Study on the Relationship between miRNA-146a Expression in Peripheral Blood Mononuclear Cells and Oxidative Stress Levels and Inflammatory Response in Patients with Chronic Obstructive Pulmonary Disease

Objective
The primary objective of this study was to explore the intricate relationship between the expression of miRNA-146a in peripheral blood mononuclear cells(PBMCs) of patients afflicted with chronic obstructive pulmonary disease(COPD)and both the oxidative stress levels and the inflammatory response.
Methods
Peripheral blood samples were procured from 52 individuals diagnosed with COPD and 52 healthy counterparts.The expression of miRNA-146a in PBMCs was detected using real-time fluorescence quantitative PCR technology.Biochemical assay kits were employed to ascertain the levels of oxidative stress markers,and ELISA was utilized to measure the levels of inflammatory response markers.Subsequently,a comprehensive comparison of the correlations among these indicators was conducted.
Results
In contrast to the healthy control group,a significant reduction in the expression level of miRNA-146a in the PBMCs of COPD patients was observed(

0.05).Notably,as the GOLD grading advanced,a downward trend in the expression level of miRNA-146a became evident.The levels of malondialdehyde(MDA)in the plasma of COPD patients were markedly higher than those in the healthy control group(
P<
0.05),while the levels of superoxide dismutase(SOD)and glutathione peroxidase(GSH-Px)were substantially lower than those in the healthy control group(
P<
0.05).Additionally,the levels of interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),and interleukin-1β(IL-1β)in the plasma of COPD patients were significantly elevated compared to those in the healthy control group(
P<
0.05).Intriguingly,the expression level of miRNA-146a in the PBMCs of COPD patients exhibited a negative correlation with MDA levels(both
P<
0.05),and a positive correlation with SOD and GSH-Px levels(
P<
0.05).Concurrently,the expression level of miRNA-146a was inversely correlated with the levels of IL-6,TNF-α,and IL-1β(all
P<
0.05).
Conclusion
The expression level of miRNA-146a is intimately intertwined with the severity of COPD and demonstrates a significant correlation with oxidative stress levels and inflammatory response indicators.It is postulated that miRNA-146a might play a crucial role in the pathogenesis of COPD.

To Evaluate the Therapeutic Effect of Low Molecular Weight Heparin on Recurrent Abortion Susceptibility Based on Coagulation Function and Platelet Level

Evaluation Value Analysis of NIHSS Score Combined with Plasma Copeptin and D-dimer in Patients with Acute Ischemic Stroke

Objective
To explore the value of National Institutes of Health Stroke Scale(NIHSS)score combined with plasma and peptidogysin(Copeptin)and D-dimer for disease progression in patients with acute ischemic stroke(AIS).
Methods
90 AIS patients were selected for the study.According to the NIHSS score,they were mild into moderate group(15 points,n=67)and severe group(>15 points n=23),and 40 healthy patients included in the physical examination in the same period were healthy group.Comparing differences in NIHSS scores,plasma Copeptin and D-dimer levels,analyzed the correlation between Copeptin and D-dimer levels and disease progression in AIS patients,binary logistic analyzed the relationship between Copeptin and D-dimer levels and disease progression,and ROC analyzed the value of Copeptin and D-dimer on disease progression in AIS patients.
Results
NIHSS score and Copeptin and D-dimer levels in AIS and severe group were higher than healthy and mild to moderate groups(

0.05);Copeptin,D-dimer levels were positively correlated with disease progression in AIS patients(

0.05);Binary logistics regression analysis showed that,Copeptin And D-dimer levels were related factors affecting disease progression in AIS patients(

Conclusion
The plasma levels of Copeptin and D-dimer in AIS patients are high,which are correlated with admission NIHSS score,are the relevant influencing factors of disease progression,and have certain reference value in clinical diagnosis.

