Clinical Prognostic Study on Intermediary Thrombolytic Therapy for Acute Ischemic Stroke Based on CTP

Objective
To investigate the prognostic evaluation value of CTP technology in AIS patients undergoing interventional thrombectomy.
Method
Clinical data of 83 AIS patients undergoing MT surgery were collected for retrospective analysis.They were divided into a good prognosis group of 52 cases and a poor prognosis group of 31 cases according to the mRS score.According to the mCTA score, they were divided into a good collateral circulation group of 54 cases and a poor collateral circulation group of 29 cases.According to the NIHSS score, they were divided into a significantly improved nerve defect group of 48 cases and a no significant improvement nerve defect group of 35 cases.The CTP perfusion parameters of patients in different groups were compared one week after surgery.
Result
The CTP perfusion parameters CBF, CBV, and HIR values of patients with good prognosis were significantly higher than those of patients with poor prognosis(
P<
0.05); The CTP perfusion parameters CBF, CBV, and HIR values of patients with good collateral circulation were significantly higher than those of patients with poor collateral circulation(
P<
0.05); The CTP perfusion parameters CBF, CBV, and HIR values of patients with significantly improved neurological deficits were significantly higher than those of patients with no significant improvement in neurological deficits(
P<
0.05).
Conclusion
CTP technology can effectively evaluate the postoperative collateral circulation status, degree of neurological dysfunction, and overall prognosis of AIS patients undergoing MT surgery by measuring parameters such as infarct core volume and ischemic penumbra.It provides a scientific reference for clinical prediction of HT risk and the formulation of next step treatment plans.

The Correlation between Serum Levels of PCT, Acidic Glyceprotein and Heptoglobin and the Severity of Acute COPD

Objective
To study PCT α 1 The correlation between the levels of acid glycoprotein and haptoglobin and the severity of acute COPD.
Methods
70 patients with acute COPD were selected as the study subjects.According to the severity of the disease, they were divided into stable COPD group of 34 patients and acute exacerbation(AECOPD)group of 36 patients, and 40 volunteers who came to our hospital for health examination at the same time were selected as the control group.Detection of serum by enzyme-linked immunosorbent assay α 1 Acid glycoprotein(α 1-AG)level, the level of procalcitonin(PCT)was detected by immunofluorescence double antibody sandwich method, the level of serum haptoglobin(HPT)was measured by immune scattering rate turbidimetry, and the level of forced vital capacity(FVC)and forced expiratory volume in one second(FEV1)were measured by pulmonary function meter; Pearson was used to analyze pulmonary function indicators and serum PCT α 1-Correlation of AG and HPT.
Results
Serum PCT α 1-AG and HPT levels were higher than those in the control group, and serum PCT α 1-AG, HPT levels and pulmonary function indicators FEV1, FEV1/FVC increased with the severity of the disease; Serum PCT and HPT were negatively correlated with FEV1 and FEV1/FVC in patients with COPD The level of α 1-AG was positively correlated with FEV1% and FEV1/FVC, and the difference was statistically significant(

Conclusion
Serum PCT The levels of α 1-AG and HPT are closely related to the severity of COPD patients, which is of great significance in evaluating the patients' condition.

The Effect of Salvia Miltiorrhiza Polyphenols on Cardiac Function and Micro Inflammatory Markers in Patients with Acute Myocardial Infarction

Objective
To explore the effect of salvia miltiorrhiza polyphenols on cardiac function and micro inflammatory markers in patients with acute myocardial infarction(AMI).
Method
89 AMI patients admitted to the hospital were selected and randomLy divided into a control group(45 cases)and a study group(44 cases)using a random number table method; The control group received routine treatment, while the research group received treatment with salvia miltiorrhiza polyphenol hydrochloride based on routine treatment; Compare the levels of efficacy, cardiac function indicators, and micro inflammatory indicators between two groups.
Result
With the control group, the total effective rate of the study group was higher, and the difference was statistically significant(

Application of Enhanced MSCT in Hepatocellular Carcinoma Ultrasound-guided Radiofrequency Ablation Treatment of Small Liver Cancer

