导管定向硬化治疗与手术切除治疗卵巢子宫内膜异位症疗效及对卵巢储备功能影响的比较

摘要目的比较导管定向硬化剂（CDS）与手术治疗卵巢子宫内膜异位症的疗效及其对卵巢储备功能的影响。方法选取160例卵巢子宫内膜异位症患者,依据双盲法分为观察组（80例）与对照组（80例）。观察组予以CDS治疗,对照组行手术切除治疗。比较患者手术前后的血清抗苗勒管激素（AMH）水平,观察并记录患者血清癌抗原125（CA-125）水平、病灶大小、复发情况和术后并发症。结果观察组术前血清AMH（3.42±1.13）ng/mL与术后血清AMH（3.37±1.39）ng/mL对比差异无统计学意义（P>0.05）。观察组术后血清CA-125（26.05±7.85）U/mL、病变损伤大小（20.06±2.97）mm显著低于术前血清CA-125（63.65±20.91）U/mL、病变损伤大小（48.62±4.20）mm,差异具有统计学意义（P<0.05）。对照组术后血清AMH（1.78±0.64）ng/mL、血清CA-125（8.96±1.47）U/mL显著低于术前血清AMH（3.21±1.13）ng/mL、血清CA-125（60.01±22.89）U/mL,差异具有统计学意义（P<0.05）。观察组症状缓解患者人数、出现并发症患者人数与对照组症状缓解患者人数、出现并发症患者人数对比差异无统计学意义（P>0.05）。观察组住院时间、复发患者人数显著低于对照组住院时间、复发患者人数,差异具有统计学意义（P<0.05）。结论 CDS治疗与手术切除卵巢子宫内膜异位症疗效相似,但CDS方式卵巢储备功能更好,住院时间短,值得临床推广。

Abstract：Objective To compare the effect of catheter directed sclerosing agent（CDS）and surgical treatment on ovarian endometriosis and its effect on ovarian reserve function. Methods A total of 160 patients with ovarian endometriosis were selected and divided into observation group（80 cases）and control group（80 cases）according to double-blind method.The observation group was treated with CDS,and the control group was treated with surgical resection.Serum anti-Mullerian hormone（AMH）levels were compared before and after surgery.Serum cancer antigen 125（CA-125）levels,lesion size,recurrence and postoperative complications were observed and recorded. Results There was no significant difference in serum AMH between the observation group before operation（3.42±1.13）ng/ml and the observation group after operation（3.37±1.39）ng/mL（P>0.05）.The serum CA-125（26.05±7.85）U/mL and lesion size（20.06±2.97）mm in the observation group after operation were significantly lower than those before operation（63.65±20.91）U/mL and lesion size（48.62±4.20）mm in the observation group.The difference was statistically significant（P<0.05）.Serum AMH（1.78±0.64）ng/ml and serum CA-125（8.96±1.47）U/ml in control group were significantly lower than those before operation（3.21±1.13）ng/ml and serum CA-125（60.01±22.89）U/mL in control group.The difference was statistically significant（P<0.05）.There were no significant statistical differences in the number of patients with symptom remission and complications in the observation group and the control group（P>0.05）.The length of stay and the number of recurrent patients in the observation group were significantly lower than those in the control group,with statistical significance（P<0.05）. Conclusion CDS treatment is similar to surgical resection of ovarian endometriosis,but CDS method has better ovarian reserve function and shorter hospitalization time,which is worthy of clinical promotion.