Distribution Characteristics of Pathogens and Risk Factors of Ventilator-Associated Pneumonia in Patients with Acute Exacerbation of COPD

Objective
To investigate the distribution characteristics of pathogens and risk factors of ventilator-associated pneumonia in patients with acute exacerbation of chronic obstructive pulmonary disease(COPD).
Methods
74 patients with acute exacerbation of COPD complicated with ventilator-associated pneumonia and 74 patients without ventilator-associated pneumonia were selected as study subjects from our hospital and were divided into the ventilator-associated pneumonia group and the control group.Deep respiratory tract secretions were collected for pathogenic microorganism typing and identification.Demographic characteristics and clinical data were collected,and the levels of serum C-reactive protein,tumor necrosis factor-α,and procalcitonin were measured.
Results
A total of 86 pathogens were detected in patients with acute exacerbation of COPD complicated with ventilator-associated pneumonia,including 54 Gram-negative bacilli(62.79%),with Pseudomonas aeruginosa,Acinetobacter baumannii,Klebsiella pneumoniae,and Escherichia coli being the most common in descending order.There were 28 Gram-positive bacteria(32.56%),with Staphylococcus aureus(14 strains,16.28%),Streptococcus pneumoniae(8 strains,9.30%),and Staphylococcus epidermidis(6 strains,6.98%)being the most common in descending order.There were 4 fungi(4.65%).The proportion of smoking history,diabetes history,SOFA score at admission,APACHE II score,endotracheal intubation,nasogastric tube placement,duration of ventilator support,proportion of parenteral nutrition,and levels of C-reactive protein(79.61±8.35)mg/L,tumor necrosis factor-α(173.57±17.80)ng/L,and procalcitonin(6.59±1.36)μg/L were significantly higher in the ventilator-associated pneumonia group than in the control group,with statistically significant differences(
P<
0.01).Multivariate logistic regression analysis showed that endotracheal intubation,nasogastric tube placement,duration of ventilator support,C-reactive protein,tumor necrosis factor-α,and procalcitonin were independent risk factors for ventilator-associated pneumonia.
Conclusion
The most common pathogens causing ventilator-associated pneumonia in patients with acute exacerbation of COPD were Pseudomonas aeruginosa,followed by Acinetobacter baumannii and Staphylococcus aureus.Risk factors included endotracheal intubation,nasogastric tube placement,duration of ventilator support,C-reactive protein,tumor necrosis factor-α,and procalcitonin.

Effect of Sevoflurane Combined with Propofol Remifentanil Intravenous Combined Anesthesia Mode in Patients undergoing Laparoscopic Cholecystectomy

Objective
To analyze effects of combined intravenous and inhalation anesthesia on patients undergoing laparoscopic cholecystectomy.
Methods
150 consecutive patients who underwent laparoscopic cholecystectomy were randomly assigned to either the observation group(n=75)or the control group(n=75)using a block randomization method.The control group was given propofol combined with remifentanil intravenous anesthesia,and the observation group was given sevoflurane combined with propofol and remifentanil intravenous inhalation combined anesthesia.The postoperative recovery,basic vital signs and stress were observed in the two groups.
Results
In the observation group,all recovery times were significantly shorter than those in the control group(

0.05).No significant differences in SpO
2
levels were observed between the two groups at different time points(

0.05).Prior to anesthesia,there were no significant differences in systolic blood pressure(SBP),diastolic blood pressure(DBP),and heart rate(HR)between the two groups(

0.05).Post-anesthesia,intraoperatively,and postoperatively,the observation group exhibited significantly lower SBP,DBP,and HR levels compared to the control group(

Conclusion
sevoflurane combined with propofol-remifentanil intravenous inhalation combined anesthesia is effective in laparoscopic cholecystectomy,which can effectively shorten the postoperative recovery time,maintain intraoperative hemodynamic stability,and inhibit stress response.

