DOI: 10.3969/j.issn.1001-8131.2026.03.002
Objective To construct an early warning machine learning model for infectious diseases based on multidimensional parameters of complete blood count(CBC),optimize decision thresholds, and improve the accuracy and clinical utility of infection diagnosis. Methods A multicenter retrospective cohort study was conducted, collecting data from 5,317 suspected infectious disease patients in the emergency departments of three research centers from January 2023 to December 2024,including 1,683 cases in the infection group and 3,634 cases in the non-infection control group.CBC results within 24 hours of hospital admission were collected, and routine, derived inflammatory, and cell population data parameters were extracted to construct three hierarchical feature sets.LASSO regression combined with random forest was used to screen features, and the predictive performance of six machine learning algorithms was compared.Six threshold determination methods were systematically evaluated, with the introduction of clinical utility weighting functions and adaptive threshold strategies.Model performance was assessed through internal and external validation. Results The extended fusion hierarchical XGBoost model achieved an AUC of 0.874 on the internal validation set, superior to the basic fusion hierarchical model;the dynamic fusion hierarchical AUC increased to 0.891.The XGBoost model outperformed other algorithms. The model demonstrated stable performance in external validation.Subgroup analysis of bacteremia showed better predictive performance for Gram-negative bacteremia.SHAP analysis indicated that the top five most contributing features were procalcitonin, etc. The adaptive threshold strategy significantly improved the net recclassification index, with a low threshold in the high-risk group achieving a sensitivity of 91.23%.Conclusion The XGBoost model based on multidimensional parameter fusion of CBC can effectively predict infectious diseases, and the introduction of cell population data enhances model performance. The adaptive threshold strategy enables individualized early warning, demonstrating clinical translational potential to provide decision support for early infection identification in emergency departments and primary healthcare institutions.

Yu Qing, Lai Wen, Xu Zuqing, Fan Lumei
DOI: 10.3969/j.issn.1001-8131.2026.03.003
Objective Septic-associated encephalopathy(SAE)is a common cerebral dysfunction associated with septic shock, which can lead to neurological and cognitive impairments.Early and adequate fluid resuscitation during septic shock not only reduces the risk of mortality but also significantly improves medium-to-long-term neurological outcomes for patients with SAE.This study investigated the relationship between fluid resuscitation and cognitive prognosis in SAE patients via TCD(Transcranial Doppler ultrasonography),with implications for early intervention. Methods In this prospective cohort study,150 patients with septic shock were enrolled and categorised into SAE and non-SAE groups on the basis of clinical diagnostic criteria.SAE patients received standardised fluid resuscitation and were stratified into successful resuscitation and unsuccessful resuscitation subgroups.TCD was performed 24 hours,48 hours,1 week, and 2 weeks after admission to evaluate cerebral perfusion.Cognitive function was assessed on day 30 using the Mini-Mental State Examination(MMSE).Results TCD effectively identified SAE in septic shock patients.Significant differences in TCD-negative detection rates were observed between the successful and unsuccessful resuscitation subgroups at 48 hours,1 week, and 2 weeks(P<0.05).Statistically distinct MMSE scores were also noted at the 30-day follow-up.Conclusion Effective fluid resuscitation in septic shock patients has positive implications for neurological recovery and medium-to-long-term cognitive improvement.TCD exhibits favourable efficacy and operational feasibility in the evaluation of neurological outcomes for prognostic purposes, thus supporting its integration into clinical decision-making protocols.

Chen Zhuoqi, Chen Yongguo, Liu Jiawei, Lei Hua
DOI: 10.3969/j.issn.1001-8131.2026.03.005
Objective To evaluate the clinical efficacy of the basal anchoring with cavity compression closure technique in preventing postoperative dead space formation after resection of subcutaneous benign cystic-solid masses. Methods This study was a retrospective case series.A total of 42 consecutive patients with clinically and ultrasonographically diagnosed subcutaneous benign cystic-solid masses who met the inclusion criteria were treated with this technique between June 2024 and August 2025(all cases were postoperatively confirmed as benign). After complete mass excision and intradermal tension-reducing suture,a “U”-shaped non-absorbable anchoring suture was placed deep in the subcutaneous layer.A molded saline-moistened gauze bolster was placed over the incision, and the suture was tightened to generate vertical, uniform pressure, which was maintained for 2~3 days. The primary outcome measure was the success rate of dead space closure. Results Postoperative dead space closure was achieved in all 42 patients, with a success rate of 100%.No patient developed persistent seroma. The overall postoperative complication rate was 4.8%(2/42).Compared with conventional methods reported in the literature, this technique achieved superior outcomes in terms of seroma and overall complication rates. At the time of suture removal, the mean Vancouver Scar Scale(VSS)score was 2.8±1.5,and the mean Patient Scar Assessment Scale(PSAS)score was 18.5±7.2,indicating favorable early healing outcomes. Conclusion The basal anchoring with cavity compression closure technique is feasible, effectively ensures definitive dead space closure, and achieves reliable primary healing.Various strategies for seroma prevention described in the literature have inherent limitations, whereas this technique eliminates dead space at the source through active mechanical compression, demonstrating clear clinical advantages.Preliminary application suggests that it is both safe and effective.

