1 Effect of Salvianolate on Pulmonary Rehabilitation and Balance of Th1/Th2 Immune Response in Patients with AECOPD

Objective To explore the effect of salvianolate assisted lung rehabilitation in the treatment of acute exacerbation of chronic obstructive pulmonary disease(AECOPD)and its influence on the balance of Th1/Th2 immune response.Methods 80 patients with AECOPD were randomly divided into observation group and control group,Lung rehabilitation therapy and salvianolate assisted lung rehabilitation therapy were selected,and the balance of Th1/Th2 immune response and lung function were compared between the two groups after 2 weeks of treatment,and the improvement time of symptoms,hospitalization time,lung injury rate and mortality rate were evaluated.Results IL-4 and IFN in the observation group were significantly higher than those in the control group-γ、FEV1,FVC and fef75% in the observation group were better than those in the control group(P<0.05),and the hospitalization time in the observation group was shorter than that in the control group(P<0.05),The rate of ventilator-associated lung injury was lower than that of control group(P<0.05).Conclusion The effect of salvianolate assisted lung rehabilitation in patients with AECOPD is significant,which can improve the immune function,enhance the lung function of patients,and improve the treatment effect.

4 Clinical Efficacy and Safety Analysis of Intravenous Thrombolysis and Bridging Stent Thrombectomy in the Treatment of Acute Cerebral Infarction

Objective To investigate the clinical efficacy and safety of intravenous thrombolysis bridging stent thrombectomy in the treatment of acute cerebral infarction.Methods 102 patients with acute cerebral infarction caused by intracranial artery occlusion were selected.The control group received intravenous thrombolysis with recombinant human tissue type plasminogen activator(trade name:alteplase)(RT PA),while the observation group was treated with solitaire AB stent bridging on the basis of RT PA intravenous thrombolysis,The clinical effect,blood vessel recanalization and activities of daily living(ADL),serological indexes and safety of the two groups were analyzed.Results There was no significant difference in NIHSS and Barthel scores between the two groups before treatment(P>0.05);After treatment,the NIHSS score of the observation group was lower than that of the control group,Barthel score was higher(P<0.05);The recanalization rate of the observation group was 70.59%,which was higher than 41.18% of the control group(P<0.05);There was no significant difference in hs CRP,sICAM-1,IGF-1 and NSE between the two groups before treatment(P>0.05);After treatment,the levels of hs CRP,sICAM-1 and NSE in the two groups decreased,while IGF-1 increased to a certain extent.The levels of hs CRP,sICAM-1 and NSE in the observation group were lower than those in the control group,while IGF-1 in the observation group was higher than that in the control group,The difference was statistically significant(P<0.05);There was no significant difference in the total incidence of complications between the two groups(P>0.05),and there was no significant difference in 90d mortality between the two groups(P>0.05).Conclusion Compared with single intravenous thrombolytic therapy,intravenous thrombolytic bridging stent can obtain better clinical efficacy in the treatment of acute cerebral infarction,The reduction of nerve injury is conducive to the recovery of nerve function,and the inflammatory reaction is relieved.The incidence of complications and 90 day mortality is less,so it has high application value.

7 Analysis of Cardiac Function and Serum Inflammatory Factors in Patients with Chronic Heart Failure Treated with AdP Combined with Benazepril

Objective To investigate the effects of injection of MCA combined with Benazepril(BNPH)on cardiac function and serum inflammatory factors in patients with chronic heart failure(CHF).Methods 118 patients with CHFwere retrospectively collected and divided into 2 groups according to different treatment methods,with 59 cases in each group.Group B received benazepril and group A received INJECTION MCA combined with BNPH.Total response rate,left ventricular end diastolic diameter(LVEDD),left ventricular ejection fraction(LVEF),left ventricular end systolic diameter(LVESD),N-terminal brain natriuretic peptide precursor(NT-probNP),inflammatory factors [interleukin-6(IL-6),tumor necrosis factor -α(TNF-α)were compared between the two groups before treatment and 14 days after treatment).Results The total effective rate of group A was 93.22%(55/59)higher than that of group B(77.97%(46/59)(P<0.05).LVESD and LVEDD in 14dA group were lower than those in B group,and LVEF was higher than those in B group(P<0.05).The level of NT-probNP in 14dA group was lower than that in B group(P<0.05).The levels of il-6 and TNF-α in 14dA group were lower than those in B group(P<0.05).Conclusion Injection MCA combined with BNPH has obvious effect on CHF patients,which can effectively relieve inflammatory reaction,improve cardiac function and improve the condition.