Objective
To explore the application of enhanced multi-slice spiral CT(MSCT)in the evaluation of efficacy of ultrasound-guided radiofrequency ablation for small liver cancer.
Methods
136 patients with small liver cancer who underwent ultrasound-guided radiofrequency ablation were selected in our hospital.All patients underwent enhanced MSCT examination and digital subtraction angiography(DSA)examination at 1 month after surgery.DSA examination Results were taken as the gold standards to assess the efficiency of enhanced MSCT in evaluating efficacy.
Results
Among 136 patients with 176 lesions at 1 month after radiofrequency ablation, DSA examination showed 119 lesions of complete ablation and 57 residual lesions, and enhanced MSCT examination showed 120 lesions of complete ablation and 56 residual lesions.The sensitivity, specificity, accuracy rate, positive predictive value, negative predictive value and Kappa value of enhanced MSCT in evaluating the ablation efficacy were 70.17%, 86.55%, 81.25%, 71.43%, 85.83% and 0.570, which had good consistency with the DSA detection results.
Conclusion
Enhanced MSCT scan can clearly observe the changes of lesions of patients after ultrasound-guided radiofrequency ablation for small liver cancer, and can effectively evaluate the efficacy and guide subsequent treatment.

The Clinical Effect of Tamsulosin in the Treatment of Ureteral Calculi after Extracorporeal Shock Wave Lithotripsy

Objective
To investigate the clinical effect of tamsulosin in assisting stone removal after extracorporeal shock wave lithotripsy for ureteral calculi.
Methods
90 patients with ureteral calculi were randomly divided into two groups.The control group was treated with extracorporeal shock wave lithotripsy(ESWL).On this basis, the observation group was assisted with tamsulosin treatment after surgery, and the stone clearance rate, lithotripsy and stone removal indicators, complications and adverse reactions of the two groups were compared.
Results
The stone free rate of the observation group at 1 week and 4 weeks after operation was higher than that of the control group(

0.05); The times of lithotripsy, the time of stone removal, the use rate of analgesics and the length of hospital stay in the observation group were less than those in the control group(

Clinical Efficacy of Early and Delayed Laparoscopic Cholecystectomy in the Treatment of Acute Cholecystitis with Mild Pancreatitis

Objective
To compare the clinical efficacy, safety and feasibility of early and delayed laparoscopy(LC)in the treatment of acute cholecystitis with mild pancreatitis.
Methods
Retrospective analysis was performed on 200 cases of patients diagnosed with acute pancreatitis and with complete course recordsand they were divided into observation group and control group according to random drawing, among which 100 patients underwent early laparoscopic cholecystectomy(observation group).Among them, 100 patients(control group)underwent laparoscopic cholecystectomy after conservative treatment.The two groups were compared to convert to laparotomy(cases), operation interval(days), total hospital stay(days), operation time(minutes), postoperative complications(cases), and preoperative biliary tract related events(cases).
Results
There was no significant difference between the observation group and the control group(PBB0 0.05).There were 4 cases of laparotomy, operation time(52.24± 6.39)min and 4 cases of postoperative complications.In the observation group, the operative interval(5.78± 0.96)days, the total hospital stay(8.51± 2.78)days and the occurrence of biliary tract related events occurred in 1 patient were significantly lower than those in the control group(41.25± 4.64)days, the total hospital stay(16.62± 2.02)days and the occurrence of biliary tract related events in 11 patients.The difference was statistically significant(

Conclusion
For acute cholecystitis with mild pancreatitis, the clinical effect of early LC is better than delayed LC in the same hospitalization.

