1 Anti Fibrosis Effect of α-MSH on Bleomycin Induced Pulmonary Fibrosis in Mice

Objective To observe the effect of α-MSH on the fibrosis factor type I collagen,key nuclear factor in TNF-α and immune-regulation pathway,we initially investigated whether α-MSH has an anti-fibrotic effect in different periods of pulmonary fibrosis. Methods The mouse lung fibrosis model was constructed by bleomycin tracheal drip method,and α-MSH was treated in the first,second and third weeks of animal model to detect the expression of type I collagen,TNF-α mRNA and production of NF-κB protein,and the generation of TNF-α in peripheral blood and alveolar lavage fluid. Results α-MSH could significantly inhibit the expression of type I collagen and TNF-α gene.The production of p-NF-κB protein in the mouse model of bleomycin-induced pulmonary fibrosis could inhibit by α-MSH,which had obvious anti-inflammatory and anti-fibrosis effect.Furthermore α-MSH began to exert this effect in the early stage of the fibrosis,and had the characteristics of long maintenance time.It suggests that α-MSH has an important role in the early immune action mechanism of pulmonary fibrosis. Conclusion α-MSH may be a new target for early treatment of IPF and has great value for early prevention and treatment of IPF.

5 Correlation Analysis between Neutrophil/lymphocyte Ratio in Peripheral Blood and T cell Mediated Rejection after Liver Transplantation

Objective To investigate the correlation between neutrophil/lymphocyte ratio in peripheral blood and T cell mediated rejection after liver transplantation. Methods This study enrolled 80 liver transplant recipients.Patients were divided into a rejection group(16 cases)and a non-rejection group(64 cases)based on the occurrence of TCMR.Preoperative,intraoperative,and postoperative clinical data were compared between the two groups.Risk factors for TCMR were analyzed using the Log-rank test and Logistic regression(univariate and multivariate analyses).Pearson correlation was used to assess associations. Results The rejection group exhibited significantly longer operative time,higher Rejection Activity Index(RAI),and elevated postoperative NLR levels.Multivariate Logistic regression identified RAI,NLR on postoperative day 3,NLR on day 5,operative duration,and red blood cell transfusion volume as independent risk factors for TCMR(Figure 2).Among these,NLR on day 1 showed the strongest association.Pearson correlation confirmed a positive correlation between these factors and TCMR risk. Conclusion Prolonged operative time,increased red blood cell transfusion volume,elevated postoperative RAI,and higher NLR values are associated with an increased risk of acute rejection after liver transplantation.These findings highlight the need for clinical monitoring and preventive strategies.

10 Comparative Study on Lactate Clearance Rate and 28-day Survival Rate of Patients with Hyperlipidemia Acute Pancreatitis Treated with CRRT at Different Intervention Opportunities

Objective To investigate the impact of different initiation timings of continuous renal replacement therapy(CRRT)on lactate clearance rate and 28-day survival in patients with hyperlipidemic acute pancreatitis. Methods 98 patients with hyperlipidemic acute pancreatitis admitted to our hospital from January were selected retrospectively and included in the case group.According to the intervention opportunity of CRRT,they were divided into early group(53 cases)and late group(45 cases),and the clinical data,lactate clearance rate and serological indexes of the two groups were compared.Kaplan-Meier survival analysis patients' 28-day survival rate. Results There was no significant difference in age,sex and respiratory rate between the early group and the late group(P>0.05).The 24-hour lactate clearance rate and 48-hour lactate clearance rate in early group were significantly higher than those in late group and,respectively(P<0.05).After treatment,the levels of AMY,TNF-αmL,IL-1βand PCT in the early group.Kaplan-Meier survival analysis showed that the overall survival rate of early group was,which was significantly higher than that of late group(P<0.05). Conclusion Early CRRT intervention significantly enhances lactate clearance,reduces inflammatory markers,and improves 28-day survival rates in hyperlipidemic acute pancreatitis(HLAP)patients,demonstrating its efficacy as a therapeutic strategy for prognosis improvement.