10 Effects of Modified Double-hole and Conventional Three-hole Laparoscopic Surgery on Stress Response in Patients with Appendicitis

Objective To explore the effect of modified double-hole and conventional three-hole laparoscopic surgery on stress response in patients with appendicitis.Methods 80 patients with appendicitis were randomly divided into two groups,each with 40 patients.The control group was treated with conventional three-hole laparoscopic appendectomy(LA),while the observation group was treated with modified double-hole LA.Operation-related indexes,stress response(norepinephrine(NE),cortisol(Cor),malondialdehyde(MDA),superoxide dismutase(SOD)and complications were compared between the two groups.Results The amount of intraoperative bleeding in the observation group was less than that in the control group,and the operation time,the first anal exhaust and hospital stay were shorter(P<0.05).On the first day after operation,Cor,NE and MDA in the two groups were higher than that before operation,but the observation group was lower than that in the control group.The SOD in the two groups was lower than that before operation,but the observation group was higher than that in the control group(P<0.05).The incidence of complications in the observation group was lower than that in the control group(P<0.05).Conclusion Compared with conventional three-hole LA,modified double-hole LA has less bleeding during operation,shorter operation time,can reduce the stress reaction of patients with appendicitis,and has fewer complications,which is beneficial to the rehabilitation of patients.

12 Risk Factors for Recurrence of Lumbar Disc Herniation After Percutaneous Transforaminal Endoscopic Treatment

Objective To investigate the risk factors for recurrence of lumbar disc herniation(LDH)after percutaneous transforaminal discectomy(PTED).Methods The research subjects selected 80 patients with lumbar intervertebral disc herniation .All subjects were treated with PTED,and they should be followed up in time within 6 months after the operation.The recurrence of LDH within 6 months after the operation was counted and grouped,and the basic information of all subjects,including gender,age,body mass index(BMI),prominent type,hyperlipidemia,hypertension,diabetes,smoking,and drinking,were recorded and recorded.To analyze the influencing factors of recurrence of LDH after PTED treatment.Results 9 cases of 80 LDH patients relapsed after PTED treatment,the recurrence rate was 11.25%;Univariate analysis showed that the recurrence of LDH patients after PTED treatment was not affected by gender,hyperlipidemia,hypertension and alcohol consumption,and the difference was not statistically significant(P>0.05),but may be affected by age,BMI,type of prominence,diabetes,and smoking,and the difference was statistically significant(P<0.05);Logistic regression analysis found that age,BMI,type of prominence,diabetes,and smoking were all significant Influencing factors of recurrence after PTED treatment in LDH patients(OR>1,P<0.05).Conclusion Patients with older age,high BMI,prominent type,diabetes,and smoking are at higher risk of relapse after PTED treatment,and targeted interventions can be given early to reduce postoperative recurrence.

15 Effects of Remazolam on Serum IL-6,CRP and Postoperative Cognitive Function in Elderly Patients with Painless Gastroenteroscopy

Objective To evaluate the sedative effect and safety of remazolam in outpatient gastroenteroscopy,the dynamic changes of serum IL-6,CRP and MMSE scores were observed to analyze the correlation between the levels of IL-6 and CRP and cognitive dysfunction(PND).Methods 120 elderly patients were randomly divided into propofol group and remazolam group.Remazolam group was given remazolam toluenesulfonic 5mg intravenous injection,an additional dose of 2.5mg/ time;Propofol group was given 1% propofol 1.5~2.0mg/kg intravenously with an additional dose of 0.2~0.5mg/kg.Blood pressure and heart rate were recorded before and after anesthesia,and MMSE score,IL-6 and CRP levels in peripheral blood were observed at different time periods before and after anesthesia.Results Compared with before administration,MAP and HR in each period after administration were significantly decreased in both groups,with statistical significance(P<0.05).Compared with the propofol group,MAP and HR decreased significantly at the beginning of operation,5min after operation and at the end of examination in the remazolam group(P<0.05),but blood pressure fluctuation was basically within the normal range.Compared with the propofol group at the same time period,MMSE,IL-6 and CRP scores of rimazolam group had no significant changes before anesthesia and 6 h after examination(P>0.05).Compared with the propofol group,the cases of nausea,vomiting and dizziness in the remazolam group were significantly reduced,and there was no injection pain,the difference was statistically significant.Conclusions Compared with propofol,relmazolam benesulfonate has little effect on hemodynamics and fewer adverse reactions in elderly patients undergoing painless gastroenteroscopy,and does not increase the occurrence of postoperative cognitive dysfunction and the changes of peripheral inflammatory factors IL-6 and CRP levels.

