1 Ultrasound-mediatedmiR-21and miR-126overcome Paclitaxel-resistant Breast Cancer

Objective To investigate the effects of ultrasound-mediatedmiR-21 and miR-126 on paclitaxel resistance in breast cancer. Methods 50 subcutaneous breast cancer transplantation tumormodels established by drug-resistant breast cancer cells were selected as research objects.Ultrasonicmicrovesicles loaded withmiR-21 and miR-126 were prepared and randomly divided into control group,empty microvesicles group,miR-21microvesicles group,miR-126microvesicles group and combination group with 10mice in each group.Tumor growth ofmice in each group was observed.Tumor growth curve was plotted and tumor inhibition rate was calculated.Real-time fluorescence quantitative RT-PCR was used to detect the levels ofmiR-21 andmiR-126 in tumor tissues,and immunohistochemistry was used to detect the protein expression levels of breast cancer drug-resistant protein(BCRP),p-glycoprotein(P-gp)and drug-resistant drug-related protein 1(mRP1)in tumor tissues. Results Compared with the control group and the vacuole group,the miR-21mRNA expression level in themiR-21microbubble group and the combined miR-126microbubble group was significantly down-regulated(P<0.05),while the miR-126microbubble group and the combinedmiR-126mrna expression level was significantly up-regulated(P<0.05).The tumor volume in the control group and the vacuole group continued to increase.Compared with the control group and the vacuole group,the combined group had the smallest tumor volume andmass,and the highest tumor volume andmass inhibitory rate(P<0.05),followed by themiR-21 microvesicle group andmiR-126microvesicle group(P<0.05).Immunohistochemical results showed that the positive rates of BCRP,P-GP andmRP1 in the combination group were 20.00%,10.00% and 20.00%,respectively,which were significantly lower than those ofmiR-126microbubble group andmiR-21microbubble group(P<0.05). Conclusion The combined intervention ofmiR-21 andmiR-126mediated by ultrasonicmicrovesicle has a synergistic inhibitory effect on the growth of subcutaneous graft tumors in paclitaxel-resistantmice,providing a new therapeutic approach and target for overcoming paclitaxel-resistant breast cancer.

5 Effect of Bilevel Positive Airway Pressure Ventilation Combined with Methylprednisolone in The Treatment of Acute Exacerbation of Chronic Obstructive Pulmonary Disease with Respiratory Failure

Objective To investigate the effect of BiPAP combined with methylprednone in the treatment of acute exacerbation of chronic obstructive pulmonary disease(AECOPD)with respiratory failure. Methods 72 patients with AECOPD combined with respiratory failure admitted were divided into 2 groups.The control group was given BiPAP therapy,and the observation group was combined with methylprednone therapy.The clinical efficacy,respiratory function,lung function,blood gas level and inflammatory factor level were compared. Results The total effective rate of the observation group was significantly higher than that of the control group(P<0.05),but there were no significant differences in the levels of RR,HR,MAP,PaO2,PaCO2,SaO2,FEV1,FVC,FEV1/FVC,PCT and CRP between the two groups before treatment(P>0.05).After treatment,the levels of RR,HR,MAP,PaO2,PaCO2,SaO2,FEV1,FVC,FEV1/FVC,PCT and CRP in 2 groups were significantly improved.After treatment,RR,HR,MAP,PaO2,PaCO2,SaO2,FEV1,FVC,FEV1/FVC,PCT and CRP in observation group were significantly improved(P<0.05). Conclusion Bilevel positive airway pressure ventilation combined with methylprednone in the treatment of AECOPD patients with respiratory failure can significantly improve clinical efficacy,inhibit airway and lung inflammation levels,improve lung function,and accelerate the recovery of patients,which is worthy of promotion.

8 Toxicity Assessment and Safety Experimental Study of the Drug Bevacizumab

The article discusses the importance of drug toxicity assessment and how to reduce drug toxicity through modifications of chemical structures.It details an experiment on the toxicity assessment of an anticancer drug(Bevacizumab).The experiment used both animal models and cell culture methods to comprehensively evaluate the safety of Bevacizumab.The results showed that Bevacizumab exhibited certain toxicity to rats and human liver cell culture systems at high doses or concentrations.It is recommended that further drug development should involve optimization of chemical structures to reduce potential toxicity risks.This research not only provides important safety data for further drug development but also offers a comprehensive experimental model for drug toxicity assessment.