14 Nicodil United Improvement of Blood Flow Reserve in Hypertensive Patients with Left Ventricular Hypertrophy

Objective Effect of Nicorandil Combined with a Low Glycemic Index Diet on Fractional Flow Reserve in Hypertensive Patients with Left Ventricular Hypertrophy. Methods A total of 98 hypertensive patients with left ventricular hypertrophy treated at our hospital were randomly assigned to either the study group(n=49)receiving nicorandil plus low-GI diet intervention or the control group(n=49)receiving nicorandil alone.Outcomes included blood pressure control,therapeutic efficacy,LVH regression,fractional flow reserve(FFR),and adverse events. Result: Post-treatment,the study group demonstrated superior outcomes:lower systolic pressure(113.35±12.06mmHg vs control,P<0.05),higher diastolic pressure,and greater clinical efficacy(93.88% vs 79.59%,P<0.05).Significant LVH regression was observed through reduced LVEDD,IVST,LVPWT and LVMI(all P<0.05).The group also showed improved fractional flow reserve(0.95±0.07 vs control,P<0.05)with comparable low adverse event rates(P>0.05). Conclusion The combination of nicorandil with a low-GI diet significantly improves fractional flow reserve in hypertensive patients with LVH,demonstrating excellent safety.This combined therapy shows superior efficacy in reversing left ventricular hypertrophy and enhancing cardiac function,warranting clinical adoption.

18 The Effect of Sintilimab Combined with GP Regimen Chemotherapy on Serum VEGF,BFGF Levels and Immune Indicators in Advanced NSCLC Patients

Objective To investigate the combined use of sintilimab and gemcitabine+cisplatin(GP) regimen chemotherapy in advanced non-small cell lung cancer. Methods 78 patients with advanced NSCLC were categorized into two groups with random numerical table.The patients of control group were administrated GP regimen,and on this basis,the patients of observation group additionally took sintilimab.The comparisons were made on clinical therapeutic effect,tumor growth factors,tumor markers,immune indicators,and adverse reactions in both groups. Results The ORR of the observation group was higher than the control group(P>0.05),while the DCR was higher,with statistical difference(P<0.05); The observation group had lower levels of vascular endothelial growth factor(VEGF),basic fibroblast growth factor(BFGF),carcinoembryonic antigen(CEA),and T cell subset CD8+ and higher CD4+ compared with the control group after treatment(P<0.05); The comparisons of adverse reactions in both groups expressed no statistical difference(P>0.05). Conclusion The combination of sintilimab and GP regimen chemotherapy can reduce serum tumor growth factors and tumor markers in patients with advanced NSCLC,and improve immune indicators,which is safe and controllable.

21 The Impact of Surgical and Non-surgical Treatments on Gait and Prognosis in Patients with Chronic ankle Instability Caused by Anterior Talofibular Ligament Injury

Objective To investigate the effects of surgical and non-surgical treatments on gait and prognosis in patients with chronic ankle instability caused by anterior talofibular ligament injury. Method A total of 95 patients with chronic ankle instability treated in our hospital were prospectively included and randomly divided into a surgical group(n=48,who underwent Brostr m-Gould ligament repair surgery)and a non-surgical group(n=47,who received 12 weeks of rehabilitation training).Using a three-dimensional motion analysis system to evaluate gait parameters,including step speed,single limb support period,ankle joint torque,and ground reaction force;Using FAOS and CAIT scales to evaluate functional improvements;Follow up for 12 months to record the rate of re injury. Result At 12 months after surgery,the surgical group showed significantly better performance than the non-surgical group in terms of step speed,stride length,single limb support period,ankle dorsiflexion torque,ankle inversion torque,and GRF asymmetry index(P<0.01);The FAOS pain score and CAIT score in the surgical group were significantly better than those in the non-surgical group(P<0.01);The surgical group had a lower rate of re sprain,and the incidence of complications was comparable between the two groups(P>0.05). Conclusion Surgical treatment can more effectively improve gait symmetry and functional prognosis in patients with chronic ankle instability,especially suitable for patients with high activity needs;Non surgical treatment can be used as an alternative for specific populations.

24 Analysis of Dietary Status and Influencing Factors in Patients with Gallstones and Acute/chronic Cholecystitis

Objective To investigate the impact of dietary factors and general demographic characteristics on the risk of developing gallbladder stones(GST)with acute or chronic cholecystitis. Methods Patients with acute cholecystitis associated with GST(n=116),chronic cholecystitis associated with GST(n=97),and a healthy control group(n=97)were collected.The differences between groups were analyzed using Pearson χ2 tests,and a multivariate Logistic regression model was employed to examine the factors influencing cholecystitis associated with GST. Results The protective factors for GST-associated acute cholecystitis are those who consume noodles once daily(OR=0.177,P=0.035),twice daily(OR=0.136,P=0.015),and three times daily(OR=0.044,P=0.006).The risk factors for GST-associated chronic cholecystitis are gender(OR=2.157,P=0.037)and fatty liver(OR=2.831,P=0.041).The protective factors for GST-associated chronic cholecystitis are those who consume eggs 4-6 times per week(OR=0.117,P=0.008)or more than once daily(OR=0.225,P=0.008);dairy products once daily(OR=0.243,P=0.011)or more than twice daily(OR=0.096,P=0.043). Conclusion Dietary factors,such as noodle intake,were associated with a reduced risk of GST with acute cholecystitis,while the intake of eggs and dairy products was associated with a reduced risk of GST with chronic cholecystitis.Appropriate adjustments to dietary structure may help reduce the risk of acute or chronic cholecystitis associated with GST.