Immunohistochemical Markers Combined with Tumor Necrosis Factor α The Clinical Significance of miRNA-107 in the Diagnosis of Gastric Cancer

Objective
To analyze the combination of immunohistochemistry indicators and tumor necrosis factor α、 The clinical value of miRNA-107 in the diagnosis of gastric cancer.
Method
48 gastric cancer patients(gastric cancer group)admitted to the hospital were included in the study.In addition,48 benign gastric cancer patients(benign gastric cancer group)and 48 healthy individuals(healthy group)who visited the hospital during the same period were selected.Immunohistochemical indicators and tumor necrosis factor tests were performed on all tested subjects- α And miRNA-107 testing.Subsequently,based on the degree of disease progression of gastric cancer patients,the gastric cancer group was divided into an early group(n=22)and an advanced group(n=26).The immunohistochemical index test and tumor necrosis factor were compared between the two groups of patients- α Tumor crossing factor- α,TNF- α)And miRNA-107 test,receiver operating characteristic(ROC)analysis of immunohistochemical indicators,tumor necrosis factor- α And the diagnostic value of miRNA-107 test in gastric cancer disease.
Result
In the gastric cancer group,the human epidermal growth factor was regulated by -2(Her-2)and TNF- α、 MiRNA-107 was higher than the other two groups(

0.05).Compared with the early group,Her-2 and TNF in patients in the advanced group- α And miRNA-107 levels were higher,with significant differences(

The Effect of Mini Locking Plate Internal Fixation in the Treatment of Patients with Ankle Fractures Combined with Inferior Tibiofibular Syndesmosis Ligament Injury

Objective
To analyze the clinical effect of micro-locking plate internal fixation in the treatment of ankle fracture combined with lower tibiofibular joint ligament injury,and to provide theoretical reference basis for clinical practice.
Methods
94 cases of ankle fracture combined with lower tibiofibular joint ligament injury were selected as the research subjects,and according to the condition and the patients' wishes,they were divided into the observation group and the control group,with 47 patients in the observation group and 47 patients in the control group.The control group was treated with percutaneous screw internal fixation,and the observation group was treated with mini locking plate internal fixation,comparing the clinical effects,perioperative indexes,pain level,foot and ankle function and daily activity ability of the two groups.
Results
The effective rate of 95.7% in the observation group was higher than 83.0% in the control group,and the difference was statistically significant(

0.05);the hospitalization time and fracture healing time of the observation group were lower than those of the control group,and the difference was statistically significant(

0.05);3 days after the operation,the difference between the VAS,BI and AOFAS-AH scores of the two groups was not statistically significant(

0.05),and 3 months after the operation,the two groups VAS scores decreased and BI and AOFAS-AH scores increased,but VAS scores in the observation group were lower than those in the control group,and BI and AOFAS-AH scores were higher than those in the control group,and the difference was statistically significant(

The Effect of Abnormal Electrocardiogram on Pregnancy Outcome in Late Pregnancy of Elderly Parturient woman

Objective
To study the effect of abnormal ECG on pregnancy outcome of elderly pregnant women in late pregnancy.
Methods
86 cases of elderly pregnant women in late pregnancy who underwent antenatal examination and delivery were selected as the experimental group,and the appropriate age pregnant women in late pregnancy who underwent antenatal examination and delivery in the same period were selected as the normal group according to the ratio of 1∶1.All pregnant women received ECG examination,and the abnormal ECG and arrhythmia types [sinus tachycardia(ST),sinus bradycardia(SB),sinus arrhythmia(SA),ventricular block(IVCB),ventricular premature beats,atrial premature beats]of the two groups were compared,and the influence of abnormal ECG on pregnancy outcome of pregnant women was analyzed.
Results
The incidence of normal ECG in the experimental group was,lower than in the normal group,and the incidence of abnormal ECG in the experimental group was higher than in the normal group(

0.05).The incidence of ST and total arrhythmia in the experimental group were respectively,higher than those in the normal group(
P<
0.05).The incidence of premature birth,cesarean section and low-quality infants in the abnormal electrocardiogram of the experimental group were higher than those in the normal ECG,and the incidence of cesarean section in the abnormal electrocardiogram of the experimental group was higher than that in the normal group(

0.05).
Conclusion
The incidence of abnormal ECG and arrhythmia in elderly pregnant women in late pregnancy is higher,and the elderly pregnant women with abnormal ECG in late pregnancy will have a variety of adverse pregnancy outcomes.

