Abstract:Objective To investigate the expression differences and correlations of Ki-67 and p53 in colorectal high-grade intraepithelial neoplasia(HGIN)and adenocarcinoma tissues, thereby providing a reference for clinical risk assessment of malignant transformation. Methods A total of 72 patients with colorectal lesions admitted to our hospital from January 2018 to December 2025 were retrospectively enrolled, including 27 cases of high-grade intraepithelial neoplasia and 45 cases of adenocarcinoma. The protein expressions of Ki-67 and p53 in lesion tissues were detected using the immunohistochemical SP method.Fisher’s exact test was used to correct statistical Results in small-sample subgroups, and Spearman’s rank correlation analysis was applied to assess correlations between indicators. Results The positive expression rate of Ki-67 in the adenocarcinoma group was significantly higher than that in the HGIN group(P<0.05).In the HGIN subgroup with malignant tendency, the positive rates of both Ki-67 and p53 were higher than those in the subgroup without malignant tendency(P<0.05).The nature of the lesion was significantly positively correlated with Ki-67 expression, and Ki-67 expression was also significantly positively correlated with p53 expression(P<0.001). Conclusion Combined detection of Ki-67 and p53 can accurately assess the risk of malignant transformation in colorectal high-grade intraepithelial neoplasia, assist in clinical pathological differential diagnosis and individualized treatment, and has high clinical application value.