30 The Impact of Insulin Combined with Dietary Intervention Therapy on the Lipid Levels and the Incidence of Gestational Complications of Gestational Diabetes Mellitus Patients

Objective To investigate the effects of insulin combined with dietary intervention on the treatment of gestational diabetes mellitus(GDM). Methods A total of 96 GDM patients were included and randomLy divided into two groups.The control group(n=48)received routine dietary intervention guidance,while the observation group(n=48)received insulin combined with dietary intervention.Blood glucose and lipid parameters were compared between the two groups before intervention and after 2 weeks of intervention.The differences in pregnancy-related complications were also compared. Results After 2 weeks of intervention,the observation group showed lower levels of fasting blood glucose(FBG),2-hour postprandial blood glucose(2hPG),glycated hemoglobin(HbA1c),total cholesterol(TC),triglycerides(TG),and low-density lipoprotein cholesterol(LDL-C)compared to the control group(P<0.05).Additionally,the high-density lipoprotein cholesterol(HDL-C)levels in the obser-vation group were significantly higher than those in the control group(P<0.05).The rate of pregnancy-related complications in the observation group was,significantly lower than the observed in the control group(P<0.05). Conclusion The use of insulin com-bined with dietary intervention demonstrates a significant improvement in glucose and lipid metabolism for GDM patients,effectively reducing the incidence of related complications,highlighting its clinical value.

33 The Anesthetic Effect of Alfentanil Combined with Cyclopofol in Gynecological Hysteroscopic Surgery and Its Influence on Postoperative Recovery

Objective To explore the anesthetic effect of alfentanil combined with propofol in gynecological hysteroscopic surgery and its influence on postoperative recovery. Methods According to the inclusion and exclusion criteria,82 patients who underwent gynecological hysteroscopic surgery were selected as the research subjects and a grouped controlled study was conducted on them.The grouping method was the random number table method,and they were divided into two groups,namely the control group and the observation group,with 41 cases in each group.The control group was given alfentanil combined with propofol anesthesia.The observation group was anestheted with alfentanil combined with cyclopofol during the operation.The anesthesia effects,vital signs,quality of anesthesia recovery,sedation and analgesia before and after the operation,cognitive conditions and the incidence of adverse reactions were compared between the two groups. Results The excellent and good rate of anesthesia in the observation group was higher than that in the control group(P<0.05).The heart rate(HR)and mean arterial pressure(MAP)of the two groups at 3 minutes after administration(T1)and during cervical dilation(T2)were lower than those at the T0 time point(P<0.05),and there was no statistical difference in HR and MAP at different time points between the two groups(P>0.05).The anesthesia induction,orientation recovery,eye opening and breathing recovery time in the observation group were shorter than those in the control group(P<0.05).The scores of the Alert/Sedation observation group(OAA/S)and the Mini-Mental State Examination(MMSE)6 hours after the operation in the observation group were higher than those in the control group.The Visual Analogue Scale(VAS)for pain was lower than that of the control group(P<0.05).The incidence of adverse reactions in the observation group was lower than that in the control group(P<0.05). Conclusion For patients undergoing gynecological hysteroscopic surgery,the use of alfentanil combined with cyclopofol anesthesia can not only effectively improve the anesthetic effect,reduce the impact on their vital signs,but also promote their postoperative recovery,enhance their sedative and analgesic effects,promote the recovery of their cognitive function,and reduce the incidence of adverse reactions.