The Influence of Continuous Positive Airway Pressure Ventilation Treatment on the Prognosis of Pregnant Women with Obstructive Sleep Apnea and Their Infants

Objective
To explore the impact of continuous positive airway pressure(CPAP)treatment on the prognosis of pregnant women with moderate to severe obstructive sleep apnea(OSA)and their infants.Currently,there are multiple clinical trial results available in the clinical setting.
Methods
A total of 89 pregnant women with OSA(moderate to severe)were included.Among them,45 had moderate OSA and were divided into a control group of 25 patients and a CPAP intervention group of 20 patients.The remaining 44 patients with severe OSA were divided into a control group of 21 patients and a CPAP intervention group of 23 patients.The pregnancy complications(severe complications of gestational hypertension,diabetes,heart disease,fetal growth restriction,and fetal distress)and maternal and infant outcomes(preterm birth,cesarean section,neonatal asphyxia,and perinatal death)were compared between the CPAP intervention group and the control group.
Results
In pregnant women with moderate OSA,the CPAP treatment group had a significantly lower incidence of severe complications of gestational hypertension compared to the control group(
P
<0.05).In pregnant women with severe OSA,the CPAP treatment group had significantly lower incidences of severe complications of gestational hypertension,diabetes,fetal growth restriction,fetal distress,and preterm birth compared to the control group(
P<
0.05).
Conclusion
CPAP treatment for pregnant women with moderate to severe OSA can significantly reduce the incidence of severe complications of gestational hypertension,as well as the incidences of diabetes,fetal growth restriction,fetal distress,and preterm birth in those with severe OSA,thereby significantly improving the prognosis of both the mother and the infant.

Diagnostic Significance of Ambulatory Electrocardiography for Pediatric Kawasaki Disease Combined with Coronary Artery Injury

Objective
To investigate the diagnostic significance of dynamic electrocardiography(DCG)for pediatric Kawasaki disease(KD)combined with coronary artery injury(CAL).
Methods
We retrospectively analyzed 70 children with KD,all of whom underwent conventional electrocardiography(ECG)and DCG,and analyzed the diagnostic results,diagnostic efficacy,and predictive value of ECG and DCG using coronary arteriography as the“gold standard” and compared the detection rate of abnormalities between the two modalities.The diagnostic results,diagnostic efficacy and predictive value of ECG and DCG were analyzed,and the detection rate of abnormalities of the two examinations was compared.
Results
Among the 70 children with KD in our group,48 cases of combined CAL and 22 cases of uncomplicated CAL were demonstrated by coronary angiography;33 cases of combined CAL and 20 cases of uncomplicated CAL were demonstrated by ECG;47 cases of combined CAL and 21 cases of uncomplicated CAL were demonstrated by DCG.The sensitivity of DCG examination was 97.92%(47/48),the accuracy of 97.14%(68/70)was higher than ECG examination 68.75%(33/48)and 75.71%(53/70),and the leakage rate of 2.08%(1/48)was lower than ECG examination 31.25%(15/48)(

DCG has high sensitivity and accuracy in the diagnosis of pediatric KD combined with CAL,and it can reduce the misdiagnosis rate and improve the negative predictive value,and it can accurately detect the abnormal conditions of the children,which can provide a reference basis for the clinical development of the treatment plan.