Effects of Vaginal Myomectomyand Laparoscopic Myomectomy on Estrogen Receptor, Sex Hormone and Quality of Sexual Life in Patients with Uterine Fibroid

Objective
To observe the effects of vaginal myomectomy(VM)and laparoscopic myomectomy(LM)on estrogen receptor, sex hormone and sex life quality in patients with uterine fibroids(UF).
Methods
A total of 100 UF patients were selected as research objects and divided into LM group and VM group according to block randomization, with 50 cases in each group.The LM group was treated with LM and the VM group was treated with VM.The basic information of the two groups during perioperative period(operation time, blood loss, first anal exhaust time)was compared, and the changes of ER, sex hormones(FSH, LH, E2)before and 1 week after surgery were compared between the two groups.The incidence of postoperative complications in the two groups was counted 1 week after surgery, and the quality of sexual life(sexual life quality scale)was observed in the two groups before surgery and 1 month after surgery.
Results
The operation time, blood loss and first anal exhaust time in VM group were lower than those in LM group(

0.05).One week after surgery, the levels of FSH and LH in both groups were increased compared with those before surgery(

0.05), and the positive rate of ER and the level of E2 were decreased compared with those before surgery(

Comparison of the Effect of Injection of Pramivir and Oral Oseltamivir in the Treatment of Children with Influenza

Effect of Arthroscopic Debridement Combined with Intra Articular Injection of Sodium Hyaluronate on Knee Function in Patients with Knee Osteoarthritis

Objective
To investigate the effect of arthroscopic cleanup combined with intra-articular injection of sodium vitrate on knee joint function in patients with knee osteoarthritis(KOA).
Methods
88 patients with KOA were selected and divided into a control group(44 cases)and a treatment group(44 cases)according to the random number table method.The control group was treated with a single arthroscopic debridement, and on this basis, the treatment group was treated with intra-articular injection of sodium vitrate.The efficacy, inflammatory response, knee function, complications, and pain level of the two groups were compared.
Results
Compared with the control group, the total effective rate of treatment was higher in the treatment group(

0.05); postoperative lysholms scores were higher and visual analog scoring method(VAS)scores were lower in the two groups than before surgery, while lysholms scores were higher and VAS scores were lower in the treatment group than in the control group(

The Value of Skin Prick Test of Allergens in Determining Allergens in Children with Asthma and Guiding Treatment after Diagnosis

Methods
In this study, 140 children with asthma were taken as the research object.All children received relevant treatment.Allergen skin prick test was carried out to determine the allergen.
Results
The test of children in this group were analyzed to determine the allergen of children in this group.According to the allergen of children, effective treatment was carried out to evaluate the guiding value of allergen skin prick test in the treatment of children. Results:The allergen skin prick test showed that the first place was house dust mite, the second was dust mite, Artemisia grandis pollen, Liliaceae pollen, humulus, willow pollen, indoor dust, cat epithelium, dog epithelium, wutong pollen, juniper, ragweed, etc., and the lowest proportion was locust tree and alternaria spp; After treatment, 63 cases were cured, 43 cases were markedly effective, 31 cases were effective, and 3 cases were ineffective.The cure rate and treatment effective rate were 45.00%(63/140) and 97.86%(137/140) respectively.
Conclusion
When determining the allergen in children with asthma, the allergen skin prick test has a high accuracy, which can provide reliable guidance for clinical treatment, and it is recommended to promote.

Effect of Gamma Globulin Treatment on Serum IGF-1 and cTnI Levels in Children with Severe Pneumonia Complicated with Heart Failure

Objective
To observe the effect of gamma globulin on serum levels of insulin-like growth factor-1(IGF-1)and troponin Ⅰ (cTnⅠ )in children with severe pneumonia complicated with heart failure.
Methods
A total of 65 children with severe pneumonia complicated with heart failure were recruited and randomly divided into conventional treatment control group(n=34)and conventional treatment plus gamma globulin treatment study group(n=31).The curative effect, inflammatory levels [C-reactive protein(CRP), tumor necrosis factor α (TNF-α ), interleukin 6(IL-6)], cardiac function indexes [troponin I(CTN I), B-type brain natriuretic peptide(BNP), insulin-like growth factor -1(IGF-1)] and immunoglobulin levels before and after treatment were compared between the two groups.
Results
After treatment, the treatment response rate, IGF-1, IgG, IgM and IgA levels in the study group were significantly higher than those in the control group(
P<
0.05), the levels of cTnⅠ , BNP, CRP, TNF-α and IL-6 were significantly lower than those in the control group(
P<
0.05), and the remission time of positive symptoms and hospital stay were significantly shorter than those in the control group(
P<
0.05).
Conclusion
Gamma globulin is effective in the treatment of severe pneumonia complicated with heart failure, which can reduce the inflammatory state of children, improve their heart function, enhance their immunity and promote the outcome.