Cao Jianing, Liu Jing, Han Lili
DOI: 10.3969/j.issn.1001-8131.2026.03.006
Objective To evaluate the diagnostic value of human papilloma virus(HPV)E6/E7 mRNA testing and PAX1/JAM3 gene promoter methylation analysis for high-grade cervical intraepithelial neoplasia(CIN2+),and to assess strategies for their combined use. Methods A total of 196 women with abnormal cervical screening results were enrolled. All participants underwent testing for HPV E6/E7 mRNA and PAX1/JAM3 promoter methylation.Using cervical biopsy histopathology as the gold standard, the positive rates of both markers were analyzed across different cervical lesion grades(including viralinfection,CIN I,CIN II,CIN III,and cervical cancer). The diagnostic performance of each marker alone and in combination was evaluated. Results The positive rates of both HPV E6/E7 mRNA and PAX1/JAM3 methylation increased significantly with the severity of cervical lesions(P<0.01).Multivariate logistic regression analysis identified both as independent risk factors for high-grade cervical lesions(CIN2+),with odds ratios(ORs)of 5.90 and 5.77,respectively.PAX1/JAM3 methylation had a greater specificity for diagnosing CIN2+(82.30%)and AUC(0.671).In the combination test,a serial(sequential)test approach greatly improved the specificity,up to 92.92%(AUC=0.687),while an approach in parallel significantly increased sensitivity up to 87.95%(AUC=0.674).Conclusion The HPV E6/E7 mRNA test has a high sensitivity and can be used as the primary method of cervical cancer screening;the PAX1/JAM3 methylation test has a high specificity, suggests considerable utility in triaging primary screen positive women.Combined usage of these two markers has complementary benefits, improving detection and accurate characterization of cervix pathology.

Sun Fei, Wang Zhuan
DOI: 10.3969/j.issn.1001-8131.2026.03.008
Objective To investigate the correlation between serum 25-hydroxyvitamin D [25-(OH)D]levels and the titers of The presence of thyroid peroxidase antibody(TPOAb)and thyroglobulin antibody(TgAb)in individuals with autoimmune thyroid disease(AITD),and to explore its clinical significance. Methods A total of 102 patients with AITD admitted to the Department of internal medicine of Wolong District Hospital of traditional Chinese medicine in Nanyang from June 2024 to December 2025 were selected retrospectively as the research group, the Control Group consisted of 102 healthy individuals who underwent physical examination in the same period.Serum levels of 25-(OH)D,TPOAb,and TgAb were detected and compared between the two groups. The Study Group was further divided into deficiency, insufficiency, and sufficiency subgroups based on 25-(OH)D levels. The differences in antibody titers among subgroups were compared, and the correlation between serum 25-(OH)D and TPOAb/TgAb was analyzed using Spearman correlation coefficient. Results A significantly reduced level of serum 25-(OH)D was observed in the study group relative to the healthy controls, while the levels of TPOAb and TgAb were higher than those in the control group, with statistically significant differences(P<0.05).Subgroup analysis showed that the levels of TPOAb and TgAb in the deficiency subgroup were significantly higher than those in the insufficiency subgroup and the sufficiency subgroup, and the insufficiency subgroup was also significantly higher than the sufficiency subgroup(P<0.05).Spearman correlation analysis indicated that the serum 25-(OH)D level in AITD patients was significantly negatively correlated with the titers of TPOAb and TgAb(P<0.001).Conclusions Vitamin D deficiency is prevalent in AITD patients.Low levels of 25-(OH)D are closely associated with high titers of thyroid autoantibodies, suggesting that Vitamin D may participate in the pathological process of AITD as an immunomodulator.Monitoring its level has important reference value for assessing disease condition and guiding treatment.