11 Feasibility Analysis of 5mm Balloon Pre-dilation in the Treatment of Severe Carotid Stenosis

Objective To analyze the feasibility of 5mm balloon pre-dilation in the treatment of severe carotid stenosis. Methods The data of 83 patients with severe carotid stenosis were reviewed.All patients received balloon dilation + carotid artery stent implantation.Patients were divided into group A and group B according to the diameter of the pre-dilated balloon.Group A used 4mm balloon(43 cases)and Group B used 5mm balloon(40 cases).The differences of operation,hemodynamics(PSV,EDV),endothelial function(ET-1,vWF),perioperative complications and prognosis between the two groups were observed. Results There was no significant difference in the success rate of operation between the two groups(P>0.05),and the rate of residual stenosis in group B was lower than that in group A,the difference was statistically significant(P<0.05).One day after operation,PSV and EDV in both groups were lower than those in the same group before operation,and group B was lower than group A,the difference was statistically significant(P<0.05).vWF and ET-1 of the two groups were higher than those of the same group before surgery,with statistical significance(P<0.05),but there was no statistical significance between the two groups(P>0.05).There were no significant differences in perioperative complications and prognosis between the two groups(P>0.05). Conclusions Stent interventional therapy after 5mm balloon pre-dilation can effectively improve the stenosis in patients with severe carotid stenosis,the residual stenosis rate is lower than that of conventional 4mm,and the hemodynamic improvement is better.At the same time,there is no significant difference in endothelial function and safety performance,which has high clinical feasibility.

14 Serum AFP β- Analysis of the Screening Value of hCG,uE3 Combined with miR-146a-5p Detection For Down's Syndrome in Mid Pregnancy

Objective To analyze alpha fetoprotein(AFP)β- Human ch orionic gonadotropin(β- HCG),free Estriol(uE3)and microRNA-146a-5p(miR-146a-5p)were used to screen Down's syndrome in the second trimester of pregnancy. Methods 53 pregnant women in the second trimester with positive results of Down's syndrome screening were divided into the observation group,and 64 healthy pregnant women in the second trimester who underwent physical examination at the same time were divided into the control group β-HCG,uE3,miR-146a-5p differences;Draw the receiver operating curve(ROC)and analyze the effectiveness of combined detection of AFP,β-hCG,uE3,and miR-146a-5p in screening for Down's syndrome. Results showed that the AFP and uE3 levels in the observation group were(20.53±2.81)μg/L and(1.46±0.34)ng/mL,respectively,lower than those in the control group(46.79±4.57)μg/L and(3.49±0.50)ng/mL,The β-hCG was(6.77±1.54)μg/L,and the miR-146a-5p was(3.24±0.67),which was higher than the control group(3.15±0.63)μg/L and(1.25±0.31),with a statistical difference(P<0.05);ROC results display:The area under the curve(AUC)of AFP,β-hCG,uE3,and miR-146a-5p combined detection for Down's syndrome was 0.907(95% CI:0.848-0.967),which was higher than the screened separately 0.740(95%CI:0.650-0.830),0.844(95%CI:0.767-0.921),0.822(95%CI:0.746-0.899),and 0.839(95%CI:0.754-0.923). Conclusion The combination of AFP,β-hCG,uE3,and miR-146a-5p can effectively screen for Down's syndrome in mid pregnancy,and has high practical value.

17 Analysis of the Test Results and Clinical Significance of Four Indicators of D-dimer and Blood Coagulation in Early,Middle and Late Pregnancy of Pregnant Women