18 Observation of Curative Effect of Arthroscopic Internal Fixation inThe Treatment of Complex Tibial Plateau Fractures

Objective To observe the efficacy of arthroscopic internal fixation in the treatment of complex tibial plateau fractures.Methods A total of 78 patients with complex tibial plateau fractures who were treatedwere selected as the research objects,and divided into the observation group(n=39)and the control group(n=39)according to random number table method.The control group received traditional open reduction and internal fixation(ORIF),and the observation group received knee arthroscopic internal fixation.The differences in surgical indicators(incision length,blood loss,postoperative drainage,operation time and ambulation time)between the two groups were compared.The differences in knee function(HSS),fracture healing effect,and complication rates were compared between the two groups before surgery and 3 months after surgery.Results The incision length,blood loss,postoperative drainage volume,operation time and ambulation time in the observation group were significantly shorter than those in the control group(P<0.05).At 3 months after operation,the HSS score of the two groups was significantly higher than that before operation,and at the same time,the HSS score of the observation group was significantly higher than that of the control group(P<0.05).The incidence of complications in the observation group was significantly lower than that in the control group(P<0.05).Conclusions Arthroscopic internal fixation has the advantages of small surgical incision and fewer complications,which can promote the recovery of knee joint function in patients with complex tibial fractures and contribute to prognosis.

21 Study On the Diagnostic Value of Multi-slice Spiral CT Post-Processing Combined with Magnetic Resonance Imaging Multi-sequence in the Diagnosis of Stress Fractures

Objective To investigate the diagnostic value of multi-slice spiral CT post-processing combined with magnetic resonance imaging multi-sequence in the diagnosis of stress fractures.Methods A total of 76 patients with stress fractures were selected,and the patients were examined by multi-slice spiral CT and magnetic resonance imaging,and the imaging characteristics of multi-slice spiral CT and magnetic resonance imaging were observed.The characteristics ofstress injury were classified,andthe sensitivity,specificity,accuracy,positive predictive value and negative predictive value of multi-slice spiral CT examination and magnetic resonance imaging examination alone and in combination were compared in diagnosing stress fractures.Results The sensitivity,specificity and accuracy of multi-slice spiral CT were 59.21%,67.11% and 69.74% in detecting stress fractures,while the sensitivity,specificity and accuracy of MRI in detecting stress fractures were 76.32%,71.05% and 78.95%,the sensitivity,specificity,and accuracy of the two combined detection of stress fractures were 90.79%,81.58%and 92.11%.The diagnostic efficiency of the combined examination was higher than that of the single examination(P<0.05).Multi-slice spiral CT detected the joint cavity.Effusion,bone marrow edema,articular cartilage damage,meniscus damage,and ligament damage were not detected.The detection rate of joint lesions by the combined examination was significantly higher than that by single examination(P<0.05).Conclusion Multi-slice spiral CT post-processing combined with MRI multi-sequence has higher diagnostic performance for stress fractures,and has a higher detection rate for joint lesions.

24 The Expression of ING4 and Hypoxia-inducibleFactor-1 α in Cervical Lesions and Its Relationship with HR-HPV Infection

Objective To analyze the expression of growth family member 4 (ING4) and hypoxia-inducible factor-1α (HIF-1α) in cervical lesions, and its relationship with high-risk HPV (HR-HPV) infection.Methods 25 cervical cancer tissuesamples(cervical cancer group),50 cervical intraepithelial neoplasia(CIN)tissue samples(CIN1 group,CIN2-3 group)and 20 normal cervical tissue samples(normal group)were collected.HR-HPV was detected before surgery.The protein expression levels and correlation of ING4 and HIF-1α in each group,and their relationship with pathological parameters were analyzed.Results The HR-HPV infection rate was higher incervical cancer group,CIN1 group and CIN2-3 group than in the normal group(P<0.05).The ING4 positive rate was the highest,and the HIF-1α positive rate was the lowest in the normal group.The HIF-1α positive rate was the highest in the cervical cancer group(P<0.05).In HR-HPV infected tissues,the ING4 positive rate was the lowest,and the HIF-1α positive rate was the highest in the cervical cancer group(P<0.05).There was a negative correlation between ING4 and HIF-1α levels in HR-HPV infected cervical cancer and CIN2-3 tissues(P<0.05).Age,lymph node metastasis,vascular invasion,and the depth of myometrial invasion were independent risk factors for ING4 negative and HIF-1α positive inpatients infected with HR-HPV(P<0.05).Conclusion The low expression of ING4 and high expression of HIF-1a in HR-HPV infected cervical lesions inhibit the proliferation and migration of vascular endothelial cells,thereby delaying the progression of cervical lesions.

