(7.05±1.97) d,and the thickness of the residual muscular layer of the incision in the postoperative period of the observation group(2.51±0.54) mm was significantly less than that in the control group(2.78±0.59)mm(

0.05).There was a 10%(4/40) adverse reaction rate in the observation group,which was lower than the 30%(12/40) in the reference group(

Conclusion
Hysteroscopic incision diverticuloplasty combined with oral contraceptive pills was effective in treating abnormal uterine bleeding due to PCSD,improving the duration of vaginal bleeding,menstrual period,and lowering the adverse effects.

Analysis of the Effect of Human Interferon α1b Aerosol Inhalation Combined with Conventional Treatment on Young Children with Respiratory Syncytial Virus Infection Pneumonia

Objective
To explore the application effect of human interferon α1b(IFN-α1b) aerosol inhalation combined with conventional therapy for young children with respiratory syncytial virus(RSV)-infected pneumonia.
Methods
A total of 82 children with RSV-infected pneumonia were randomLy selected as research samples and randomLy separated into two groups according to the digital table method.The conventional group adopted conventional treatment,and the aerosol group was treated with IFN-α1b aerosol inhalation on the basis of conventional treatment.The clinical efficacy,duration of symptom subsidence,time required for white blood cell count to return to normal,length of hospital stay,C-reactive protein(CRP) and incidence of adverse reactions between the two groups were compared.
Results
The clinical efficacy of the nebulizer group was higher than that of the conventional group,and the duration of symptom subsidence,the time required for the white blood cell count to return to normal,and the length of hospital stay were all lower than those of the conventional group(
P<
0.05).After 6 days of treatment,the CRP level of the nebulizer group was also lower than that of the conventional group(
P<
0.05).No notable disparity was observed in the occurrence of adverse reactions between the two groups(
P>
0.05).
Conclusion
IFN-α1b nebulizer inhalation can effectively relieve symptoms,improve inflammatory status,and has high safety,which can provide a reference for the formulation of treatment strategies for children with RSV-infected pneumonia in clinical practice.

Objective
To investigate the effect of full process digital guide plate on the accuracy of implantation site during immediate implantation and restoration of anterior teeth.
Method
72 patients who underwent pre implantation of anterior teeth were selected.The patients were divided into an observation group and a control group,with 36 patients in each group(random number table method),as the study subjects.The observation group underwent immediate implantation and restoration of anterior teeth with the assistance of a fully digital guide plate,while the control group underwent immediate implantation and restoration of anterior teeth as a control.Compare the accuracy of the upper anterior tooth implantation site(neck,root tip,axial angle,and depth deviation of the implant),gingival health indicators(bleeding and plaque index),aesthetic effects(red and white aesthetic effect scores),and complications between the observation group and the control group.
Result
One week after surgery,the observation group had smaller deviations in all aspects of the implant placement site compared to the control group,with statistical differences(

<0.05).The observation group had lower bleeding and plaque index than the control group at 3 months after surgery,with statistical differences(
P
<0.05).The red aesthetic effect score of the observation group was higher than that of the control group at 3 months after surgery,and there was a statistical difference(
P
<0.05); There was no statistically significant difference in the white aesthetic effect score between the two groups(
P
>0.05).There was no statistically significant difference in complications between the two groups(

>0.05).
Conclusion
The application of fully digital guide plates in immediate implantation and restoration of anterior teeth can improve the accuracy of implantation sites and postoperative aesthetic effects,reduce bleeding and plaque,and do not increase the incidence of postoperative complications.

Analysis of the Effect of GTR Combined with Fixed Orthodontics on Patients with Periodontal Disease Accompanied by Pathological Displacement of Anterior Teeth as Assessed by Inflammatory Factors in Gingival Crevicular Fluid

Objective
To analyze the application value of guided tissue regeneration(GTR) combined with fixed orthodontics in the treatment of periodontal disease accompanied by pathological displacement of the anterior teeth.
Methods
This study collected data from 80 patients with periodontal disease and pathological displacement of anterior teeth who were admitted and diagnosed for retrospective research.According to the treatment plan,41 patients who underwent GTR were grouped as the control group,and 39 patients who underwent GTR combined with fixed orthodontics were grouped as the study group.The periodontal status,anterior tooth displacement angle,depth of alveolar bone defect,quality of life and inflammatory factors in gingival crevicular fluid were compared between the two groups before and after 6 months of treatment.
Results
After 6 months of treatment,the study group had lower scores in periodontal pocket depth(PD),SNA angle,bleeding index(SBI),vertical bone absorption depth,gingival crevicular fluid interleukin-6(IL-6),ANB angle,tumor necrosis factor-α(TNF-α),UI-SN angle,matrix metalloproteinase-9(MMP-8),and clinical attachment loss(CAL) than the control group,while the upper central incisor convex angle difference and the score of the General Quality of Life Inventory-74(GQOLI-74) were higher than those of the control group(

0.05).
Conclusion
GTR combined with fixed orthodontics is used for periodontal disease with pathological displacement of anterior teeth,effectively improving the periodontal condition,vertical bone absorption depth,anterior tooth displacement,expression of inflammatory factors in gingival crevicular fluid,and improving quality of life.