Objective To analyze the test results and clinical significance of D-dimer and blood coagulation in early,middle and late pregnancy. Methods 247 pregnant women were selected as the research subjects.The selected women were counted and divided into early pregnancy group(67 cases),mid pregnancy group(81 cases),and late pregnancy group(99 cases)based on their pregnancy period.50 healthy non pregnant women who underwent physical examination in the hospital during the same period were included as the control group.Compare the test results of four indexes of D-dimer and coagulation(fibrinogen,thromboplastin time(TT),activated partial thrombin time(APTT)and Prothrombin time)in four groups,and analyze the test results and clinical significance of four indexes of D-dimer and coagulation in early,middle and late pregnancy of pregnant women. Results D-dimer and fibrinogen in the third trimester group were higher than those in the first trimester group and the second trimester group,and D-dimer and fibrinogen in the second trimester group were higher than those in the first trimester group;The TT,APTT,and PT levels in the late pregnancy group were lower than those in the early and mid pregnancy groups.The TT,APTT,and PT levels in the mid pregnancy group were lower than those in the early pregnancy group,and all three groups were lower than the control group,with statistically significant differences(F=539.341,90.144,73.226,32.676,93.263,P<0.05).Pearson correlation analysis showed that D-dimer and fibrinogen were positively correlated with pregnancy progress(r=0.898,0.679,P<0.05);TT,APTT,PT were negatively correlated with pregnancy progression(r=0.649,0.497,0.693,all P<0.05). Conclusion The results of D-dimer and blood coagulation of pregnant women in early,middle and late pregnancy are quite different.Regular detection of pregnant women can effectively monitor the status of blood coagulation function in different pregnancy periods,and comprehensively warn of blood coagulation dysfunction and the risk of Deep vein thrombosis in middle and late pregnancy women.

20 To Compare the Effects of Hysteroscopic Resection after Fenestration and Direct Resection on the Reproductive Function of Patients with Submucous Myoma of Uterus

Objective To analyze the effects of hysteroscopic electrotomy and direct resection on the reproductive function of patients with submucous myoma of uterus. Methods 110 patients with submucous myoma of uterus were enrolled,55 patients in the control group and 55 patients in the experimental group were designed by complete randomization.The control group was given the direct resection method of hysteroscopic electrotomy(Shorthand:direct excision method),while the experimental group was given the resection method after opening the window of hysteroscopic electrotomy(Shorthand:fenestration).The changes of ovarian function related indexes,the incidence of intrauterine adhesion and pregnancy status were compared between the two groups. Results There was no significant difference in ovarian function between the experimental group and the control group(P>0.05).The incidence of intrauterine adhesions in the experimental group was 9.09%,3.64% and 0.00% at 1,3 and 6 months after operation,which was lower than that in the control group(23.64%,14.55% and 9.09%)(P<0.05).The incidence of intrauterine adhesions in the experimental group was 0.00% 12 months after operation,which was lower than that in the control group(3.64%),with no significant difference(P>0.05).The pregnancy rate in the experimental group was 58.18%,which was higher than that in the control group(32.73%).The incidence of complications in the experimental group was 3.64%,which was lower than that in the control group(14.55%)(P<0.05).The first pregnancy time after operation in the experimental group was(15.82 3.48)months,which was not significantly different from that in the control group(16.13 3.45)months(P>0.05). Conclusion Hysteroscopic resection of submucous myoma after fenestration is more effective than direct resection,which can effectively improve its reproductive function and reduce the incidence of complications,which is beneficial to prognosis and worthy of popularization and application.

23 Clinical Observation of the Combination of Oxytocin and Suppository for Pregnancy in Treating Postpartum Hemorrhage after Natural Delivery

Objective To investigate and analyze the clinical effect of combined use of oxytocin and pregnancy-suppository on postpartum hemorrhage after natural delivery. Methods A total of 120 cases of postpartum postpartum hemorrhage were selected and divided into control group 1,control group 2 and experimental group.Control group 1 only received oxytocin treatment,control group 2 only received pregnancy suppository treatment,and the experimental group received combined treatment with oxytocin and pregnancy suppository.The amount of blood loss,hemostatic time,hemostatic effect,coagulation factor level before and after treatment and the occurrence of adverse reactions were compared among the three groups. Results Postpartum blood loss and hemostatic time were significantly different among the three groups(P<0.05).Postpartum blood loss and hemostatic time were significantly reduced in the experimental group compared with control group 1 and control group 2(P<0.05).The total effective rate of hemostatic effect was significantly different among the three groups(P<0.05).The total effective rate of hemostasis in the experimental group was 97.50%,which was significantly higher than that in control group 1(65.00%)and control group 2(62.50%)(P<0.05).There were no significant differences in the levels of coagulation factor Ⅶ,FIB,PTT,PT and PLT in the three groups before treatment(P>0.05).The levels of coagulation factor Ⅶ,FIB,PTT,PT and PLT in the three groups were significantly improved after treatment,and the levels of coagulation factor Ⅶ,FIB,PTT,PT and PLT in the experimental group were more significantly improved than those in control group 1 and control group 2(P<0.05).The incidence of adverse reactions during medication in experimental group,control group 1 and control group 2 was 10.00%,20.00% and 17.50%,respectively,with no statistical significance(P>0.05). Conclusion The combination of oxytocin and pregnancy suppository plays a synergistic role in the treatment of postpartum postpartum hemorrhage,with significant complementary effect,improving maternal coagulation function and shortening hemostasis time,good safety,worthy of promotion.