27 Significance of Combined Detection of Serum Carcinoembryonic Antigen and CA125 in Cervical Cancer and its Relationship with High Risk Factors

Objective The aim of this study was to investigate the significance of serum carcinoembryonic antigen(CEA)and carbohydrate antigen 125(CA125)in cervical cancer and its relationship with high risk factors,and to provide reference for the clinical diagnosis of cervical cancer.Methods 110 cases of cervical cancer patients were selected as the observation group,and 100 healthy women were selected as the control group in the same period.The venous blood was taken from the fasting morning and the serum was collected by centrifugation.The CEA and CA125 of the two groups was measured by the electrochemical luminescence method.The accuracy,sensitivity and specificity of CEA combined with CA125 in the diagnosis of cervical cancer were analyzed by comparing the positive rate of serum CEA and CA125 in the observation group according to the clinical stage.In addition,Test(HC-II)was used to determine human papillomavirus(HPV)infection in both groups.Results The levels of serum CA125 and CEA in the patients with cervical cancer were(43.29±16.82)U/mL and(25.19±21.36)ng/mL respectively,which were significantly higher than those in the control group.The positive rate of CEA and CA125 in cervical cancer patients(P<0.05).The difference of CEA and CA125 between the two groups was statistically significant(P<0.05),and the stage Ⅲ and Ⅳ were significantly higher than those of stage Ⅰ and stage Ⅱ.The accuracy and sensitivity of CEA combined with CA125 in the diagnosis of cervical cancer were significantly higher than those of CEA or CA125 alone(P<0.05).There was no significant difference in the specificity of CEA,CA125 or CEA combined with CA125 in the diagnosis of cervical cancer(P>0.05).The positive rate of HPV infection in patients with cervical cancer was 100%,which was significantly higher than that in healthy controls(P<0.05).Patients with HPV positive infection and serum CEA and CA125 content of the same trend,the relationship is closely related.Conclusion EA combined with CA125 has a good sensitivity,accuracy and specificity in the diagnosis of cervical cancer,and is highly correlated with HPV infection in patients with high clinical value.

30 To Investigate the Value of Esketamine in the Postoperative Rehabilitation of Shoulder Arthroscopy in Ultrasound-guided Selective Cervical Nerve Root Block

Objective To explore the value of esmketamine in ultrasound-guided selective cervical nerve root block for rehabilitation after shoulder arthroscopy.Methods 80 patients who underwent shoulder arthroscopic surgery were selected as the research objects.They were randomly divided into control group(n=40)and observation group(n=40).Both groups underwent selective cervical nerve root block under ultrasound guidance.The control group was given normal saline intravenous infusion before block,and the observation group was given esmketamine intravenous infusion before block.3h,6h,12h Visual analogue pain(VAS)score and motor nerve block(MBS)score at 24 hours,and hemodynamic changes before operation,5 minutes after operation,15 minutes after operation and 30 minutes after extubation.Results The VAS score of the observation group was significantly lower than that of the control group at 6h and 12h after operation(P<0.05),the MBS score of the observation group was significantly higher than that of the control group at 3H and 6h after operation(P<0.05),and the map and HR levels of the observation group were significantly lower than those of the control group at 5 min,15 min and 30 min after extubation(P<0.05).Conclusion The effect of selective cervical nerve root block with esmketamine under ultrasound guidance is accurate,which can effectively maintain the stability of intraoperative hemodynamics,have good postoperative analgesic effect,and quickly restore the motor function of forearm.