Analysis of the Clinical Feasibility of Electron Bronchoscopic Alveolar Lavage in the Treatment of Children with Lobar Pneumonia in the Solid Phase

To investigate the feasibility of electron bronchoscopic alveolar lavage for the treatment of children with lobar pneumonia in the solid stage.
Methods
Selected 68 cases of lobar pneumonia in the solid stage of children as the research object,according to the numerical table method randomly divided into two groups,in which the conventional group to receive conventional drug therapy,electronic bronchoscopy group to receive electronic bronchoscopy alveolar lavage treatment,compare the efficacy of the two groups,fever time,cough time,temperature recovery time,the disappearance of the lung rales time.Comparison of inflammatory indexes and immune function indexes between the two groups.
Results
The efficacy of the electronic bronchoscopy group was higher than that of the conventional group(

Effect of Different Volume of 0.25% Ropivacaine on Analgesia after Cesarean Section with Bilateral TAP Block under Ultrasound Guidance

Objective
To analyze the analgesic effect of 0.25% ropivacaine in different volumes on bilateral TAP block under ultrasound guidance after cesarean section.
Methods
A total of 146 ACOS patients were selected as the research objects.The patients were randomly allocated into two groups:the observation group and the control group.While the control group received solely inhaled budesonide aerosol therapy,the observation group was administered glycine-theophylline sodium sustained-release tablets in addition to the same treatment regimen as the control group.The lung function,inflammatory factors and clinical effects were compared between the two groups.
Results
There were significant differences in pain relief time and spontaneous urination time among the three groups(

0.05),but there was no significant difference between group M and group H(

0.05).There were significant differences in the time of first pressing the analgesic pump and the number of pressing the analgesic pump within 24 hours among the three groups(

The Effect of Gestational Age on the Enteral Feeding Status of Ultra-Low Birth Weight Infants

Objective
To investigate the effect of gestational age on enteral feeding status of ultra-low birth weight infants.
Methods
60 cases of ultra-low birth weight infants were selected as the study subjects.They were divided into group A and group B according to gestational age(≥28 weeks,<28 weeks)by using internal control.The amount of milk started and the amount of milk at different times were recorded,and the incidence of feeding intolerance,hospitalization time and discharge weight were compared between the two groups.
Results
There was no statistical difference in the incidence of feeding intolerance between the two groups in terms of the amount of milk started,the amount of milk at 5d postpartum,and the amount of milk at the time of discharge from the hospital within 1 week postpartum(

The Differential Diagnostic Value of CT Examination and Magnetic Resonance Imaging for Ligament and Tendon Injuries around Knee Joint Fractures

Objective
Exploring the varying diagnostic benefits of CT and magnetic resonance imaging in ligament and tendon injuries around knee joint fractures is the objective.
Methods
Select 80 patients with knee joint fractures accompanied by peripheral ligament and tendon injuries as the research subjects.The patients were divided into three categories based on the type of examination:CT,MRI,and joint inspection.Subsequently,the exactness,sensitivity,and precision of each single examination was compared and evaluated.
Results
(1)48 suspected cases were diagnosed by the "gold standard" examination,and the diagnosis rate was 60.0%.There were 35 patients with positive CT examination,40 patients with positive MRI examination and 46 patients with positive combined examination.(2)In this study,the accuracy of CT is 65%,the accuracy of MRI is 77.5%,and the accuracy of combined examination is the highest,reaching 95%.The sensitivity of CT is 72.92%,that of MRI is 83.33%,and the sensitivity of combined examination is the highest,which is 95.83%.The specificity of CT is 53.13%,the specificity of MRI is 68.75%,and the specificity of combined examination is the highest,which is 93.75%.Compared with CT and MRI,the combined examination showed significant statistical differences(

0.001),in which the accuracy,sensitivity and specificity were 95.00%,95.83% and 93.75% respectively.Combined examination is superior to CT or MRI alone in diagnostic accuracy.
Conclusion
The combined examination of CT and MRI has high diagnostic efficiency in the diagnosis of ligament and tendon injuries around knee joint fractures,with accuracy,sensitivity and specificity of 95.00%,95.83% and 93.75% respectively,which is worthy of clinical promotion.