36 The Expression of Serum D-dimer,Interleukin-6,and PCT in Children with Mycoplasma Pneumoniae Pneumonia and Their Value in Disease Assessment

Objective Taking children with Mycoplasma pneumoniae pneumonia as the research objects,serum D-dimer(D-D),interleukin-6(IL-6),and procalcitonin(PCT)were used as research indicators to analyze and explore the expression levels of the three research indicators in this disease and their clinical value for disease assessment. Method Based on inclusion and exclusion criteria,66 children with Mycoplasma pneumoniae pneumonia were selected as the study subjects and named the observation group;In addition,66 healthy children who underwent physical examinations at our hospital during the same period were selected as the control group to compare the expression levels of serum D-D,IL-6,and PCT between the two groups.At the same time,the observation group was divided into mild group(n=36 cases)and severe group(n=30 cases)according to the severity of the disease.The expression levels of serum D-D,IL-6,and PCT were compared between the two groups,and the correlation between serum D-D,IL-6,and PCT expression levels and disease severity was analyzed.Compared with the control group,the observation group showed higher levels of serum D-D,IL-6,and PCT expression(P<0.05);In the observation group,the expression levels of serum D-D,IL-6,and PCT in the severe group were higher than those in the mild group(P<0.05);Through Pearson correlation analysis,it was found that the expression levels of serum D-D,IL-6,and PCT were positively correlated with the severity of Mycoplasma pneumoniae pneumonia(r>0,P<0.05). Conclusion In the diagnosis process of Mycoplasma pneumoniae pneumonia in children,serum D-D,IL-6,and PCT all show high expression levels,and the above three indicators are positively correlated with the severity of the disease,which has high application value in disease assessment.

39 Effects of Different Doses of Dexmedetomidine Combined with Intraspinal Anesthesia on Hemodynamic Stability and Postoperative Delirium in Elderly Patients Undergoing Hip Surgery

Objective To investigate the effects of different doses of dexmedetomidine(Dex)combined with intraspinal anesthesia on hemodynamic stability and postoperative delirium in elderly patients undergoing hip surgery. Methods Eighty elderly patients who underwent hip surgery were selected.Using a random number table,they were divided into two groups.Both groups received intraspinal anesthesia combined with Dex sedation regimen.The maintenance doses for the low-dose group(40 cases)and the high-dose group(40 cases)were 0.2 and 0.4 μg/(kg·h),respectively.The sedative effect [Observer's Assessment of Alertness/Sedation(OAA/S)score and Ramsay sedation scale(RSS)score],hemodynamic indexes [heart rate(HR)and mean arterial pressure(MAP)],stress indexes [norepinephrine(NE),epinephrine(E)and cortisol(Cor)],and occurrence of postoperative delirium and adverse reactions were compared between the two groups. Results The two groups had similar OAA/S and RSS scores at different time points(P>0.05).HR and MAP of the two groups were comparable before anesthesia(T0)and at the end of surgery(T3)(P>0.05).At 20 min after anesthesia(T1),decreases in HR and MAP were significantly greater in the high-dose group than in the low-dose group(P<0.05).HR and MAP of the high-dose group were significantly lower at T1 and T3 compared to T0(P<0.05).However,the low-dose group only showed a significant decrease at T1(P<0.05).At 60 min after anesthesia(T2)and T3,the levels of NE,E and Cor in the high-dose group were significantly higher than those in the low-dose group(P<0.05).The incidence rates of postoperative delirium in the low-dose group and the high-dose group were respectively.The total incidence of adverse reactions in the low-dose group wasin the high-dose group(P>0.05). Conclusion Low-dose Dex combined with intraspinal anesthesia can achieve better effects in maintaining hemodynamic stability,controlling stress response,and reducing adverse reactions.The sedative effect and prevention of postoperative delirium are comparable to high-dose Dex,making it more suitable for intraoperative sedationof elderly patients undergoing hip surgery.

42 Observation on the Therapeutic Effect of High Dacryocystorhinostomy with Extended Bone Window and Continuous "Z" -Shaped Nasal Cavity-Dacryocystorhinostomy under Nasal Endoscopy in the Treatment of Chronic Dacryocystitis

Objective To explore the clinical efficacy and safety of nasal endoscopic extended bone window with high dacryocystornostomy combined with continuous "Z" -shaped nasal cavity-dacryocystorous flap anastomosis(DCR)in the treatment of chronic dacryocystitis,and to conduct a comparative analysis with simple conservative drug treatment. Methods Retrospective clinical study.A total of 76 patients(83 eyes)diagnosed with chronic dacryocystitis were selected.The patients were divided into a conservative treatment group of 38 cases(40 eyes)and a surgical treatment group of 38 cases(43 eyes).Among them,the surgical group underwent high dacryocystornostomy with extended bone window under nasal endoscopy and continuous "Z"-shaped nasal cavity-dacryocystornomucosal flap anastomosis.The improvement rate of epiphora symptoms,lacrimal duct patency rate,complication rate and recurrence rate were compared between the two groups. Results Three months after the operation,the improvement rate of epiphora symptoms in the surgical group was 97.6%,and that in the conservative group was 50.0%(P<0.001).The lacrimal duct patency rate was 97.6% in the surgical group and 25.0% in the conservative group(P<0.001).The incidence of complications was 4.6% in the surgical group and 15.0% in the conservative group(P=0.082).The recurrence rate was 2.3% in the surgical group and 20.0% in the conservative group(P<0.001). Conclusion Compared with conservative treatment,the use of transnasal endoscopic extended bone window high dacryocystorhinostomy combined with continuous"Z" -shaped nasal cavity-dacryocystorhinostomy flap anastomosis for the treatment of chronic dacryocystitis can significantly improve the therapeutic effect,improve the patency of the lacrimal duct,reduce the risk of recurrence,and have fewer complications.It is an effective method for the treatment of chronic dacryocystitis.