33 Study on Pathogenic Bacteria and Infection Risk Factors in 115 Patients with Suppurative Keratitis

Objective To analyze the pathogenic bacteria and risk factors of infection in patients with purulent keratitis.Methods A total of 115 patients with purulent keratitis were selected,their clinical data were collected,pathogenic examinations,drug resistance analysis,and risk factors for infection were analyzed.Results The main pathogenic bacteria in 115 patients with purulent keratitis were gram-positive bacteria.Among the main gram-positive bacteria,Staphylococcus epidermidis was highly resistant to ampicillin,piperacillin and erythromycin,and streptococcus hemolyticus was resistant to ampicillin.Pseudomonas aeruginosa,which is the main gram-negative bacteria,is highly resistant to ampicillin and erythromycin.Multi-factor analysis found that the history of eye surgery,wearing contact lenses,Ocular surface disease,use of antibacterial drugs,eye trauma or corneal foreign body,TC≤4.31mmol/L,WBC>6.67×109/L,ALB≤39.21g/L,Hb≤134.87g/L,CRP≥1.18mg/L All are risk factors for infection in patients with purulent keratitis(P<0.05).Conclusion History of eye surgery,wearing contact lenses,ocular surface diseases,use of antibiotics,ocular trauma or corneal foreign bodies are risk factors for patients with purulent keratitis;the main pathogenic bacteria in patients with purulent keratitis are gram-positive bacteria,And the bacteria have the problem of multi-drug resistance,the clinic should choose sensitive drugs based on the results of drug susceptibility tests to reduce the migration of bacteria.

36 Effects of Capsular Bag Tension Ring Implantation Combined with Phacoemulsification on Anterior Chamber Depth and Angle size in Patients with Cataract

Objective To investigate the effect of capsular bag tension ring implantation combined with phacoemulsification on anterior chamber depth and angle size in patients with cataract.Methods A total of 92 cataract patients were selected and divided into two groups by random number table method,with 46 cases in each group.The control group was treated with cataract phacoemulsification,and the observation group was treated with capsular bag tension ring implantation on the basis of the control group.The patients were followed up for 3 months.The improvement of anterior chamber depth,anterior chamber angle,uncorrected visual acuity and the incidence of complications were compared between the two groups before operation and 3 months after operation.Results At 3 months after operation,the anterior chamber depth,chamber angle and uncorrected visual acuity of the two groups were all increased compared with those before operation,and the observation group was higher than the control group(P<0.05).There was no significant difference in the incidence of complications between the two groups.(P>0.05).Conclusion Capsular bag tension ring implantation combined with phacoemulsification can improve the anterior chamber depth,angle and visual acuity of cataract with high safety.

38 Effect of Colostrum Oral Immunotherapy Combined with Probiotics on Gastrointestinal Function and Intestinal Flora in Premature Infants

Objective To observe the effect of colostrum oral immunotherapy combined with probiotics on gastrointestinal function and intestinal flora in premature infants.Methods 86 premature infantswere randomly divided into 2 groups with 43 cases in each group.The control group received routine intervention plus probiotics,and the observation group received colostrum oral immunotherapy.The effect and safety were observed,and the concentrations of Bifidobacterium,Lactobacillus,secretory immunoglobulin A(sIgA)and immunoglobulin M(IgM)were observed after 2 days,1 week and 2 weeks of intervention.Results The recovery time of the observation group was relatively short,and the growth rate of body mass was higher than that of the control group(P<0.05),2.3,3%,18.60%,P<0.05)and(P<0.05)and SIgA and IgM after 1 week and 2 weeks,(P<0.05).Conclusion Colostrum oral immunotherapy combined with probiotics intervention in preterm infants can regulate the intestinal flora,enhance their immunity,improve their gastrointestinal function,and reduce the occurrence of adverse events.