26 Correlation between Serum CEA,CA125 and AFP the Illnesscondition of Chronic Hepatitis B

Objective To investigate the correlation between serum carcinoembryonic antigen(CEA),carbohydrate antigen 125(CA125),and alpha fetoprotein(AFP)and the illnesscondition of chronic hepatitis B(CHB). Methods 78 cases of patients with CHB in our hospital were selected,and they were divided into a mild group(n=26),a moderate group(n=26),and a severe group(n=26)based on the severity degree of the irillness condition.In addition,26 healthy individuals in the same period were selected as the control group.All personnel were tested for serum CEA,CA125,and AFP levels,the patients' liver and kidney functions were recorded,and the scores of Model for End Stage Liver Disease(MELD)were analyzed.The positive detection rates of serum CEA,CA125,and AFP were analyzed,and the levels of serum CEA,CA125,AFP,and as well as MELD scores among the groups were compared to analyze the correlation. Results There were no significant differences in the detectable rate of CEA positive 3.85%(1/26)and detectable rate of CA125positive7.69%(2/26)in the severe group compared with 3.85%(1/26)and 7.69%(2/26)in the moderate group and 3.85%(1/26)and 3.85%(1/26)in the mild group(P>0.05).The detectable rate of AFP positivein severe group was 50.00%(13/26),which was higher than 26.92%(7/26)in the moderate group and 19.23%(5/26)in the mild group(P<0.05);There were no significant differences in serum CEA and CA125 levels among the severegroup,the moderategroup,the mildgroup,and the control group(P>0.05);Compared with the control group,the AFP levels and MELD scores in the severegroup,the moderategroup,and the mild group were higher(P<0.05);Pearson analysis showed that serum AFP level(r=0.658,r=0.671)was positively correlated with MELD scores and severity degree of illnessconditionof chronic hepatitis B(P<0.05). Conclusion The serum AFP level will gradually increase with the aggravation of CHB patients,and there is a positive correlation between the AFP level and the severity of illnesscondition of CHB.

29 Effect of Glycine Theophylline Sodium Sustained-release Tablets on Bronchial Asthma and Its Influence on Oxidative Stress Index

Objective To understand the effect of glycine theophylline sodium sustained-release tablets in the treatment of bronchial asthma and its influence on oxidative stress indicators. Methods 100 patients with asthma were selected as study samples.The patients were divided into two groups by systematic allocation method(50 cases in the observation group and 50 cases in the control group).The observation group was treated with routine treatment plus glycine theophylline sodium sustained-release tablets,while the control group was treated with routine treatment. Results The effective rate of treatment in the observation group(96.00%)was higher than that in the control group(82.00%)(P<0.05).The time of clinical symptom disappearance in the observation group was shorter than that in the control group(P<0.05).After intervention,the indexes of lung function in the observation group were significantly better than those in the control group(P<0.05).After intervention,the inflammatory factors in the observation group were better than those in the control group(P<0.05).After intervention,the oxidative stress index in the observation group was better than that in the control group(P<0.05). Conclusion The effect of glycine theophylline sodium sustained-release tablets in the treatment of bronchial asthma is significant,reducing inflammatory factors,improving lung function and optimizing oxidative stress indicators.