Observation on the Curative Effect of Infantile Cough Syrup Combined with Ambroxol Hydrochloride in the Treatment of Neonatal Pneumonia

Objective
To study the effect of pediatric cough syrup combined with ambroxol hydrochloride on neonatal pneumonia.
Methods
60 cases of neonatal pneumonia were randomLy divided into two groups with 30 cases in each group.Both groups received routine treatment.The control group was given intravenous infusion of ambroxol hydrochloride, and the observation group was given pediatric cough syrup combined with ambroxol hydrochloride.The levels of serum C-reactive protein(CRP)and white blood cell(WBC)were measured before and after treatment, the effective rate of the two treatment was compared, and the adverse reactions were recorded during the whole treatment process.
Results
No adverse drug reaction occurred in both groups during treatment.After treatment, the time of cough symptom disappearing, pulmonary rales disappearing and hospital stay in observation group were shorter than those in control group(

0.05).After treatment, the cough score of observation group was lower than that of control group(

0.05).The phlegm viscosity grade in observation group was better than that in control group(

Conclusion

Comparative Analysis of Consistency between Abnormal Results of Thin-layer Liquid-based Cytology and Histopathological Diagnosis in 89 cases

Objective
To explore the consistency between the abnormal results of thin-layer liquid-based cytology(TCT)and histopathological diagnosis in 89 cases.
Methods
The clinical data of 1113 female patients examined by TCT and histopathology were retrospectively analyzed, and the consistency between abnormal results of TCT and histopathological diagnosis was analyzed.
Results
Among the 89 cases of abnormal TCT, 40 cases were atypical squamous cells(ASC-US), 8 cases were atypical squamous cells(ASC-H)with high epithelial lesions, 27 cases were low-grade squamous intraepithelial lesions(LSIL), 13 cases were high-grade squamous intraepithelial lesions(HSIL)and 1 case was squamous cell carcinoma(SCC).The coincidence rate between TCT diagnosis of SCC and pathological diagnosis was 100.00%.The diagnostic coincidence rate of TCT for HSIL was 76.92%, which was higher than that of LSIL(40.74%,

< 0.05).The abnormal results of TCT showed that 41 cases(46.07%)were positive for LSIL and above, and 47 cases(52.81%)were positive for pathological diagnosis.There was no difference between them(

0.05).
Conclusion
The abnormal results of TCT in 89 cases are consistent with the histopathological diagnosis, and TCT can be used as an effective supplementary method for the diagnosis of cervical lesions.

Relationship Between CT Measurement of Optic Nerve Sheath Diameter and Postoperative Intracranial Pressure Increase in Patients with Craniocerebral Trauma

Objective
To explore the relationship between CT measurement of optic nerve sheath diameter(ONSD)and postoperative intracranial pressure(ICP)in patients with craniocerebral trauma.
Methods
60 patients with craniocerebral trauma who underwent decompressive craniectomy were selected as the research objects.Invasive ICP monitoring was implanted during the operation, and they were divided into 34 normal groups(ICP≤ 20mmHg)and 26 increased groups(IC

20mmHg)according to the postoperative ICP levels.All patients underwent thin-slice CT examination of the head to measure their ONSD.To analyze the relationship between ONSD and postoperative ICP in patients with craniocerebral trauma.
Results
ONSD of ICP group was higher than that of normal group(

0.05).Pearson correlation analysis showed that ONSD was positively correlated with postoperative ICP in patients with craniocerebral trauma(

0.05).Glasgow coma scale(GCS)in ICP group was lower than that in normal group(

0.05).Multiple regression analysis showed that ONSD and GCS scores were the influencing factors of postoperative ICP in patients with craniocerebral trauma(

Conclusion
CT measurement of optic nerve sheath diameter is closely related to postoperative intracranial pressure increase in patients with craniocerebral trauma, and can be used as an important index for clinical evaluation of postoperative intracranial pressure increase in patients.