Objective To investigate the predictive value of composite index alveolar arterial oxygen difference(A-aDO2),arterial alveolar oxygen partial pressure ratio(a/A)ratio and oxygenation index(PaO2/FiO2)in predicting respiratory distress with respiratory failure in late preterm infants value research.Methods According to the simple random number table method,80 cases of late preterm infants with respiratory distress syndrome were selected,40 cases did not require mechanical ventilation as the control group,and 40 cases required mechanical ventilation as the observation group.To analyze the prediction of respiratory failure parameters on the need for mechanical ventilation in late preterm infants with respiratory distress,the composite index A-aDO2,the difference between a/A ratio and PaO2/FiO2,the sensitivity,specificity,positive predictive value,negative predictive value,Positive rate,negative rate,Youden index and make ROC curve.Results There were no differences in the AUC curves of the maximum A-aDO2,the minimum a/A ratio,and the minimum PaO2/FiO2 between the two groups(P>0.05).The three AUC curves were higher than that of PaCO2,and the A-aDO2 curve exceeded the maximum FiO2 curve(P<0.05).A-aDO2>200 mmHg was proved to be the most effective index for predicting respiratory failure,and all children in the observation group reached the standard of A-aDO2>200 mmHg.Conclusion The composite index A-aDO2,a/A ratio and PaO2/FiO2 can reasonably predict respiratory distress with respiratory failure in late preterm infants,and the prediction effect is obvious,which has a certain research value.

43 Correlation Between Serum HB-EGF and GDF15 Levels and Prognosis of Ovarian Cancer Patients undergoing Surgical Treatment

Objective To investigate the correlation between serum levels of heparin-binding epidermal growth factor(HB-EGF)and growth differentiation factor 15(GDF15)and the prognosis of patients with ovarian cancer undergoing surgical treatment.Methods A prospective randomized trial method was used to select 90 patients with ovarian cancer who received surgical treatment as the research objects.All the selected patients received serum HB-EGF and GDF15 levels before surgery,followed up for 3 years,observed the prognosis of patients,and analyzed the correlation between serum HB-EGF and GDF15 levels and the prognosis of patients with ovarian cancer surgically treated.Results Among the 90 ovarian cancer patients,14 had poor prognosis,accounting for 15.56%;76 had good prognosis,accounting for 84.44%;the serum HB-EGF and GDF15 levels in the poor prognosis group were higher than those in the good prognosis group,and the difference was statistically significant(P<0.05).general bivariate Pearson correlation analysis found that serum HB-EGF and GDF15 levels were positively correlated with the occurrence of poor prognosis(r>0,P<0.05);ROC curve was drawn,the results showed that serum HB-EGF and GDF15 levels predicted The AUCs of poor prognosis were all>0.8,which had a certain predictive value.Conclusion The levels of serum HB-EGF and GDF15 in patients with ovarian cancer are closely related to the prognosis after surgical treatment.In the future,the detection of serum HB-EGF and GDF15 levels can be used to evaluate the prognosis of patients with ovarian cancer and provide a reference for early prevention and treatment.

46 Anesthesia Effect and Resuscitation Quality of Remimazolam Combined with Sufentanil on Patients Undergoing Hysteroscopy

Objective To explore anesthesia effect and resuscitation quality of remimazolam combined with sufentanil on patients undergoing hysteroscopy.Methods 118 cases undergoing hysteroscopywere included as the research subjects,and they were divided into remimazolam group and propofol group according to the random number table,with 59 cases in each group.The patients in the remimazolam group were treated with remimazolam combined with sufentanil,while those in the propofol group were treated with propofol combined with sufentanil.The mean arterial pressure,heart rate,Oxygen saturation and respiratory rate of the two groups at different time points were compared.The induction time,operation time,awakening time and recovery time,the grade of anesthesia efficacy,the scores of BCS and anesthesia satisfaction in each group were recorded.The adverse reactions in each group were counted.Results Compared with T0,the MAP,RR at T1,T2 and T3 in two groups were decreased(P<0.05),The HR of propofol group at T1,T2 and T3 showed a downward trend(P<0.05),and the HR of remimazolam group at T1 was mild increased(P<0.05),There was a transient decrease in SpO2 at T1 and T2 in two groups(P<0.05).Compared with the propofol group,the MAP,HR and RR of remimazolam group at T1,T2 and T3decreased less,the differences were statistically significant(P<0.05),There was no significant difference in SpO2 at each time point between the two groups(P>0.05).There was no significant difference in the effect of therapy,BCS and anesthesia satisfaction score between the two groups(P>0.05).Compared with the remimazolam group,the incidence of injection pain and gloss optosis was significant lower than in the propofol group,less jaw thrust and assisted ventilation were required,the differences were statistically significant(P<0.05).One case of intraoperative awareness was reported in the propofol group.Conclusion Remimazolam combined with sufentanilis safe and effective on patients undergoing hysteroscopy,with more stable vital signs and fewer adverse reactions,and can significantly improvethe quality of resuscitation,the satisfaction with anesthesia is high,it is worthy of promotion.