32 Endoscopy Assisted Sheath in the Application of Hypertension Cerebral Hemorrhage Brain Hematoma Removal

Objective Analysis of endoscope assisted sheath in hypertensive intracerebral hemorrhage(HICH)the application of brain hematoma removal. Methods A study was conducted on 89 patients with hypertensive intracerebral hemorrhage .The patients were divided into two groups based on whether a new adjustable neuroendoscope-assisted sheath was used during the hematoma clearance surgery:the sheath group(n=47)and the non-sheath group(n=42).The study aimed to compare the surgical conditions,clinical indicators,complications,and prognosis of the two groups. Results The surgical time and intraoperative blood loss of the sheath group were less than those of the non-sheath group,and the hematoma clearance rate was significantly higher than that of the non-sheath group(P<0.05).Compared with before the surgery,the sepsis organ function failure score(SOFA),the Acute Physiology and Chronic Health Evaluation Ⅱ(APACHE-Ⅱ),and the National Institutes of Health Stroke Scale(NIHSS)score of both groups significantly decreased after the surgery,and the scores in the sheath group were lower than those in the non-sheath group(P<0.05).The incidence of intracranial infection,epilepsy,pulmonary infection,and stress ulcers in the sheath group was significantly lower than that in the non-sheath group(P<0.05).The prognoses of patients in the sheath group with grades Ⅱ-V were 3,9,21,and 14,respectively,while those in the non-sheath group were 11,15,11,and 5,respectively.The difference between the two groups in prognosis was statistically significant(P<0.05). Conclusion The clinical practical value of the new adjustable neuroendoscopic-assisted sheath in the treatment of HICH intracerebral hematoma clearance surgery is high,which can expand the clinical application range and improve the clinical surgical treatment efficiency.

35 Effect of Dexmedetomidine Preemptive Analgesia on Serum Inflammatory Factors and Hemodynamics in Severely Burned Patients

Objective To analyze the application effect of dexmedetomidine preemptive analgesia in severe burn patients. Methods 86 severe burn patients diagnosed and treated were selected and randomly divided into two groups,with 43 patients in each group.Observation group injected 10 minutes before anesthesia 1μg/kg dexmedetomidine,followed by 0.5 μg Pump at a rate of 1g/(kg·h),discontinue medication 30 minutes before surgery,and inject the same amount of physiological saline into the control group in the same manner.Compare the sedative and analgesic effects,serum inflammatory factor levels,and hemodynamics between the two groups. Results The analgesic onset time of the observation group was(2.06±0.41)minutes,shorter than the control group's(4.89±0.95)minutes,the analgesic duration was(123.48±10.67)minutes,and longer than the control group's(95.31±8.46)minutes,with a statistical difference(P<0.05);Before anesthesia(T0),two groups of C-reactive protein(CRP)and tumor necrosis factor- α(TNF- α)、There was no statistically significant difference in levels of interleukin-6(IL-6),heart rate(HR),and mean arterial pressure(MAP)(P>0.05);30 minutes after the start of surgery(T1)and at the end of surgery(T2),the observation group showed CRP and TNF- α、 IL-6,HR,and MAP were lower than the control group,with statistical differences(P<0.05). Conclusion Dexmetomidine can exert preemptive analgesic effects in severe burn patients,reduce inflammatory reactions,stabilize patient hemodynamics,and has high clinical application value.It can be vigorously promoted and applied.

38 Clinical Characteristics of Children with Short Stature and the Effect of Recombinant Human Growth Hormone Combined with Letrozole on p21waf/cip1,25-OH-D Levels

Objective To investigate the clinical value of recombinant human growth hormone(RH GH)combined with letrozole in the treatment of idiopathic short stature(ISS)and its effect on the levels of p21waf/cip1 and 25 hydroxy Vitamin D(25-OH-D)in children. Methods 86 children with ISS were selected and randomly divided into control group and study group,43 cases each;The control group was given RH GH,and the study group was treated with letrozole for 12 months.Bone age,PINP,predicted lifetime height,p21waf/cip1,bone alkaline phosphatase(BALP),25-OH-D,osteocalcin(OC),clinical efficacy and adverse reactions were compared between the two groups. Results The excellent and good rate of curative effect in the study group(95.35% vs 76.74%)was higher than that in the control group(P<0.05).Before treatment,there was no significant difference in bone age,PINP,predicted final height,BALP,OC,p21waf/cip1 protein,25-OH-D between the two groups(P>0.05);After 12 months of treatment,the PINP,bone age,25-OH-D,OC and predicted final height of the study group were larger than those of the control group,and the p21waf/cip1 protein and BALP of the study group were smaller than those of the control group,with statistical significance(P<0.05).There was no significant difference in the total adverse reaction rate between the two groups(11.63% vs 6.98%)(P>0.05). Conclusion Combined with letrozole on the basis of RH GH has a good effect on ISS,which can effectively improve the bone metabolism index and 25-OH-D,p21waf/cip1 levels of children,promote the height growth and bone development of children,and improve the lifelong height value of patients.