Effect of Adjuvant Steel Plate Combined with Intraoperative and Postoperative Autologous Platelet-Rich Plasma(PRP)on the Healing of Nonunion after Intramedullary Fixation of Femoral Fractures

Objective
To investigate the efficacy of adjuvant steel plate combined with autologous platelet-rich plasma(PRP)in patients with nonunion after intramedullary fixation of femoral fractures.
Methods
Thirty-eight patients with nonunion after intramedullary fixation of femoral fractures were randomly divided into a control group(n=19)and an observation group(n=19)using a random number table.The control group retained the intramedullary fixation with additional reconstructive steel plate and received intraoperative autologous iliac bone grafting.The observation group,on the basis of the control group,was treated with intraoperative and postoperative autologous PRP implantation at the bone defect site.Perioperative indicators,fracture healing,callus formation,and excellent limb recovery were compared between the two groups.
Results
The perioperative indicators in the observation group were all lower than those in the control group(

Effect of Ginkgo Diterpene Lactone Meglumine Injection Combined with Acupuncture on Limb Function Recovery and Serum Hcy and D-D in Patients with Hemiplegia after Stroke

Objective
To investigate the effect of ginkgo diterpene lactone meglumine injection combined with acupuncture on limb function recovery and serum homocysteine(Hcy)and D-dimer (D-D)levels in patients with hemiplegia after stroke.
Methods
A total of 84 patients with hemiplegia after stroke were selected as subjects and randomLy divided into a drug group and a drug-acupuncture group with 42 cases in each group according to the enveloperandom method.Patients in the drug group were treated with conventional treatment and ginkgo diterpene lactone meglumine injection,and patients in the drug-acupuncture group were treated with acupuncture on this basis.The traditional Chinese medicine(TCM)syndrome scores,limb motor function[Fugl-Meyer Assessment Scale(FMA)],daily self-care ability[Modified Barthel Index(MBI)],and nerve defect[National Institutes of Health Stroke Scale(NIHSS)]of the two groups of patients after treatment were compared.The changes of hemorheological indexes[whole blood viscosity,fibrinogen(Fib),hematocrit(HCT),erythrocyte sedimentation rate(ESR)],high-sensitivity C-reactive protein(Hs-CRP),Hcy,and D-D levels of the two groups of patients before and after treatment were observed.
Results
After treatment,it was observed that the TCM syndrome scores of both groups decreased,but the decreasing trend in the drug-acupuncture group was more significant than that in the drug group(

0.05);the upper-limb FMA score,lower-limb FMA score,and MBI score all showed an increasing trend,but the increasing trend in the drug-acupuncture group was greater than that in the drug group(

0.05);the NIHSS score,high-shear whole-blood viscosity,low-shear whole-blood viscosity,Fib,HCT,ESR levels,Hs-CRP,Hcy,and D-D levels of the two groups all showed a decreasing trend,but the decreasing trend in the drug-acupuncture group was greater than that in the drug group(

Conclusion
Ginkgo diterpene lactone meglumine injection combined with acupuncture treatment can reduce nerve function damage in patients with hemiplegia after stroke,restore limb function more quickly,improve hemorheology and reduce inflammation,which is beneficial to improving the prognosis.

Prediabetes is a critical stage in the development of diabetes,characterized by insulin resistance and metabolic disorders.Recent studies have found that the imbalance of gut microbiota is closely related to this condition.Berberine,as a potential bioactive component,has shown effects in regulating gut microbiota and improving metabolic disorders,thus attracting widespread attention.At the same time,tryptophan metabolism also plays an important role in the occurrence and development of diabetes.This article aims to comprehensively explore the application of berberine in patients with prediabetes,focusing on its impact on gut microbiota and tryptophan metabolism,in order to provide theoretical basis and new treatment ideas for clinical practice.Through a comprehensive analysis of existing literature,we will elucidate the value of berberine in regulating gut microbiota balance,improving metabolic abnormalities,and its potential clinical application value,providing new perspectives for the prevention and treatment of prediabetes.