49 Effects of Transumbilical Single-port Laparoscopic Surgery on Microinflammatory Indexes and Stress Response in Children with Appendicitis after Operation

Objective To explore the effects of transumbilical single-port laparoscopic surgery on microinflammatory indexes and stress response in children with appendicitis after surgery.Methods A total of 80 children with appendicitis were selected and divided into two groups with 40 cases in each group according to random number table method.The control group was treated with conventional three-hole laparoscopic appendectomy(LA),and the observation group was treated with transumbilical single-hole LA.Surgery-related indicators,microinflammatory indicators [C-reactive protein(CRP),interleukin-6(IL-6),tumor necrosis factor-alpha(TNF-alpha)],and stress responses [norepinephrine(NE),cortisol(Cor)] were compared between the two groups.Results There was no significant difference in operation time between the two groups(P>0.05).The blood loss in the observation group was less and the anal exhaust time was shorter than that in the control group(P<0.05).1 day after surgery,CRP,TNF-alpha and IL-6 in both groups were reduced compared with those before surgery,and the observation group was lower than the control group(P<0.05).One day after surgery,Cor and NE were higher in both groups,but lower in observation group than in control group(P<0.05).Conclusion Compared with conventional three-hole LA,transumbilical single-hole LA can reduce the amount of blood loss in children with appendicitis,shorten the time of anal exhaust,and reduce the microinflammatory reaction and stress response.

129 Effect of Buyanghuanwu Decoction Combined with Acupuncture on Sequela of Cerebral Hemorrhage

Objective To analyze the effect of Buyanghuanwu decoction combined with acupuncture in the treatment of sequelae of cerebral hemorrhage.Methods 80 patients with sequela of cerebral hemorrhage were selected in our hospital as the research object,and the clinical statistics from August 2021 to September 2022 were taken as the research subject.Subjects were divided into control group(2021.08—2022.02)and experimental group(2022.03—2022.09)according to the order of admission time,and 40 patients in each group received different treatment interventions.Control group:routine western medicine treatment,experimental group:Buyanghuanwu decoction combined with acupuncture treatment,comparing TCM symptom score,NIHSS score,motor function,daily living ability,total effective rate,quality of life and incidence of adverse reactions.Results Before treatment,there was no difference compared with the scores of symptoms of traditional Chinese medicine,namely dizziness,hemiparalysis and dysphoria,P>0.05.After treatment,the scores of dizziness,hemiparalysis and dysphoria in the experimental group were lower than those in the control group,P<0.05.The NIHSS scores of the two groups were compared,that is,the NIHSS scores were compared at 1 week,4 weeks and 12 weeks after treatment,and the experimental group was lower than the control group,P<0.05;The motor function of the two groups was compared,that is,the FMA tool was used to evaluate.

132 Effect of Jiawei Zhenwu Decoction on renal fibrosis and renal function in patients with chronic kidney disease of spleen-kidney yang deficiency

Objective To analyze the effect of Jiawei Zhenwu decoction on renal fibrosis and renal function in patients with chronic kidney disease(CKD) stage 2-3 of spleen-kidney yang deficiency type.Methods 76 patients with chronic kidney disease(CKD) stage 2-3 of spleen-kidney yang deficiency type were selected and randomly divided into a control group(n=38) and a test group(n=38).The control group was treated with conventional western medicine,and the test group was treated with Jiawei Zhenwu Decoction on the basis of the control group.The curative effect,TCM syndrome score,renal fibrosis[hyaluronic acid(HA),collagen Ⅳ(Col Ⅳ)] and renal function [blood urea nitrogen(BUN),creatinine(SCR)] of the two groups were compared.Results The total effective rate of the test group94.74%(36/38) was higher than that of the control group(71.05%(27/38)(P<0.05);Compared with the control group,the TCM syndrome score of the test group after treatmentwas lower(P<0.05);Compared with the control group,the levels of HA and Col - Ⅳ in the test group after treatment were lower(P<0.05);Compared with the control group,the levels of BUN and SCR in the test group after treatment were lower(P<0.05).Conclusion The application of Jiawei Zhenwu decoction in the treatment of chronic kidney disease with spleen-kidney yang deficiency type can significantly improve the therapeutic effect,relieve clinical symptoms,inhibit renal fibrosis,improve renal function,and has outstanding clinical application value.