41 T2DM Comparison of the Hypoglycemic Effect of Daggliptin and Liggliptin in Obese T2DM Patients with Poor Insulin Control

Objective To compare the hypoglycemic effect of daggliptin and liggliptin in obese T2DM patients with poor insulin control. Methods A total of 74 patients diagnosed with obese T2DM were selected as the study subjects.They were randomly divided into dagliazine group and lipliptin group,with 37 cases in each group.Both groups were treated with dagliazine or lipliptin,respectively.Fasting blood glucose(FBG),postprandial 2h blood glucose(2h PBG)and glycosylated hemoglobin(HbA1c)were compared between the two groups.Body mass index(BMI),waist circumference(WC)and body FAT percentage(FAT)were compared between the two groups. Results After intervention,the level of lipid metabolism in both groups decreased compared with that before intervention.However,the FBG(8.31±; 0.71),2h PBG(9.13±; 1.63)and HbA1c(8.86±; 0.82)in Ligliptin group were significantly better than those in Dagliegine group(9.78±; 0.91),(10.37±; 1.94)and(8.11±; 0.45).Obesity indexes in both groups were improved,but the decreasing trend in the lipliptin group was better than that in the dagliegine group,with statistical significance(P<; 0.05). Conclusion Daggliptin and liggliptin can effectively control blood sugar and reduce the weight of obese T2DM patients with poor insulin control,which is worthy of clinical application.

Elemental impurities affect the safety and efficacy of drugs, and the International Council for Harmonization of Drugs for Human Use (ICH) released the final version of Q3D(R2): Guiding Principles for Elemental Impurities in April 2022. In this paper, we analyze the elemental impurities in chemical APIs with reference to Q3D(R2), identify the possible elemental impurities from various potential sources, and discuss the control strategies of elemental impurities in each link from the production perspective, which provides new research ideas for the follow-up research on drug R&D, production, quality control and marketing changes.

47 Analysis of the Diagnostic Value of Low-frequency Combined with High-frequency Ultrasound in Gallbladder Polypoid Lesions

Objective To explore the diagnostic value of low-frequency combined with high-frequency ultrasound in gallbladder polypoid lesions(PLG),in order to provide scientific basis for accurately formulating prevention and treatment strategies in clinical practice. Methods 93 PLG patients were selected as the survey subjects,all of whom underwent low-frequency ultrasound,low-frequency combined high-frequency ultrasound examination,and surgical pathology examination.Record the surgical pathological results,count the detection results of PLG by low-frequency ultrasound alone or low-frequency combined high-frequency ultrasound,and use the surgical pathological results as the gold standard for PLG diagnosis.Analyze the diagnostic efficacy of low-frequency ultrasound and low-frequency combined high-frequency ultrasound in detecting PLG,and analyze the consistency between the results of low-frequency ultrasound and low-frequency combined high-frequency ultrasound in diagnosing PLG and the pathological results. Results Among the 93 PLG patients,77(82.80%)were found to be benign PLG and 16(17.20%)were found to be malignant PLG through surgical and pathological examination;The diagnostic sensitivity of low-frequency combined high-frequency ultrasound examination for PLG(93.75%)was slightly higher than that of low-frequency ultrasound examination alone(75.00%),and the difference was not statistically significant(P>0.05);The diagnostic specificity(92.21%)and accuracy(92.47%)of low-frequency combined high-frequency ultrasound examination for PLG were higher than those of low-frequency ultrasound examination alone(71.43%,72.04%),and the difference was statistically significant(P<0.05);The consistency between low-frequency ultrasound diagnosis of PLG and surgical pathological results was poor(Kappa=0.321,P=0.000),while the combination of low-frequency and high-frequency ultrasound diagnosis of PLG showed high consistency with surgical pathological results(Kappa=0.765,P=0.000). Conclusion The application value of low-frequency combined with high-frequency ultrasound in clinical diagnosis of PLG is high,which can reduce misdiagnosis and missed diagnosis,and provide accurate basis for clinical prevention and treatment strategies.