47 Comparative Study on the Effect of Endoscopic Inner Lining Repair on Hearing Recovery and Inflammatory Response in Patients with Chronic Suppurative Otitis Media

Objective Exploring the different effects of endoscopic inner lining tympanoplasty and traditional microscopic tympanoplasty on hearing recovery and inflammation control in patients with chronic suppurative otitis media. Method Using a random number table method,60 patients with chronic suppurative otitis media admitted to our hospital from September 2022 to September 2024 were divided into a control group and a study group,with 30 patients in each group.The control group patients received traditional microscopic tympanoplasty treatment,while the study group patients received ear endoscopic tympanoplasty treatment.Compare the perioperative indicators,pain levels within 24 hours after surgery,hearing recovery before and 2 months after surgery,as well as postoperative efficacy and complications between two groups of patients. Result Compared with the control group,the study group had a shorter surgical time(55.23±5.34)minutes,intraoperative blood loss(8.25±0.81)mL,and hospital stay(3.44±0.32)days(P<0.05);At 12 and 24 hours after surgery,compared with the control group,the VAS scores(3.58±0.31)and(2.26±0.21)in the study group were significantly lower(P<0.05);After surgery,the study group showed significantly better improvement in air conduction hearing valve(23.43±2.02)dB,bone conduction hearing valve(21.04±2.04)dB,and air bone conduction difference(10.68±4.21)dB compared to the control group,(P<0.05); The incidence of complications in the study group was significantly lower at 3.33% compared to the control group,and the total effective rate between the two groups was compared(P>0.05). Conclusion Compared with traditional microscopic tympanoplasty,otoendoscopic tympanoplasty can significantly compress the required time during the surgical process,reduce intraoperative bleeding,and alleviate postoperative pain and discomfort to the greatest extent possible.It also reduces the incidence of complications such as inflammation and infection,and is more helpful for patients' hearing recovery.

Objective To investigate the dynamic impact mechanism of different doses of Butylphthalide capsules on neural function recovery in patients with acute cerebral infarction. Methods This study included 120 patients with acute cerebral infarction admitted to our hospital's neurology department from January 2022 to January 2025.They were randomly assigned to a high-dose group(60 cases)and a low-dose group(60 cases)using a coin toss method.On the basis of conventional treatment,the high-dose group was given 200 mg of Butylphthalide soft capsules,while the low-dose group received 100 mg.The efficacy,hemorheological parameters,neurological function recovery,neuroregeneration factor levels,and adverse reactions were compared between the two groups. Results The clinical efficacy rate in the high-dose group(91.11%)was higher than that in the low-dose group(80.00%).After treatment,the erythrocyte aggregation index(2.32±0.44),whole blood viscosity(4.36±0.81 mPa.s),and plasma viscosity(1.44±0.42mPa.s)in the high-dose group were significantly lower than those in the low-dose group(P<0.05).The modified Rankin Scale(mRS)score in the high-dose group(1.98±0.68)was also significantly lower(P<0.05).Furthermore,the levels of brain-derived neurotrophic factor(BDNF)(3.50±1.28ng/L),nerve growth factor(NGF)(710.72±108.60ng/L),and gamma-aminobutyric acid(GABA)(4.26±0.74 μmol/L)in the high-dose group were significantly higher(P<0.05).Regarding adverse drug reactions,the incidence was 3.33% in the high-dose group,which was not statistically different from the low-dose group(P>0.05). Conclusion High-dose Butylphthalide capsules demonstrate better efficacy in treating acute cerebral infarction,significantly improving hemorheological parameters,promoting neurological function recovery,enhancing neuroregeneration-related factors,and showing a favorable safety profile.
