妊娠合并阻塞性睡眠呼吸暂停患者应用持续气道正压通气治疗对母儿预后的影响

摘要目的探讨妊娠合并中重度阻塞性睡眠呼吸暂停（OSA）患者应用持续气道正压通气治疗（CPAP）对母儿预后的影响。方法纳入89例妊娠合并OSA（中重度）患者,中度OSA患者45例,按患者是否接受CPAP治疗分为对照组25例,CPAP干预治疗组20例;重度OSA患者44例分为对照组21例,CPAP干预治疗组23例。分别比较CPAP干预治疗组与对照组患者的妊娠合并症（妊娠期高血压、妊娠期高血压严重并发症、糖尿病、心脏病、胎儿生长受限、胎儿窘迫）及母婴结局（早产、剖宫产、新生儿窒息、围产儿死亡）。结果妊娠合并中度OSA患者 CPAP治疗组妊娠期高血压严重并发症与对照组比较显著降低（P<0.05）;妊娠合并重度OSA患者 CPAP治疗组患者妊娠期高血压严重并发症、糖尿病、胎儿生长受限、胎儿窘迫、早产与对照组比较显著降低（P<0.05）。结论妊娠合并中重度OSA患者进行CPAP治疗能显著降低妊娠期高血压严重并发症的发生,降低妊娠期重度OSA患者糖尿病、胎儿生长受限、胎儿窘迫及早产的发生率,明显改善母婴预后。

Abstract：Objective To explore the impact of continuous positive airway pressure（CPAP）treatment on the prognosis of pregnant women with moderate to severe obstructive sleep apnea（OSA）and their infants.Currently,there are multiple clinical trial results available in the clinical setting. Methods A total of 89 pregnant women with OSA（moderate to severe）were included.Among them,45 had moderate OSA and were divided into a control group of 25 patients and a CPAP intervention group of 20 patients.The remaining 44 patients with severe OSA were divided into a control group of 21 patients and a CPAP intervention group of 23 patients.The pregnancy complications（severe complications of gestational hypertension,diabetes,heart disease,fetal growth restriction,and fetal distress）and maternal and infant outcomes（preterm birth,cesarean section,neonatal asphyxia,and perinatal death）were compared between the CPAP intervention group and the control group. Results In pregnant women with moderate OSA,the CPAP treatment group had a significantly lower incidence of severe complications of gestational hypertension compared to the control group（P<0.05）.In pregnant women with severe OSA,the CPAP treatment group had significantly lower incidences of severe complications of gestational hypertension,diabetes,fetal growth restriction,fetal distress,and preterm birth compared to the control group（P<0.05）. Conclusion CPAP treatment for pregnant women with moderate to severe OSA can significantly reduce the incidence of severe complications of gestational hypertension,as well as the incidences of diabetes,fetal growth restriction,fetal distress,and preterm birth in those with severe OSA,thereby significantly improving the prognosis of both the mother and the infant.