89 Application of Video EEG in Early Diagnosis and Prognosis Evaluation of Epilepsy Secondary to Cerebral Infarction

Objective To analyze the efficacy of video EEG in the early diagnosis of epilepsy secondary to cerebral infarction and its prognostic value. Methods 110 patients with epilepsy secondary to cerebral infarction were studied,and they were followed up for one year.Routine electroencephalogram and video electroencephalogram were performed.The detection rate of abnormal electroencephalogram was compared between the two methods,and the detection rate of epileptic focus in different infarcts was counted;Then the patients were divided into groups according to whether they had relapse or not,and the initial video EEG signals and medication were observed in the two groups. Results The detection rate of abnormal EEG in video EEG was higher than that in routine EEG,and the detection rate of abnormal EEG in epilepsy without cerebral cortex infarction was higher than that in routine EEG;The detection rate of diffuse slow waves in video EEG in the epilepsy recurrence group was lower than that in the non recurrence group,and the detection rate of abnormal epileptiform discharge waveform was higher than that in the non recurrence group,with statistical significance(P<0.05). Conclusion It is feasible to use video EEG to diagnose epilepsy secondary to cerebral infarction,especially to effectively detect the epilepsy focus formed after deep cerebral infarction,and to evaluate the prognosis of the disease according to epileptic discharges.

119 Analysis of the Effect of Chinese Herbal Foot Bath Combined with Acupressure on Sleep Quality in Patients with Achalasia with Sleeplessness

Objective To evaluate the effect of Chinese medicine foot bathing combined with acupressure on the sleep quality of patients with hypersomnia. Methods 120 patients with achromatopsia with insomnia were selected.Patients were randomly divided into the herbal foot bath combined with acupressure treatment group(combined group,n=60)and the control group(n=60).The control group was treated with conventional Western medicine,while the combined group was treated with traditional Chinese medicine foot massage combined with acupoint application.The blood glucose index,Pittsburgh Sleep Quality Index,and TCM evidence score were compared between the two groups before and after treatment.The clinical efficacy and incidence of adverse reactions were compared between the two groups. Results Fasting blood glucose(t=4.743,P<0.001)and 2h postprandial blood glucose(t=6.734,P<0.001)were significantly lower in the combined group after treatment compared with the values in the control group during the same period.The Pittsburgh Sleep Quality Index was significantly lower in the combined group compared to the control group(t=4.717,P<0.001).The overall effective rate was significantly higher in the combined group compared to the control group[93.3%(56/60)vs.80.0%(48/60),χ2=4.615,P=0.032].The TCM evidence score was significantly lower in the combined group after treatment compared to the control group during the same period(t=0.120,P<0.001).The incidence of adverse reactions was significantly lower in the combined group compared to the control group[6.7%(4/60)vs.55.0%(33/60),χ2=32.862,P<0.001]. Conclusion Patients with diabetes mellitus with insomnia showed significant improvement in blood glucose,sleep quality and TCM evidence score,significantly improved clinical efficacy and significantly reduced incidence of adverse effects after conventional Western medical treatment with the combination of Chinese herbal foot bath and acupressure.

Acute pesticide poisoning is a common life poisoning with high mortality,especially chlorfenapyr poisoning.Chlorfenitrile,as a highly effective and broad-spectrum arylpyrrole insecticide,is widely used in agriculture.It can be absorbed through the digestive tract,skin,respiratory tract and eyes,among which the digestive tract is the most important absorption mode.Once it enters the human body,it can interfere with the normal metabolic process of cells,leading to cell energy metabolism disorders,triggering total hyperthermia response,interfering with nerve signal transmission,leading to serious damage to the function of the nervous system,and the potential harm to the environment and human health cannot be ignored.This study reviewed the mechanism,clinical manifestations,diagnostic strategy,treatment strategy,prognosis assessment,acute poisoning treatment system construction and other aspects of the latest research progress,aimed at providing clinicians with comprehensive understanding and prevention strategies.This paper also discusses the necessity of establishing and improving the regional poisoning treatment system in order to improve the safe use level of chlorfenapyr and the success rate of treatment.