The Value of High-Resolution CT in Differentiating Benign and Malignant Solid Solitary Pulmonary Nodules

Objective
To investigate the value of high-resolution CT in differentiating benign and malignant solid solitary pulmonary nodules(SPN).
Methods
A total of 60 patients with solid SPN admitted were retrospectively selected as research objects and divided into benign group(n=22)and malignant group(n=38)according to the Resultsof pathology/puncture examination.Both groups underwent high-resolution CT to analyze the differential value of high-resolution CT in the benign and malignant diagnosis of solid SPN.
Results
The coincidence rates of high-resolution CT in the diagnosis of benign solid SPN and malignant solid SPN were 86.36% and 73.68%, respectively.There was no significant difference in the coincidence rates between the two groups(

0.05).There were statistically significant differences between benign group and malignant group in boundary characteristics, cavity, tractive emphysema and vessel convergence sign(
P<
0.05).Binary logistic regression analysis showed that lobulation sign, non tractive emphysema and vessel convergence sign were all independent factors influencing the benign and malignant diagnosis of solid SPN by high-resolution CT(
P<
0.05).
Conclusions
High-resolution CT is of great value in the differential diagnosis of benign and malignant solid SPN.It can be used to assist in the differential diagnosis of solid SPN according to the boundary characteristics, tractive emphysema, and vessel convergence sign.

Exploration and Thinking of Network Distance Teaching and Training of Clinical Operation Skills for Primary Doctors

Objective
To explore the effectiveness of remote teaching and training mode through online and video conference-based training in improving the clinical operational skills of primary care physicians in the context of mobile internet and network mode.
Methods
A total of 128 primary care physicians from healthcare institutions in Ganzi Prefecture, Sichuan Province, were randomly divided into an experimental group(n=64) and a control group(n=64) in a 1:1 ratio.The experimental group received a mixed mode of online and video conference-based training, with online training and assessment, while the control group received traditional offline training.Both groups had training sessions of 3 school hours, twice a week.After 4 weeks of training, both groups received face-to-face video sessions and autonomous training for 2 weeks.Online questionnaires and video assessments were conducted to evaluate the training effects, and the collected data were analyzed and compared.
Results
Both groups achieved a pass in the theory and operational skills assessments, but the experimental group performed significantly better than the control group in terms of assessment scores(

< 0.05).Furthermore, in the satisfaction survey, the experimental group outperformed the control group in terms of objective indicators such as video playback smoothness, as well as subjective indicators such as teacher clarity, course difficulty, course appropriateness, and mastery of course content(

Conclusion
Remote teaching and training mode for the clinical operational skills of primary care physicians, including basic clinical skills, specialized clinical skills, comprehensive clinical skills, and clinical scenario training, can be effectively delivered through 3 school hours of online training.By analyzing and improving the training content and process, primary healthcare staff can selectively learn based on their specific needs, thereby enhancing their professional skills and service quality, and ultimately better serving the people's health.

The Value of Transnasal High Flow Oxygen Therapy in Bronchoscopy Examination of Patients with Respiratory Failure

Objective
To observe the value of transnasal high flow oxygen therapy in bronchoscopy examination of patients with respiratory failure.
Methods
60 patients with respiratory failure for bronchoscopy examination were retrospectively analyzed.The patients were divided into groups according to the different oxygen therapy support in bronchoscopy examination.Among them, 30 patients with conventional flow oxygen support were the control group, and 30 patients with nasal high flow oxygen therapy were the study group.To compare the vital signs of patients in different periods of bronchoscopy examination, the rate of adverse events in the examination process, and the rate of successful completion of bronchoscopy examination.
Results
During the bronchial operation, the oxygen saturation index of the study group was higher than that of the control group, the heart rate and respiratory rate were lower than that of the control group, the rate of adverse events during the operation was lower than that of the control group, and the successful completion rate of bronchoscopy was higher than that of the control group(

0.05).
Conclusion
In the bronchoscopy examination of respiratory failure, nasal high flow oxygen therapy has a positive impact on the stable vital signs of patients, and can improve the examination rate and reduce adverse events.
